碩士 / 中國醫藥大學 / 公共衛生學系碩士班 / 100 / Background: Shipbreaking was a kind of high polluted work. Shipbreaking was closed in Taiwan. We wanted to evaluated the cancer frequency experience of 4303 ship-breaking worker in Taiwan.
Method: A total of 4303 ship-breaking workers who registered members of Kaohsiung Ship-breaking Workers Union in 1985 and jointed the national health insurance were included in the study. The control group was matched with age, gender and living area. Four job groups were created according to their work duty and exposure. The cancer incidence was compared between ship-breaking workers and control group. The different job cancer incidence odd ratio was calculated after adjusted with gender and age.
Result: Compared to control group, ship-breaking worker had more cancer frequency and short life span (p-value<0.001). All cancer frequency is higher in ship-breaking worker (OR 1.65; 95% CI 1.31 to 2.08). Brachial and lung cancer (OR 1.9; 95% CI 1.06 to 3.42), oral and pharyngeal cancer (OR 3.22; 95% CI 1.77 to 5.87) were higher in study group. The longer work duration in ship-breaking, the cancer incidence were increased. The incidence of brachial and lung cancer increased in the group of supervisor and other (OR3.26; 95%CI 1.05 to 10.1) and flame cutters (OR2.27; 95% CI 1.12 to 4.58). Oral and pharyngeal cancer were increased in flame cutter (OR 3.09; 95% CI 1.6 to 5.96), lifter (OR 3.91; 95% CI 1.71 to 8.95) and supervisor and other(OR 6.8. 95% CI 2.33 to 19.81).
Conclusion: Ship-breaking worker had a small but significant increased all cancer incidence, such as brachial and lung cancer and oral and pharyngeal cancer. Those may be related to asbestos and some heavy metal exposure.
