碩士 / 國立雲林科技大學 / 工業工程與管理系 / 104 / The success of Less Than Container Load (LCL) in cold chain transportation is achieved with the breakthrough of cold chain transportation technology in the recent years. However due to the inherent risk of mixed cargo and higher complexity in LCL cold chain transportation, a number of forwarders tend to take smaller steps when approaching cold chain LCL market. In order to provide a guide to the industry to identify and minimize the risk, This study aims to use FMEA (Failure Mode and Effects Analysis)’s HFMEA to evaluate the potential risk causes which is risk priority number (RPN) in the process of export operation in Freight Forwarder. Then, risk matrix would arrange the priority of risk priority number (RPN). And, decision tree may decide the failure mode of RPN should be improved or not. Next, FTA (Failure Tree Analyze) would point out root causes of the failure mode and develops some ameliorative plans. Finally, a significant reduction of the risk priority number (RPN) was obtained when applying FMEA to the process of Refeer container consolidation in Freight Forwarder. Hopefully, using these methods could solve practical problems an reduce operational risk, as the index for future research.
