Life beyond infection : home-based pastoral care to people with HIV-positive status within a context of poverty.

Thesis (DTh (Practical Theology and Missiology))--University of Stellenbosch, 2005. / The basic premise of this study is that the congregation is the key to providing homebased pastoral care support to HIV-positive people in poor contexts. In so doing, the church does not only perform a social function to poor HIV/AIDS-affected families, but it also acts in accordance with the calling of mediating God’s Kingdom (diakonia), thus spreading the gospel, and showing unconditional sacrificial love and compassion. The Church embodies the gospel, which is the instrument of hope and salvation to despairing HIV/AIDS-people in the community.

HIV/AIDS alienation : between prejudice and acceptance

Thesis (DTh (Practical Theology and Missiology))--University of Stellenbosch, 2004. / ENGLISH ABSTRACT: This dissertation aimed to ascertain, in a practical theological way, how to bridge the gulf between the congregation and the AIDS community using home based care as the vehicle of change. The initial hypotheses of the research were based upon a model initially developed by World Vision in Nkhotakota, Malawi. The research question, which the study address, is as follows: 1. Can the negative attitudes, prejudices and behaviours which are held and demonstrated by many in the church towards those suffering with HIV/AIDS, be changed by using deliberate attempts to alter their perspective of this pandemic by providing accurate information, in juxtaposition with the demonstration of Christ’s love and compassion to this community? 2. In conjunction to this first question comes a second: Can the compassionate outreach of the church, as it follows Christ’s mandate to love change the perspective of those in the HIV/AIDS community so that instead of viewing the church (as a whole) as cold and unloving, their perception will change with the demonstration of such love and compassion by its membership that they begin to see the church as a source of hope and love? To describe the contextual situation in which this study takes place, the historical background concerning the church’s response to the issue of the HIV/AIDS pandemic was explored in chapter two. The third chapter discusses the cultural paradigmatic focuses, unique to the African situation, with attention to the role in the church as it seeks to comply with its mandated mission. Chapter four then examines the cultural practices found, specifically in the Malawian context, that promote the spread and transmission of the HIV/AIDS virus amongst the Chewa people and the surrounding tribes. Over and against the identified traditional practices and their interrelationship with the worldviews of the people of Malawi, chapter five focuses on the practical theological implications of the church seeing to find identity in Christ. The ramifications of the praxis process regarding this hermeneutic, in consideration with the response of the Church as it seeks to reflect the character of the God, as represented primarily by the attribute of love towards those who are suffering from being infected or affected by this disease are explored in this chapter. It deals with the theological ramifications concerning the faith community as it represents the body of Christ by providing eschatological hope to this suffering world. The remaining chapters describe the methodology and praxis process utilizing the research hypothesis developed from the Nkhotakota model. Final conclusions were then drawn in order to provide understanding as to how to obediently participate in God’s witness to the world as the faith community addresses the problem of the HIV/AIDS pandemic in very poor areas in Malawi. Although the initial optimistic goals of this research were not met as anticipated, essential discoveries that illuminate the faulty paradigms associated with critical issues such as the effects of abject poverty were exposed. Difficulties that were initially considered secondary to the main thrust of this research, whose complexities are generally misunderstood by the western paradigm came to light as the praxis process unfolded. Attempts to sidestep stark issues such as poverty, in order to address the ‘real’ issues under study served to highlight these problems as their magnitude forced their recognition and consideration. This research has exposed is the necessity for further exploration into the intricate ramifications of issues such poverty by demonstrating the unfortunate fact that for those struggling to survive at the lowest levels of Maslow’s hierarchy, the luxury of benevolent service is simply not possible. For these, there is no other issue but survival. Calling on these sufferers to act as ‘the church’ when their need is so dire is not only unrealistic, it borders on sacrilege. / AFRIKAANSE OPSOMMING: Hierdie tesis beoog om op ‘n prakties-teologiese wyse vas te stel hoe om die kloof tussen ‘n gemeente en ‘n Vigs-gemeenskap te oorbrug met tuisversorging as medium van verandering. Die aanvanklike hipoteses van die navorsing is gebaseer op ‘n model wat oorspronklik deur World Vision by Nkhotakota, Malawi, ontwikkel is. Die navorsingsvraag waarop dié studie gerig word, is die volgende: 1. Kan die negatiewe houding, vooroordele en optrede van baie kerklidmate teenoor MIV/Viglyers verander word deur die gebruik van doelgerigte pogings om hul siening van hierdie pandemie te wysig deur die verskaffing van die korrekte inligting, tesame met blyke van Christus se liefde en medelye aan hierdie gemeenskap? 2. Al is die aanvanklike optimistiese doelwitte van hierdie navorsing deels bereik, is wesenlike ontdekkings gemaak wat die foutiewe paradigmas in verband met kritieke vraagstukke, soos die invloed van volslae armoede, blootgelê. Probleme wat aanvanklik sekondêr beskou is tot die hoofmikpunt van die navorsing het aan die lig gekom in die navorsingsproses. Om vanuit ‘n lewe vol sekuriteite hierdie armes op te roep om op te tree as onbetaalde dienswerkers, terwyl hul eie nood so groot is, is nie net onrealisties nie, maar ongevoelig.

Combating HIV : a ministerial strategy for Zambian churches

Thesis (DTh (Practical Theology and Missiology))--Stellenbosch University, 2008. / ENGLISH ABSTRACT: This work is about a missiological praxis for the creation of 'Circles of Hope' as an entry point for faith based organisations and, particularly, local churches in Zambia for an effective battle in the fight against HIV (Human Immunodeficiency Virus). The HIV pandemic is one of the worst tragedies to have befallen humankind in the 21st century. Lost to many people is the fact that it does not affect all regions of the globe equally. Figures show that over 70% of PLWHA (people living with HIV) are in sub Saharan-Africa while most affected are young and therefore, in theory, energetic. With an overall HIV prevalence rate of 16% and a life expectancy of 34 years, HIV has severely impacted the lives of Zambians across the country. Stigma remains one of the most significant challenges in Zambia across the prevention-to-care continuum. The wider environment of these effects and statistics has provided for us the wider contextual situation. The Church in Zambia and, indeed, in the entire sub-Saharan African region cannot afford to hide its head in the sand anymore. The impact of HIV is being felt at all levels of society. This has posed a threat to economic progress and human development by attacking the most economically productive age group and reversing gains in life expectancy and child survival. The increasing burden on health budgets has stretched national and community resources to the limit, leaving no room for complacency or pretence about the magnitude of the problem. Since some members of the Church are positively infected, we can safely say of the Church: the body of Christ has HIV. HIV is a national disaster. It cannot be managed without mobilising all the sectors within a nation. The Church in Zambia needs to make HIV prevention a matter of compelling priority. The Church is an instrument for the missional praxis of the triune God. Theology in this work is not so much a scientific endeavour that begins and ends with analysing contexts and texts, rather it is an imaginative way of finding new answers to the new situation brought about by the presence of HIV (Hendriks, 2004: 30). In this work the researcher contends that measures are required to develop strong theological reflections and education which would result in the establishment of Circles of Hope in all local congregations. These Circles of Hope would act as a fountain for promoting behavioural change, support action for safer sexual behaviour, and combat stigmatisation and discrimination against people with known HIV infection. There must be appropriate theological reflections that touch on the work of the reign of God. A relevant HIV theology will necessarily involve the laity, will watch out for fundamentalist views, will be biblical in nature and will draw from Trinitarian understanding. The basis of moving from a theology of punishment to that of care, truth, freedom, justice and peace is the theology of community and companionship. This reflection is an attempt to have constructive interpretation of the present realities brought about by a time of HIV. One can only discern God‘s will for the present situation through critical and sensitive leadership in order to bring about genuine healing. The role of the local church and networking activities are essential commodities to realise a vision of a Zambia that is HIV competent. This then is the new ministerial strategy being spearheaded by the 'Circles of Hope' programme of the Council of Churches in Zambia. It is a challenge for Zambian churches. / AFRIKAANSE OPSOMMING: Die navorsing handel oor ʼn missiologiese begronde praksis vir die skepping van 'Kringe van Hoop' as 'n beginpunt vir geloofsgebaseerde organisasies en spesifiek die plaaslike kerk in Zambië in hulle stryd teen MIV (Menslike Immuniteits Virus). Die MIV pandemie is een van die ergste tragedies wat die mensdom in die 21ste eeu getref het. Wat baie mense nie altyd besef nie, is dat dit nie oral in die wêreld dieselfde impak het nie. Statistiek wys daarop dat meer as 70% van alle MIV positiewe mense Suid van die Sahara woon. Die meerderheid van die geïnfekteerdes is jonk met baie potensiaal. 16% van die bevolking in Zambië is MIV positief en het 'n lewensverwagting van 34 jaar. Die uitwerking hiervan raak die land en al sy mense. Naas voorkoming en versorging bly een van die grootste uitdagings wat die gemeenskap in Zambië moet aanspreek stigmatisering. Die navorsing beskou teologie nie as 'n wetenskaplike onderneming wat bloot handel oor 'n analise van kontekste en tekste nie. Die navorsing werk met die hipotese dat die probleem vanuit 'n teologiese hoek benader moet word sodat teologiese beginsels die praktykteorie van Kringe van Hoop in plaaslike gemeentes sal onderskraag. Die studie voorsien dat die Kringe van Hoop die hoof dryfveer sal wees wat gedragsverandering sal bevorder, veiliger seksuele gedrag sal aanmoedig, en die stryd teen die stigmatisering en diskriminasie sal voer. Gepaste teologiese refleksie oor die heerskappy van God is noodsaaklik. 'n Revelante HIV teologie sal gewone lidmate insluit, sal bedag wees op fundamentalistiese sieninge, sal bybels wees en sal gebaseer wees op ʼn trinitariese godsbegrip. Die teologiese basis vir die wegbeweeg van 'n teologie van straf/oordeel na een van versorging, waarheid, vryheid, geregtigheid en vrede, is geleë in gemeenskap en kameraadskap. Hierdie refleksie is 'n poging om in 'n tyd van HIV 'n konstruktiewe interpretasie te gee van die huidige realiteite. Kritiese en sensitiewe leierskap behoort in die huidige situasie te poog om God se wil te soek om die gebrokenheid van 'n MIV siek gemeenskap aan te spreek. Die rol van die plaaslike kerk en netwerkingsaktiwiteite is onontbeerlik vir die realisering van die visie van ʼn Zambië sonder MIV. Die 'Kringe van Hoop'-program van die Zambiese Raad van Kerke is 'n bedieningstrategie wat die MIV pandemie wil aanspreek en wat die kerk in Zambië uitdaag om mee te doen.

Religious assets, health outcomes and HIV/AIDS : a challenge and an opportunity for St. Paul's Anglican Church, Pietermaritzburg.

The study examines the role religious institutions play in responding to health crises in the community with particular focus on mV/AIDS. The thesis argues for the importance of focusing on health outcomes rather than the traditional "economic" models for assessing health sector in time of AIDS crisis. Health outcomes measures: good practice, increases accountability of services, quantifies the values of interventions where traditional research data may be impractical or lacking. It even assists in determining resource allocations and help to monitor and improve standards of care. This opens the door to focus on religious institutions, where selecting and measuring outcomes could be deeply connected to a community's or institution's mission and be able to describe a specific desirable result or quality of institution's services. The role of religious institutions has not been well recognized in dealing with health issues, and in particular within the religious community itself. Religious communities have not recognized their enormous assets, which they could mobilize in an effort to create good health conditions while facing the challenges of the HIV / AIDS pandemic and other diseases. The study examines the involvement of a local Anglican Church, St Paul's in health in the urban context of Pietermartzburg, KwaZulu-Natal. The asset-based approach guides the study in capturing the basic notion that assets carry value and may be used to create greater value. The research findings show that the worshippers of St Paul's Anglican Church seem to have little understanding of their religious asset portfolio, which can be used effectively to improve the health conditions and health prospects of those in need in order to build healthy communities. The study argues that religious congregations and other faith-based organizations can play a vital role in local public health systems and community-based health improvements initiatives. The onset and rapid spread of the HIV/AIDS pandemic in Sub-Saharan Africa has challenged and continues to challenge the church in its doctrine as well as its practical ministries. The Evangelical Lutheran Church in Namibia has been no exception. The disease challenges the theological and pastoral disciplines, especially in the area of contextuality. This thesis is developed at the very site of the struggle to care for the infected and affected individuals and families in the ten Evangelical Lutheran Church parishes in the Oshana Region, Namibia. Healing and caring for the sick is the primary mission of this church. Therefore, the quest of this study is to investigate how ELCIN through pastoral care and counselling helps HIV/AIDS infected and affected family members cope with their situation. The study concentrated mainly on ten ELCIN parishes in the Oshana Region, Namibia. Chapter one is an introduction to the whole thesis. Included is the statement of the problem, the methodology used to collect data and the literature review. Chapter two deals with pastoral care and counselling of HIV/AIDS infected and affected families. The African understanding of heath and illness is also considered as well. Chapter three is about the Church and HIV/AIDS in the Oshana Region, Namibia. This chapter investigates the responses of ELCIN's pastors towards HIV/AIDS affected families in the Oshana Region, and how they understand HIV/AIDS biblically. Chapter four deals with the impact of HIV/AIDS on affected families in Oshana Region, Namibia. This chapter discusses how HIV/AIDS affects the family members, nurses, and community ministers and how pastoral care and counselling help the widowers, widows, orphans, caregivers of orphans and nurses to take care of orphans. This is the main chapter of this thesis. Chapter five is about data analysis, recommendations and research findings using the Christian theoretical framework of Mwaura, van Dyk, Msomi, Snidle and Welsh, and Dube. The dissertation seeks to investigate, examine, and critically analyse the reasons why Pietermaritzburg churches lack gender sensitivity in combating HIV/AIDS. The dissertation's focus is on Springs of Hope Support Group Project (SOH) - a support group that seeks to meet the felt needs amongst the HTV positive people around Pietermaritzburg. Amongst other motivations, the dissertation was undertaken as a contribution to the church in its fight against the spread of HIV/AIDS in South Africa. The methodology that was used involved field and library research as well as observations of other HIV/AIDS support groups. The primary source of this dissertation consists of interviews that were conducted among SOH members, NGOs workers, and Church ministers. Chapter one is an introduction to the whole dissertation and includes an introduction to chapter one, experiences of African women, the story of Ann Ntombela, the background of the study, statements of the problems and motivations, objectives of the study, research hypothesis/promises, significance of the study, the theoretical frameworks, critical reviews of existing literature, research methodology, research ethics, expected results, limitation of the study and a summary and conclusion. Chapter two deals with the negative effects of colonialism and failure of development on African women. Its objective is to unearth the factors behind the deplorable social, political, and economic position of African women before HIV was reported. It seeks to find out why the plight of African women has worsened since the coming of colonialism and the start of development efforts. Chapter three deals with gender. It relates the effects of development failure to the plight of African women. Matters of marginalization, exploitation and oppression of African women are dealt with at length. Chapter four focuses on HIV/AIDS infection, transmission, prevention, cure and treatment. It also tackles the matter of the vulnerability of African women to HIV/AIDS at length by relating chapter two to the realities that facilitates the infection of the virus especially on women. Chapter five deals with the field research and formulation of a gender sensitive approach to combating HIV/AIDS. It also seeks to formulate 'a church based gender sensitive approach' as the way forward in combating the spread of HIV/AIDS amongst African women in Pietermaritzburg. This chapter elaborates on how the church should reposition itself in order to be relevant and effective to women who are HIV positive. Chapter six is the conclusion of this dissertation. HIV and AIDs and its implications for the ministry of healing in some Pentecostal churches in Namibia.

Healing has long been considered part of the Church's pastoral and diaconal calling. For Christians the ministry of healing is grounded in the Word, sacraments and prayer. Based on this understanding some of the Pentecostal churches in Namibia proclaim that HIV and AIDS can be cured through divine healing. It was revealed through interviews with both the practitioners of healing and recipients of healing that the claims of healing HIV and AIDS are not conclusive. It has been established through this study that the implications of a failed healing for the recipients include; lost of faith, hatred towards the practitioners, church, substance abuse and attempts at suicide. A close study of the available literature substantiates the fact that healing of HIV and AIDS in Namibia remains only a claim. The logical conclusion derived was that people confuse healing of HIV and AIDS with spiritual and psychological strength and tranquility that they receive from Pentecostal churches through their healing services. This study recognises the importance of "healing" and "coping" and the need for appropriate theological and psycho-social support for the recipients of divine healing. This study also reveals that HIV and AIDS has forced some Pentecostal churches to revisit their approach of healing. Thus, some of the Pentecostal churches have embarked upon Home Based Care, feeding schemes and material support for PLWHA. The interviews could barely establish a definite case of a successful outcome of healing of someone who was HIV and AIDS positive and was healed through divine intervention. The spread of HIV/AIDS in South Africa has evoked many responses from the national government, Non-Governmental Organizations and the church. There are many reasons why the church should respond to this challenge of HIV/AIDS and join hands with the worldwide effort to provide care and support for people living with HIV/AIDS in our society. As the epidemic increases, many people in South Africa are falling sick, suffering physically, emotionally and spiritually and many are abandoned and desolate. Men, women, young people and children are dying; families and communities are severely affected socially and economically. This thesis examines the response of the Evangelical Lutheran Church in Southern Africa (ELCSA) to AIDS and suggests a more adequate strategy to deal with HIV/AIDS. First, it examines the incidence and· impact of HIV/AIDS, noting the emotional, physical and socio-economic impact of HIV/AIDS. Secondly, this thesis develops a theological response to AIDS. The involvement of ELCSA is examined via research into six parishes in the Umgeni circuit of the South Eastern Diocese (Kwa-Zulu Natal). It argues that a seven-fold framework best describes the sort of strategy needed to fight against the spread of HIV/AIDS. The essential question behind this thesis is: How can we respond to the pastoral needs of Christian Indians in Northdale/Raisethorpe, with regards to the HIV/AIDS pandemic, when there is this silence among those that are infected and affected and the lack of concern from the church ? This topic desires to research the silence among the Christian Indians of Northdale/Raisethorpe to look into the possibility that it is a myth that HIV/AIDS is not a threat to the Christian Indians of Northdale/Raisethorpe. Certain aspects of this problem need to be investigated to prove the myth and to open an avenue for pastoral counselling and care. In this investigation I intend to revisit and open a new dialogue with the clergy to set up combined structures that will alleviate the suffering in the Northdale/Raisethorpe community in regards to the HIV/AIDS pandemic. Untold memories of HIV and AIDS among pastoral agents : the case of the Anglican manyano leaders in the KwaZulu-Natal Midlands, 1990-2010.

Through the oral history approach, this study documents shifts in the pastoral agency of manyano leaders in the HIV and AIDS context of the KwaZulu-Natal Midlands from 1990-2000 and 2000-2010. The two time periods are significant as they mark changes in socio-economic and socio-political influences on HIV and AIDS in South Africa. The model used to investigate the agency of the manyano leaders has been drawn from James Scott’s (1990) theory of power relations between dominant and subordinate groups. Scott’s (1990) theory guided this study in analysing the agency of manyano leaders in two ways. Firstly, the theory guided the study’s analysis of the interviews beyond the superficial level to uncover discourses that in Scott’s (1990) terms operate in the public realm, hidden realm and in the realm of ‘infrapolitics’. Secondly, Scott’s (1990) theory helped uncover some of the shifts in the subversive agency of the manyano leaders’ response to the HIV and AIDS epidemic during the two time periods of 1990-2000 and 2001-2010. In both periods, the manyano leaders’ agency revealed that they opt for just and liberative responses on behalf of those that have been marginalised by the epidemic. The study revealed that in the first period, the agency of the manyano leaders’ faced more resistance in the public realm due to dominant political, religious and social attitudes that fuelled HIV and AIDS-related stigma and denial. As a result, much of their response to challenges raised by the epidemic took place in the hidden realm. In the second period, discourses of HIV and AIDS became more public and as a result, this study has argued, their agency has also become more public.
