Supply chain modeling has become crucial as companies face more complex globalization and increasingly higher customer expectations. Also, integrated logistics model and environmental awareness have attracted considerable interest from both the practitioners and academics. Due to the uncertain environments, a body of researchers have used fuzzy set to handle uncertain demands and external raw material problems. Business communities have increasingly faced more risks recently. Now in the supply chain, many real cases are closely related to a supplier-originated risk. This research defines some supply chain risk, which will be quantified as the multiplication of corresponding probability of occurrence of the risk factors and the corresponding cost of occurrence of the risk factors. The objective of this research is to propose a fuzzy multiple objective integrated logistics model in a green supply chain considering risk factors and to discuss the maximizing profit of both manufacturing and reverse chain in a fuzzy environment. Finally, numerical example study was conducted to illustrate the results of the proposed model. The results of this example illustrate a series of parameter values to make this model the total profit maximization. Furthermore, if there is a real case, then those values from the real case can be applied to the proposed model.
