Abstract
Major administrative and practical changes at both the national and the local level took place in psychiatry in the 1990s. In Northern Ostrobothnia in Northern Finland, specialised psychiatric care was integrated as part of the health care district in 1988. Three separate psychiatric hospitals – Heikinharju Hospital, Oulunsuu Hospital and the Department of Psychiatry in Oulu University Hospital – were merged in 1990. The purpose of this case study is to describe and assess the history and current status of the new amalgamated Department of Psychiatry in Oulu University Hospital. Change is here approached from the viewpoints of the operating environment, the history preceding the change and the actors operative in the process of change.
The report is divided into threes section: history, assessment and instrument testing. The historical section describes the operation of the three independent hospitals before their amalgamation in 1990. The assessment section aims to evaluate the change and overall situation of the Department of Psychiatry in Oulu University Hospital based on data collected in 1993–1997 and also to analyse the change in terms of public control and legislation. In the third section, the theoretical structure of the instrument is evaluated.
Both published and unpublished sources were used in the historical analysis. The data were further supplemented by interviewing medical professionals, patients and relatives in the three hospitals. Data for the assessment survey were collected using a questionnaire, which included an instrument. The instrument consisted of 20 questions evaluating the status of the key subfunctions of the care organisation. Five groups of informants – staff, patients, relatives, line management and partners – responded to the inquiries arranged in 1993, 1995 and 1997. The material comprised a total of 2,176 responses. Frequency and percentage distributions, variation and mean values and cross-tabulation were used to outline the results. The responses to open-ended questions were analysed with methods of content analysis. The theoretical structure of the instrument was evaluated with explorative and confirmatory factor analysis.
The individual histories of the three hospitals were visible in the operation of the amalgamated clinic of psychiatry. The impacts of the merger were manifested as tensions between the old operating cultures in practical routines, management, professional identity and tasks. The simultaneous major upheavals: the merger of two psychiatric hospitals, the close-down of one hospital and the discharge of nearly a thousand psychiatric inpatients into community care altered the operating environment and the unit's relationships with health care centres. The changes in the operating system were first felt to be contradictory, and the rapid change caused feelings of uncertainty. The overall status of the Department of Psychiatry was considered quite satisfactory, however, and its operation improved during the follow-up period. An analysis of the instrument indicated that the basic subfunctions of a care organisation are care, atmosphere and management.
The Department of Psychiatry in Oulu University Hospital survived the drastic change relatively well. The final stage resembled a "top organisation", which typically has a self-orienting staff and awareness of the results combined with high-standard competence. The established professional identity of the staff in the Department of Psychiatry and their goal awareness in implementing the change were reflected in the competent work with the patients, the active development of care and the anticipation of changes. / Tiivistelmä
Psykiatriassa tapahtui suuria hallinnollisia ja sisällöllisiä muutoksia 1990-luvulla sekä valtakunnallisesti että paikallisesti. Pohjois-Pohjanmaalla alueellinen psykiatrinen erikoissairaanhoito integroitiin osaksi sairaanhoitopiiriä vuonna 1988. Kolme erillään toiminutta psykiatrista sairaalaa - Heikinharjun sairaala, Oulunsuun sairaala ja Oulun yliopistollisen keskussairaalan psykiatrian klinikka - yhdistettiin vuonna 1990. Tässä tapaustutkimuksessa kuvataan ja selitetään syntyneen Oulun yliopistollisen sairaalan psykiatrian klinikan historiaa ja nykytilaa. Muutosta tarkastellaan toimintaympäristön, muutosta edeltäneen historian sekä toimintaan osallistuneiden näkökulmasta.
Tutkimus jakautuu historialliseen osaan, arviointiosaan ja mittarin testausosaan. Historiallisessa osassa kuvataan kolmen erillään toimineen sairaalan toimintaa ennen niiden yhdistymistä vuonna 1990. Arviointiosan tavoitteena oli arvioida Oulun yliopistollisen sairaalan psykiatrian klinikan muutosta ja kokonaistilannetta vuosina 1993 - 1997 kerätyn aineiston avulla, samoin tavoitteena on tarkastella muutosta julkisten ohjausjärjestelmien sekä lainsäädännön näkökulmasta. Mittarin testausosassa tutkittiin mittarin teoreettista rakennetta.
Historiaosassa käytettiin painettuja ja painamattomia lähteitä. Tietoja täydennettiin haastattelemalla eri sairaaloissa toimineita hoidon ammattilaisia, potilaita ja omaisia. Arviointitutkimuksen tiedot kerättiin mittarin sisältävällä kyselylomakkeella. Mittari muodostui hoito-organisaation keskeisten osatoimintojen tilaa arvioivasta 20 kysymyksestä. Viisi ryhmää - henkilökunta, potilaat, omaiset, linjajohto ja yhteistyökumppanit - vastasivat vuosina 1993, 1995 ja 1997 järjestettyihin kyselyihin. Aineisto käsittää kaikkiaan 2 176 vastausta. Tuloksia kuvataan frekvenssi- ja prosenttijakaumina, hajonta- ja keskilukuina sekä ristiintaulukoinneilla. Avovastaukset analysoitiin sisällönerittelyllä. Mittarin teoreettista rakennetta tutkittiin eksploratiivisella ja konfirmatorisella faktorianalyysillä.
Eri sairaaloiden historiat näkyivät yhdistetyn psykiatrian klinikan toiminnassa. Yhdistämisen vaikutukset ilmenivät toimintakulttuurien välisinä jännitteinä toimintatavoissa, johtamisessa, ammatillisessa identiteetissä ja tehtävissä. Samanaikainen kahden psykiatrisen sairaalan yhdistäminen ja yhden lakkauttaminen sekä lähes tuhannen sairaalapotilaan siirtyminen avohoitoon muuttivat perustehtävää, toimintaympäristöä ja suhteita terveyskeskuksiin. Aluksi ympäristö- ja toimintarakenteiden muutokset koettiin varsin ristiriitaisina, ja nopea muutos herätti epävarmuutta. Psykiatrian klinikan tila arvioitiin kokonaisuudessaan varsin tyydyttäväksi ja toiminta parani seuranta-aikana. Käytetyn mittarin analysointi osoitti, että hoito-organisaation osatoiminnat ovat pelkistettävissä hoitamiseen, ilmapiiriin ja johtamiseen.
Oulun yliopistollisen sairaalan psykiatrian klinikka selvisi 1990-luvun suuresta muutoksesta kohtalaisen hyvin. Toiminnan lopputila muistutti osin niin sanottua huippuorganisaatiota, jonka tunnusmerkkejä ovat henkilöstön itseohjautuvuus ja korkeatasoiseen osaamiseen yhdistynyt tulostietoisuus. Psykiatrian klinikan työntekijöiden vakiintunut ammatti-identiteetti ja päämäärätietoinen muutoksen toteutus näkyivät hyvin toteutuneessa potilastyössä, toiminnan aktiivisessa kehittämisessä ja muutosten ennakoinnissa. Silti psykiatriaan kohdistuneet taloudelliset leikkaukset ja muut suuret muutokset näkyivät epävarmuutena ja tulevaisuuden pelkona.
