O processo de geração de laudos em radiologia pode ser dinamizado e melhorado com a utilização de sistemas eletrônicos baseados em tecnologias de informação e comunicação que podem trazer benefícios como o aumento de produtividade e redução de tempo e custo. Esta pesquisa comparou tempos de geração e digitação de laudos entre um sistema eletrônico, que possibilitou ao médico radiologista gravar seus laudos gerados por voz em formato digital e o sistema tradicional no qual o radiologista escreve o laudo a mão. Para realização da pesquisa foi necessário modelar e construir o sistema eletrônico proposto para fins de comparação com o tradicional já existente. Por meio de formulários, radiologistas e digitadores anotaram os tempos de geração e digitação dos laudos nos dois sistemas. Comparadas as médias entre eles, o sistema eletrônico apresentou redução de 20% (p=0,0410) do tempo médio de geração do laudo em comparação com o sistema tradicional. Por outro lado, o sistema tradicional foi mais eficiente em relação ao tempo de digitação já que a média de tempo do sistema eletrônico foi três vezes maior (p<0,0001). / The patient medical record is extremely important in medicine. The radiology report generation process can be improved using electronic models based in communication and information technologies that can improve productivity, reduce time and cost. This research compares generation and transcription times of the radiology report between a radiology information system where the radiologist can record radiology reports by voice in digital format and the traditional system in which the radiologist writes the radiology reoport by hands. To conduct the study was necessary to model and construct the electronic system for comparison with the existing traditional system. Using forms, radiologists and transcriptionists register the generation and transcription times in both systems. Comparign the averages, the electronic system reduced 20% (p=0,0410) the generation average time of radiology report compared with traditional system. Moreover, the traditional system was more efficient in relation to transcription time whereas the average time of eletronic system was three times bigger (p<0,0001). / TEDE

The effective accuracy of dental records in forensic dental identification in Sudan

Magister Scientiae Dentium - MSc(Dent) / In Sudan there are currently no programmes targeting dentists to improve their own knowledge about how they can be part of human identification by keeping good dental records. In addition, no guidelines are stated by the health authorities about making, keeping and retention of dental records. The aim of this research was to assess the accuracy of dental records drawn up by the general dentists in Sudan with regard to forensic dental identification and compare these records to an ideal dental record. Dental records of 180 patients obtained from six sites (major dental sectors) were reviewed and compared with an ideal dental record. The data was captured in Excel and statistically analyzed. The results showed that two third of the dentists do not undertake full tooth charting prior to treatment and sometimes this is not shown in their dental records; The dentist name who examined and treated the patient was clearly mentioned in 55.6% of the dental records examined. Dentists in Sudan do not request many radiographs but they depend mainly on intra oral periapical views (PV) and Orthopantomographs (OPG) with a fair to good quality. The medical history was recorded in 44% of the total number of records examined. The quality of dental records in this study was poor in general dental practices but was fair in governmental hospitals. An integrated education programmes to increase the awareness of the dentists in Sudan about accurate record keeping is recommended. Clear guide lines from the health authorities for dental recording system should be developed.

M.Cur. (Intensive Care Nursing) / The nurse working in an intensive care unit is legally accountable for complete and accurate record-keeping. Record-keeping is especially important during a crisis incident as the sequential management or treatment depends on what happened before and during the crisis incident. Before she can be held accountable for complete and accurate record-keeping, the nurse must have the necessary abilities (knowledge, skills and values) . She will have to accept the responsibility and legally she will receive the authority for it. Complete and accurate record-keeping helps to maintain, improve and restore the critical patient's health. In this way the nurse working in an intensive care unit facilitates the patient's aim for wholeness and at the same time achieves her own aim for' quality nursing. The purpose of this research is accurate nursing records are kept intensive care unit. A contextual-descriptive research design, including a survey method and retrospective auditing were used to determine the completeness and accuracy of record-keeping by nurses during a crisis incident. Auditing was done on the basis of three structured control lists. The results showed that the graphic-symbolic report was more complete and accurate with regard to the patients' particulars and graphic accuracy. The results also showed that there is a great variance in the knowledge and skills of the nurses working in the intensive care unit, regarding record-keeping. Recommendations evolving from this study are: The purpose and use of the records should be clarified. The record system must be revised. Temporary staff must be accompanied on day duty by permanent staff members before they start working night duty. The allocation of the staff must be revised. A policy for record-keeping must be established. In-service training for expanding knowledge regarding record- keeping must be initiated. The purpose and the use of record-keeping must be included in the orientation program. Records must be evaluated to identify shortcomings.

Canadian archives and the corporate memory : a case of amnesia?

For some years North American archivists have argued that public archival repositories are unable to bear the burden of preserving the records of modern business corporations and have suggested that firms should shoulder this burden themselves by establishing corporate archives. This suggestion, however, fails to address the issue of preserving the records of small businesses, and the records of defunct firms, that are unable to support a corporate archives. Before one can predict the effect the advocates of corporate archives will have on the preservation of business records, one needs to know what degree of responsibility repositories have accepted for preserving business records. Unfortunately, little information has been published concerning the business records holdings of Canadian archival institutions. A questionnaire asking for information on acquisitions policies and strategies, and on the characteristics of business records collections, was therefore sent to federal, provincial, municipal, and university archives. The business records holdings of the Public Archives of Canada, the Provincial Archives of British Columbia, the City of Vancouver Archives, and the Special Collections Division of the University of British Columbia Library were examined in detail to augment the information obtained from questionnaire returns. The value of the business records collections held by Canadian repositories was further examined by means of a reference analysis of journal articles and monographs, published by economic and business historians, which measured the use made of business records collections by these historians. Although some substantial collections of business records have been preserved, it appears that over three quarters of the business records collections preserved by Canadian repositores are so small and fragmentary in nature that they are of dubious value to researchers. This impression is borne out by the sparse use of business records as sources by business and economic historians. The experiences of European and American archival institutions indicate that Canadian repositories will have to accumulate a significant body of business records collections before they will see increased use of their business records holdings. Repositories cannot reasonably be expected to devote their limited resources to collecting the records of large integrated corporations. Nonetheless, archival institutions have a responsibility to ensure the preservation of records documenting the Canadian economy. Archival institutions, and the archivists they employ, should continue their efforts to persuade companies to set up corporate archives. Many firms, however, are unwilling or unable to take such a step. Canadian repositories must therefore continue to acquire business records if they wish to fulfil their mandates of preserving comprehensive collections documenting the various aspects of the history of their respective regions. This must be done in an active and systematic manner, using such strategies as public relations programs and the provision to firms of assistance in scheduling records. The use of evaluation in the design and development of interactive medical record systems

An explorative study was done to develop an evaluation methodology. This method can be applied during the development of interactive medical record systems in order to provide information which can be used to improve user interaction with the system. Th e evaluation methodology consists of a number of interactive sessions with potential users of the interactive medical record system. During the first two sessions the subjects are trained to use the system. During the third and last session the subjects are videotaped while they are doing a set of benchmark tasks on the system under evaluation. The video recordings are analysed to obtain performance data. This performance data consists of task timings and a list of problems experienced (errors made) by the subjects. The systems evaluated during the study were a problem-oriented manual medical record and an interactive computerized medical record. The computerized record system was specifically developed for this study. The design and subsequent improvements to this system are documented in the study.
