Potential Effects On Oral Health And Otitis In 3-Year Old Children Consuming Probiotic Lactobacilli

Background: Probiotic products are used to improve health, cure disease and in a prophylactic aim. However, there is limited research about its general effects on health in children, especially on oral health.  Aim: This study aimed to evaluate potential effects on oral health and otitis in 3-year- old children consuming probiotic Lactobacillus reuteri 17938 and Lactobacillus reuteri 5289.  Methods: This was a quantitative cross-sectional study where questionnaires concerning health and lifestyle of children (n=169) participating in the longitudinal study NorthPop Child, at the age of 4, 9, 18 and 36 months old, were compared to clinical parameters from a visit at the Public Dental Health Care at 3 years of age. Saliva samples from 142 children were analyzed by PCR. Results: This study showed that children consuming Lactobacillus reuteri 17938, had prevalence of the bacteria in saliva. Prevalence of Lactobacillus reuteri 5289 in saliva was not indicated. The prevalence of caries disease tended to be lower in probiotic- consuming children, though not statistically significant. Children consuming probiotics showed a higher prevalence of otitis and consumption of antibiotics compared to non- eaters.  Conclusion: Any conclusions about consumption and frequency required to gain and maintain the bacteria in the oral microbiome could not be made. Contradictory to other studies, this study found that children who consumed probiotics had more occasions of otitis media and antibiotic use. Future studies with a higher number of participants would be of interest to confirm these potential findings.

Educação infantil de 0 a 3 anos: um estudo sobre demanda e qualidade na região de Guaianazes, São Paulo / Early childhood education 0 to 3 years: a study on demand and quality in the region of Guaianazes, São Paulo

A pesquisa realizada na região de Guaianazes, periferia da cidade de São Paulo, teve por objetivo compreender como são implementadas as políticas públicas, aqui as de Educação Infantil, particularmente as dirigidas às crianças de 0 a 3 anos de idade. Para tanto, foram realizadas entrevistas com diretores, professoras, coordenadoras pedagógicas e mães de duas creches da região, denominadas centros de educação infantil, além da diretora regional de educação. Foram ainda realizadas doze horas de observação em cada um dos centros. Um estudo documental completou o conjunto de recursos necessário para a investigação. Como resultado foi verificado que a demanda por vagas é a tônica e seu equacionamento, um poderoso pretexto para a expansão da rede conveniada de creches, seguindo a tendência geral do município. Dessa forma, o estudo indicou que aspectos importantes para a caracterização da educação infantil de qualidade, como espaços físicos ou formação profissional são deixados em segundo plano. As entrevistas, principalmente, evidenciaram que a situação de pobreza da região e uma suposta desestruturação familiar, presente solidamente no discurso da dirigente entrevistada e no dos demais produzem ações para crianças carentes, e não competentes como se reconhece na literatura recente. Os vários documentos federais e municipais apresentam uma visão de educação infantil, que concebem uma criança rica, ativa e produtora de cultura, mas são as condições materiais dessas crianças que definem as concepções dos sujeitos envolvidos na área, bem como a forma como essas concepções adentram na política pública para determinar a sua conformação. / The research conducted in the region of Guaianazes, periphery of São Paulo city, had the objective of understanding how public policies are implemented, here specifically those concerning Early Childhood Education, particularly the ones addressed to children at the ages from 0 to 3 years. Therefore, interviews were conducted with principals, teachers, pedagogical coordinators and mothers from two nurseries schools in the region, called early childhood education centers, besides of the regional director of education. We performed also twelve hours of observation in each of the centers. A documental study completed the set of resources required for the investigation. As a result, it was verified that the demand for places is the major issue and its implementation a powerful reason for the expansion of the associated network of nurseries, following the general trend of the municipality. Thereby, the study indicated that important aspects for characterizing a quality early childhood education, such as physical spaces or professional background, are left out as low priority. The interviews primarily showed that the poverty situation of the region and an alleged family disruption, which are solidly present in the speech of the interviewed the director in that of the other ones interviewed, bring forth actions for needy children and not competent children as it is recognized in the recent literature. The various federal and municipal documents present a vision of early childhood education that conceives a rich, active and producer of culture child, however it is the material conditions of these children that defines the conceptions of the subjects involved in the fild, as well as how these conceptions step into the public policies in order to determine its conformation.
