Facial Emotion Recognition In Children With Asperger's Disorder And In Children With Social Phobia

Recognizing emotion from facial expressions is an essential skill for effective social functioning and establishing interpersonal relationships. Asperger's Disorder (AD) and Social Phobia (SP) are two clinical populations showing impairment in social skill and perhaps emotion recognition. Objectives: The primary objectives were to determine the uniqueness of facial emotion recognition abilities between children with AD and SP relative to typically developing children (TD) and to examine the role of expression intensity in determining recognition of facial affect. Method: Fifty-seven children (19 AD, 17 SP, and 21 TD) aged 7-13 years participated in the study. Reaction times and accuracy were measured as children identified neutral faces and faces displaying anger, disgust, fear, happiness, and sadness at two different intensity levels. Results: Mixed model ANOVAs with group and emotion type revealed that all children responded faster and more accurately to expressions of happiness, but there were no other group differences. Additional analyses indicated that intensity of the displayed emotion influenced facial affect detection ability for several basic emotions (happiness, fear, and anger). Across groups, there was no pattern of specific misidentification of emotion (e.g., children did not consistently misidentify one emotion, such as disgust, for a different emotion, such as anger.) Finally, facial affect recognition abilities were not associated with behavioral ratings of overall anxiety or social skills effectiveness in structured role play interactions. Conclusions: Distinct facial affect recognition deficits in the clinical groups emerge when the intensity of the emotion expression is considered. Implications for using behavioral assessments to delineate the relationship between facial affect recognition abilities and social functioning among clinical populations are discussed.

The development of posttraumatic stress disorder (PTSD) and severe weather phobia (SWP) symptoms in relation to weather-related trauma was examined. Participants were college students (N = 815) enrolled at Mississippi State University. Findings suggest that distinct factors (specifically, disorder-specific cognitions and anxiety sensitivity) contribute to the development of PTSD and in the onset of SWP symptoms following exposure to weather-related trauma. A weather trauma model taking into account disorder-specific cognitive vulnerabilities, previous exposure to weather-related trauma, and levels of anxiety sensitivity is suggested to provide an explanation for these differences. The results suggest that prevention and treatment efforts should address learned fear response, aim at reducing anxiety sensitivity, and target disorder-specific cognitions.

Peer victimization among youth with anxiety disorders

Objective: This study examined whether overt and relational peer victimization were associated with the severity of Social Phobia (SoP) symptoms and whether frequent victimization was more common among youth with SoP as compared to youth with other anxiety disorders. In addition, the study examined whether self-esteem, peer beliefs, and emotional lability were linked to internalizing symptoms above and beyond overt and relational victimization severity. Method: Participants were 90 youth (47 boys, 43 girls; M age = 11.06 years; SD = 3.09) and their parents. Youth had been referred to an outpatient child and adolescent anxiety disorders clinic. Measures included (a) a semi-structured diagnostic interview, (b) youth self-report forms assessing peer victimization, anxiety, depressive symptoms, loneliness, and global self-worth, and (c) parent-report forms assessing anxiety and emotion regulation. Results: Results showed a concurrent positive association between peer victimization and self-reported social anxiety, with relational victimization providing unique information above and beyond overt victimization. Peer victimization was not associated with a specific diagnosis, but was related to multiple internalizing problems (negative beliefs about the peer group accounted for some of this relation). Conclusions: Peer victimization is important to assess for and consider in the treatment of childhood and adolescent anxiety disorders. Peer victimization is associated with social anxiety symptoms, and relational victimization, in particular, is associated with internalizing problems among youth with anxiety disorders. Victimization appears to be associated with symptomatology rather than diagnosis. / Psychology

Threat-related attentional bias in adolescents with social phobia

The present study compared attentional disengagement from threat-related stimuli in socially phobic (SP) and non-anxiety-disordered (NAD) adolescents. The associations between trait anxiety and state anxiety and attentional bias in SP adolescents were assessed. Furthermore, the present study compared the attentional control abilities of SP and NAD adolescents. Twenty-eight SP participants aged 12-17 and 27 NAD controls, matched on age and IQ, were administered a computer task to measure attentional disengagement from threat-related words. Participants completed the State-Trait Anxiety Inventory and subtests of the Test of Everyday Attention for Children (TEA-ch). Mixed ANOVA analyses indicated that SP and NAD adolescents did not differ in their disengagement from threat-related stimuli. Correlational analyses indicated that state anxiety was associated with disengagement from threat, but only when SP participants with comorbid ADHD were excluded from analyses. Trait anxiety was not significantly associated with attentional disengagement from threat. Finally, SP participants performed more poorly than NAD participants on the TEA-ch subtests, indicating poorer attentional control in SP participants. These results suggest that SP adolescents experience a deficit in executive attentional skills. The clinical implications of these findings are discussed. / Psychology

Correlation of Dental Anxiety, Dental Phobia, and Psychological Constructs in a Sample of Patients Receiving Dental Care at Temple University Kornberg School of Dentistry

Introduction: Dental anxiety, fear, and phobia are common factors that prevent individuals from seeking dental care by delaying dental care or terminating the visit entirely, which may cause a decline in oral health-related quality of life. Dental anxiety often has a correlation with other psychological constructs. There is a significant prevalence of dental anxiety among various ethnic groups, which highlights the importance of understanding and addressing these issues to improve oral health outcomes and access to care.Aim: To examine the associations between psychological constructs, dental anxiety, and phobia, as well as the associations between key demographics and dental anxiety and phobia among patients seeking dental care at TUKSoD. Methods: Baseline data from 499 patients who participated in a randomized clinical trial that evaluated the efficacy of an online intervention in managing dental anxiety (National Institute of Health U01DE027328) was used. Subjects completed a semi-structured interview according to the Diagnostic Schedule Manual-IV (DSM-IV) criteria and self-reported measures on dental anxiety (Modified Dental Anxiety Scale (MDAS)), fear or avoidance of dental care, sensitivity to pain (PSI), ability to tolerate distress (DTS), blood-injection-injury (BII) phobia, and other psychological factors. Bivariate analysis was done using paired sample t-tests and analysis of variance to evaluate differences in mean scores for MDAS by sex, race, gender, and ethnicity. Multivariable Regression Models were used to explore the association between MDAS, selected demographic variables and psychological constructs while controlling for potential confounders, likewise for clinical severity rating (CSR). These analyses were performed using R 4.3.2 and statistical significance was set at two-sided p-value < 0.05. Results: Mean age of subjects was 48.9±14.7 years old, most were female (71.6%), non-Hispanic (88.6%) with an income lower than $30,000 (40.0%) and most had completed a high school diploma /GED (26%). The majority (63.3%) reported high dental anxiety (MDAS≥19) and 64.57 % met criteria for specific phobia (CSR≥4), with mean scores of 19.53 ± 3.62 and 4.49 ± 1.69 respectively. Mean scores for FQBII, PSI, DTS, and ASI were 15.51 ± 10.15, 68.02±22.9, 44.73±13.2, and 31.0 ± 16.87 respectively. Significant differences in Clinical Severity Rating were observed by age (p< 0.01), sex (p = 0.02), and race (p<0.01), while the psychological constructs FQBII, DTS and PSI varied significantly across age, race, and ethnicity (p<0.05). The Hispanic group had clinical severity ratings that were approximately 10.1% higher than the non-Hispanic group. This was not statistically significant (p=0.07). Conclusion: Dental anxiety scores were higher among African American low-income women compared to Caucasians. Age, sex, income, and race demonstrated to have a strong association with Dental anxiety. A Quantitative Neural Biomarker for Rejection Estimation: A Neuroeconomic Approach for Evaluating Theory of Mind

The clinical presentation of social phobia suggests that alterations in theory of mind (TOM) may play a systematic role in the development and maintenance of the disorder. In the current study we leverage a quantitative neuroeconmic approach to probe for neural and behavior markers of cognitive TOM, as well as rejection estimation, with a particular focus on social phobia. Participants comprised a non-clinical sample that was divided into low (N = 10) and high (N = 7) social anxiety groups based on self-report. Participants completed a one-sided uncertainty ultimatum game designed to probe individual differences in cognitive TOM, as well as rejection estimation. Contrary to predictions, there were no behavioral differences between high and low social anxiety groups in terms of rejection estimation. Although no between-group differences emerged in the traditional TOM network, significant differences were observed in subregions of the striatum during formulation of offers, likely corresponding to estimation of reward expectations. As hypothesized, and consistent with past research, imaging results support the existence of a network regions implicated in TOM, including the medial prefrontal cortex (MPFC) and the temporal parietal junction (TPJ). In addition to these regions, additional areas, including the caudate and insula, were also active during mentalizing components of the task. Collectively, results suggest a novel role for expected-value computations in the development and maintenance of social phobia. / Master of Science

Comorbid ADHD: Implications for Cognitive-Behavioral Therapy of Youth with a Specific Phobia

Objective: Although findings have been mixed, accumulating evidence suggests that co-occurring attention-deficit/hyperactivity disorder (ADHD) diagnoses and symptoms negatively predict cognitive-behavioral therapy (CBT) outcomes for anxious youth. The current study extends past research by examining the association of not only ADHD but also other features of ADHD with treatment outcomes of youth who received an intensive CBT for a specific phobia. Method: 135 youth (ages 6-15; 52.2% female; 88.2% white) were randomized to either an individual or parent-augmented intensive CBT targeting a specific phobia. Latent growth curve models were used to explore the association of ADHD symptoms, effortful control, sluggish cognitive tempo, maternal depression and the two treatment conditions (i.e., individual versus parent-augmented) with pre-treatment severity of the specific phobia and the trajectory of change in the severity of the specific phobia from pre-treatment to the 6-month follow up after the intervention. Results: As expected, higher levels of ADHD symptoms were associated with lower levels of effortful control and increased maternal depression at pre-treatment. Contrary to expectations, ADHD symptoms and its associated difficulties were not significantly associated with treatment outcomes. Conclusion: Overall, the findings lend support to the generalizability of intensive CBT for a specific phobia to youth with comorbid ADHD and associated difficulties. This thesis examines futōkō (school non-attendance) in Japan from the perspective that futōkō is a social movement. It analyses citizens’ activism in support of futōkō students over the twenty year period from 1984. Drawing upon social movement approaches the thesis examines how futōkō citizens successfully grasped political opportunities, established a network of organizations, launched a new interpretive frame for futōkō, and challenged the dominant representation of futōkō in society –that 'futōkō is an illness’. To explore in detail the ideological aspect of the futōkō movement’s framing, a content analysis of 140 editorials in the movement newspaper – the Futōkō Shimbun (School Non-attendance Newspaper) was conducted. Commencing with a critique of schooling practices that create futōkō, over the survey period Futōkō Shimbun expands its analysis to develop a critical appraisal of Japanese society that has broad implications for many different aspects of the everyday life of its citizens. Adopting the typology of movement outcomes, this thesis assesses the outcomes of futōkō movement framing through two sets of factors: first, changes in government policies and attitudes and second, cultural and ideational changes as experienced by the movement organizations, futōkō activists and children, and as represented in shifts in media representation of futōkō. Government policies in the handling of futōkō students have become more flexible although not always positive, while government interactions with the movement are suggestive of recognition that futōkō citizens are valid spokespersons with a legitimate set of interests in relation to futōkō children. Futōkō movement activists have been successful in framing their movement in terms of: expanding and increasingly sophisticated networks of futōkō movement organizations; the professional development of activists; and a more positive media discourse. The thesis concludes that the futōkō movement has influenced government policies, the media and wider social commentary about the phenomenon of and responses to futōkō.
