Association between Nickel Exposure and Metabolic Syndrome in Shipyard Workers / 造船廠作業人員鎳暴露與代謝性症候群之關係

碩士 / 國防醫學院 / 公共衛生學研究所 / 103 / Shipyard workers will expose to various types of hazardous compounds during their work, especially heavy metals from welding, and grinding processes. Fume contain different composition of metal such as nickel has been classified as Group 1(carcinogenic to human) by international agency for research on cancer(IARC). Evidence from animal study showed nickel exposure has effects on metabolic and inflammatory parameters.

We assessed the relations between exposure to nickel and metabolic syndrome and indicator of Insulin Resistance (IR) among exposed and office-based workers.

The study population, including 109 Taiwanese male shipward workers and a reference group of office workers. We used personal samplers to collect 8 hours PM2.5 exposure samples during their work. The study population underwent an interview and a health examination on the next day morning, where blood and urine samples were collected. Waist circumference, triglyceride, high density lipoprotein, systolic pressure, diastolic pressure, fasting glucose are measured as indicators of MS. HOMA-IR, McAuley index were computed measured as indicator of IR.

This study showed urinary nickel concentration in exposed group is higher than that of reference group (exposed:10.89 μg/L reference:8.47μg/L). Using multivariate-linear regression, after adjusting confounder, HOMA-IR is not significantly associated with urinary nickel level, urinary nickel increases per loge (one-unit), McAuley index decreases 0.86 (β=-0.86 95%CI:-1.67, -0.05). Using logistic regression, after adjusting confounder, when urinary nickel increases per loge (one-unit) , the odds ratio of metabolic syndrome will increase 14.57 times(OR=14.57 95% CI:1.00, 211.71).

In conclusion, Nickel exposure causes enhancement of HOMA-IR and reduction of McAuley index presenting insulin resistance increases. Nickel exposure increases odds ratio in HOMA-IR, McAuley index and MS.
