Pilot Trials of Advanced Long-Term Care Assessment Suite / 國外先進長期照護需求評估工具之試用

碩士 / 國立臺北護理健康大學 / 長期照護研究所 / 103 / Taiwan has already stepped into an aging society, but only focused on multi-fragmented assessment tools derived from domains ADL, IADL, MMSE…., rather than an integrated long-term care(LTC) suite widely used in advanced countries. Currently, there still no unique LTC needs or care plan in Taiwan, which unfavorable for resource positioning and integrating. Hence, to organize a national LTC assessment suite is in urgency in Taiwan to face LTC scheme establishment. Therefore, this study be aimed to compare the available suites of advanced countries, such as Germany, Japan and Korea in order to accelerate the development of the counterpart national LTC assessment suite for policy. We had translated those assessment suites into Madarin/Chinese based on our expertise experiences from the academic learning, and had a pilot trial at the LTC facility for evaluation in northern Taiwan.
The result illustrates that the NBA (Neues Begutachtungsassessment) from Germany integrates the concept of the A & P and focused on the independency of the individuals; The Kaigo Care-Needs Certification Scale (Kaigo-NCS) from Japan has the more questions for IADLs. The manual of the Kaigo-NCS illustrates the definition of every question in detail, which could evaluate rapidly. In addition, the "special issues" must be taken into account to supplement the LTC needs and stay in personal "care plan" in some way. The Korean LTC Needs Certification Scale (Korean LTC-NCS) established has been referred to the experiences from Germany and Japan, extended from medical care axis to cover the related insurance. And the suites from Germany and Japan be close, but not for the suite from Korea.
We hope policy suite would be accelerated to be formed just under the evidence-based approaches from advanced experiences. Besides, incorporating special issues from the care manager, to cope with the LTC needs & well-met. Furthermore, the limited resource be well allocated and matched, to be the bases for the development on tele-care and in the clouds.
