Articulation and Speech Intelligibility after Glossectomy: A Pilot Study / 舌切除術後的構音及說話清晰度之初步研究

Introduction: The human tongue plays a vital role in speech production and glossectomy is known to reduce the lingual mass which might affect speech outcome. This study, therefore, aims to investigate the articulation error pattern after glossectomy. More specifically, to examine whether there was a significance difference in the percentage of correct (POC) in articulation and speech intelligibility between glossectomy with non-reconstruction and reconstruction.
Methods: 20 patients who had undergone glossectomy were recruited in this study. Patients were divided into group 1 (non-reconstruction) and group 2 (reconstruction). Speech samples including consonant, vowel, word, sentence and conversation were collected and analyzed before surgery and 3 months after surgery. The scores for articulation and speech intelligibility were rated by one speech pathologist and six graduate speech pathology students. The correlation between the POC in articulation and speech intelligibility was also examined.
Results: It was noted that manner substitution was the most frequently occurred articulation error pattern after surgery. The POC in consonant articulation and intelligibility was the worst performed for both groups. In addition, the POC in word and sentence articulation were also reduced significantly, while the POC in vowel articulation and intelligibility were least affected. In comparison between the two groups, group 2 had significantly poorer performances for the POC in consonant, vowel, word, sentence, and conversation articulation as well as intelligibility. The POC in consonant, vowel, word, sentence, and conversation articulation and intelligibility were also highly correlated with one another after surgery.
Conclusions: The most common articulation error was manner substitution and the POC in consonant articulation and intelligibility were the worst after glossectomy with both the non-reconstruction and reconstruction groups. In sum, the POC in consonant, vowel, word, sentence, and conversation articulation and intelligibility was significantly poorer in patients after glossectomy with reconstruction.
