Incidence, Survival Rate and Complications in Patients with Beta-Thalassemia Major in Taiwan: Nationwide Survey from 2007-2011. / 重度乙型地中海型貧血患者之發生率、存活率與併發症之探討:2007-2011全國性調查

碩士 / 弘光科技大學 / 護理研究所 / 103 / Background: β-Thalassemia major is a genetically inheritable blood disorder and patients withβ-Thalassemia major needed to treat with transfusion and deferoxamine lifelong. However, little is known about the incidence, survival rate and complications ofβ-thalassemia major in Taiwan. It raised a motivation for researchers to investigate this study.
Purpose: The incidence, survival rate and complications ofβ-thalassemia major in Taiwan were analyzed. In the meanwhile, we compared the difference between groups of demographic variables, such as age, gender and complications of patients with thalassemia major during 2007-2011 in Taiwan.
Methods: Retrospective secondary data analysis method was used to analyze the characteristics of population withβ-Thalassemia major by using National Health Insurance (NHI) database. Besides, the Kaplan-Meier method and log-rank test were used to explore the survival rate.
Results: From 2007 to 2011, patients with β-Thalassemia major were reviewed in our study. There are 414 patients withβ-Thalassemia major (male 47.36% versus female 52.64%) in Taiwan. The incidence gradually declined and the survival rate increased by year to year from 97.12% to 99.34%. The mortality rate of birth cohort before 1995 is higher than the population born after 1995 (10.4% decreased to 5.7%, p = .24). In addition, hypogonadism (23.1%), diabetes (21.2%) and heart failure (18.9%) were observed as the most common complications of patients withβ-Thalassemia major in this study.
Conclusion: Even though it showed that the incidence gradually declined and the survival rate increased in this study, they still suffered from complications of disease. We suggest that the family, medical staff, and government will keep concern to the health issue of improving the quality of life for the patients withβ-Thalassemia major.
