Introduction. The HIV prevalence among men who have sex with men (MSM) in China keeps increasing sharply. A high proportion of the MSM in China have male regular sex partner (RP) and prevalence of unprotected anal intercourse (UAI) involving such RP is higher than when non-RP is involved. Trust, intimacy and cognitive factors are the factors associated with UAI with RP. Several cross-sectional studies have been demonstrated the important factors associated with UAI with RP among MSM in China. However, no study about intervention for MSM-RP is found to be conducted. To reduce UAI with RP, an intervention tailored to RP is urgently developed and identified its efficacy. / Objectives. The study aimed to describe the prevalence of UAI, as well as of which associated factors among MSM-RP in Beijing and Chengdu, China, and to evaluate the efficacy of an enhanced HIV voluntary counseling and testing (VCT) in increasing condom use with RP among MSMRP in China by a randomized controlled trial (RCT). / Subjects and Methods. A cross-sectional survey and a randomized controlled trial have been conducted. For the cross-sectional survey, total 307 HIV negative MSM who have RP have been recruited by three ways. Face to face interview has been conducted to participants. Based on the associated factors found in the cross-sectional survey, interventions including video, education leaflets and enhanced counseling contents have been tailored to RP among MSMRP. For the randomized controlled trial, total 336 MSMRP have been recruited and randomly assigned 169 subjects to the Intervention Group in which participants have been given enhanced VCT plus an audio-visual and four leaflets components and 167 subjects to the Control Group in which participants have been given only standard-of-care VCT at the baseline. Evaluation was conducted at Month 3 and 6. Statistical methods such as descriptive analyses, Chi-square test and logistic regression were used in this study. / Results. The results have been found were the prevalence of UAI with RP among MSMRP was 52.4%, and the Theory of Planned Behavior (TPB) related cognitions, trust, intimacy, depression and anxiety were associated with UAI with RP among MSMRP. In the RCT study, participants in the Intervention Group had less UAI (36.1% vs. 49.1%) than those of the Control Group at Month 3. / Conclusions. This study showed a high prevalence of UAI among MSMRP, whilst trust, intimacy and cognitive factors were associated with UAI with RP. The efficacy of Enhanced VCT tailored to RP has been identified. The acceptability and feasibility of the tailored intervention were demonstrated. In the future HIV prevention programs, the effective intervention should be considered to be incorporated into standard-of-care VCT procedures and be implemented in the specific population. / Li, Chunrong. / Thesis Ph.D. Chinese University of Hong Kong 2015. / Includes bibliographical references (leaves 149-178). / Abstracts and appendixes also in Chinese. / Title from PDF title page (viewed on 05, October, 2016). / Li, Chunrong. / Detailed summary in vernacular field only. / Detailed summary in vernacular field only. / Detailed summary in vernacular field only. / Detailed summary in vernacular field only.

Attitude Functions and Political Behavior: The Issue of Gay Civil Rights

Young, Josephine 18 June 1992 (has links)
This study explored the relationship between voters' political behavior regarding the issue of gay rights and the underlying psychological needs served by their attitudes on this issue. A telephone survey of 100 randomly selected local voters was conducted, during which Herek's (1987) Attitude Functions Inventory (AFI) was administered. Three of the four attitude function subscales (Experiential- Schematic, Social-Expressive and Ego-Defensive) were confirmed using a LISREL confirmatory factor analysis. The Value-Expressive subscale was not confirmed and showed poor reliability. Pro- and anti-gay rights behavior was measured using a Political Behavior Index (PBI) developed for this study. Regression analyses and MANOVAs were employed to test six hypotheses, all of which received some support. Pro-gay rights behavior was associated with ExperientialSchematic attitudes and a Value-Expressive item regarding civil liberties. Anti-gay rights behavior was associated with Ego-Defensive attitudes and a Value-Expressive item regarding moral beliefs. Men scored higher on the EgoDefensive function than did women. Those who knew more gay people were more likely to exhibit Experiential-Schematic attitudes and were less likely to hold Ego-Defensive attitudes. These findings imply that efforts to change anti-gay rights behavior need to address the underlying motivations of Ego-Defense and Value-Expression regarding the construct of moral beliefs. An additional implication is a potential for increasing support for the legal rights of gay people by increasing association with people one knows to be gay.

The primary health care experiences of gay men in Australia

Sinclair, Andrew James, n/a January 2006 (has links)
The present research, consisting of two studies, was designed to examine the primary health care experiences of gay men in Australia and assess doctors? attitudes and training with regard to gay men and their health care. In the first study, 195 gay men were surveyed regarding their health issues and their primary health care experiences. The most important health concerns of gay men were stress and depression followed by HIV/AIDS, body image disorder and other sexually transmissible infections. Including those participants who were unsure, approximately one-half reported experiencing homophobia and almost one?quarter reported experiencing discrimination in the provision of health care. Despite this, respondents were generally satisfied with their primary health care, although respondents felt that all GPs should receive additional undergraduate medical education regarding gay men?s health. In the second study, 25 doctors (13 gay specialists and 12 non-gay specialists) were surveyed regarding their knowledge of gay men?s health and their comfort working with gay men. Non-gay specialist GPs were less comfortable treating gay men, reported poorer communication and were more homophobic than their gay specialist counterparts. Further, doctors perceived their medical education regarding gay men?s health has been inadequate. Together, the results of the two current studies suggest that disclosure of sexuality is an important issue for both gay men and doctors, and has the potential to impact on the quality of health care that gay men receive. In order to improve the level of disclosure, the pervasiveness of homophobia and discrimination in primary health care must be reduced. Finally, the results indicate that medical education must be updated to reflect current knowledge regarding the health issues of gay men. Failure to address these issues will condemn gay men to continued health inequality.
