The Autonomic Function in Alzheimer's Disease / 阿茲海默症患者的自律神經系統功能

population is becoming older. In Taiwan, the signs are even more evident, according to
the report of Administration Yuan in Taiwan. Elder population results in new problems
and Alzheimer's disease is one of them. Alzheimer's disease is the most common type of
degenerative dementia. It mainly causes loss of memory and in turn induced difficulties
in behavior and care giving. Incidents of elderly getting lost is often reported in the
news and many of them should attribute to Alzheimer's disease. Symptoms of
Alzheimer's disease also involves autonomic system. Due to the less extent of
involvement, autonomic dysfunction gained less spotlight in previous studies in
Alzheimer's disease. However, dizziness, incontinence, general malaise, palpitation and
even syncope due to postural hypotension could be debilitating or even lethal.
Our study intended to evaluate the autonomic system in patients with Alzheimer's
disease through heart rate variability and questionnaire on quality of life. We studied on
patients with Alzheimer's disease with an outpatient base. We included 40 patients and
30 control matched with age, gender and education. We recorded clinical symptoms and
arranged test on questionnaire and 24 hours of continuous electrocardiography
recording. We analysis the data with Microsoft Excel 2007 and SPSS 19.0.
The result showed that patients with Alzheimer's disease had difference in
autonomic function compared with normal control. The difference was larger with lower
MMSE. However, only parasympathetic indicators showed this relationship. In quality
of life, patients with Alzheimer's disease had lower scores compared with normal
control. The difference was larger with lower MMSE. Most difference was focused on
mental and social aspect. The usage of acetylcholinesterase inhibitor showed no
correlation with all indicators of autonomic dysfunction.
