Clinical efficacy of acupuncture as an adjunct to methadone treatment services for heroin addicts / 針刺合併美沙冬替代療法對海洛因成癮者之臨床效益

This study explored the clinical benefit of combining acupuncture and methadone maintenance therapy for the treatment of heroin addiction.
Methods:
Ninety subjects were recruited and thirty subjects who received only methadone maintenance therapy were designated as the control group. The other sixty subjects were randomly divided into a true acupuncture group and a sham acupuncture group. Inclusion criteria were: fulfillment of DSM IV criteria for opiate dependence, reception of methadone maintenance therapy for more than one month, and age older than 20. Acupuncture was given at three acupuncture points (Hegu, Zusanli and auricular shenmen point) twice a week for 4 weeks. The primary outcome measure was change in Short Form Health Survey (SF-36) scores that evaluated quality of life and the daily consumption of methadone. Secondary outcome measures were change in the Pittsburgh Sleep Quality Index (PSQI) scores which evaluated sleep quality, change in the visual analog scale scores that evaluated heroin craving and severity of constipation, and data about heart rate variability.
Results:
There were no differences in demographic data such as age, education, consumption of methadone, or history of heroin or amphetamine use. After treatment, subjects in the true acupuncture group showed significant improvements in role limitations due to emotional health and general health on the SF-36 health survey, significant decreases in the daily consumption of methadone in the 2-4 weeks after acupuncture treatment, and significant improvements in subjective sleep quality, sleep latency, daytime dysfunctions and PSQI total score. The scores for heroin craving and severity of constipation were all significantly improved in both the true acupuncture group and sham acupuncture group. There were no significant differences in heart rate variability in the three groups.
Conclusions:
Our study showed that when acupuncture was provided as an adjunct, standard methadone maintenance therapy had greater clinical efficacy in terms of improving quality of life, decreasing the daily consumption of methadone, improving the quality of sleep, decreasing the craving for heroin and the severity constipation. Further studies with more subjects are needed.
