Espera-se, com o avanço do campo médico e das tecnologias atuais, que o ser humano, além de superar a expectativa de vida, atinja um envelhecimento bem-sucedido. Deste modo, foi proposto o estudo atual com objetivo de avaliar o envelhecimento bem-sucedido em idosos longevos assistidos pela Estratégia de Saúde da Família, utilizando quatro modelos diferentes, compostos por critérios físicos, psicológicos e sociais. Trata-se de um estudo analítico de corte transversal, cujos dados foram coletados nas Unidades de Saúde da Família da microrregião 4.2 do município do Recife, em Pernambuco. Na caracterização de envelhecimento bem-sucedido considerou-se a ausência de condições crônicas de saúde, independência funcional e cognitiva, engajamento e satisfação com a vida e saúde auto-percebida. Os quatros diferentes modelos propostos foram comparados entre si, com o modelo de Rowe e Kahn e com o envelhecimento bem-sucedido auto-referido. Como fatores associados foram analisados dados sócio-demográficos, morbidades diagnosticadas em prontuário e apoio social. Participaram do estudo 172 idosos longevos, a média de idade foi de 85,51 anos, havendo predominância do sexo feminino (80,2%), de viuvez (69,2%), baixa escolaridade (43,6%) e baixos níveis de renda (76,7%). A doença mais prevalente foi a hipertensão arterial (73,3%) seguida pela diabetes (33,7%) e situação previdenciária mais comum foi a de aposentado (68,0%). A prevalência de envelhecimento bem-sucedido correspondeu a 39%, variando conforme modelos empregados, sendo: modelo 1 (4,1%), modelo 2 (2,9%), modelo 3 (14,0%), modelo 4 (18,0%), segundo o modelo de Rowe e Kahn (8,1%) e envelhecimento bem-sucedido auto-referido (87%). Na avaliação de fatores relacionados ao envelhecimento bem-sucedido, foi observado significância estatística para idade (p=0,032), situação previdenciária (p=0,01639), depressão (p=0,00011), diabetes (p=0,01203) e doenças neurológicas (p=0,00001). Por sua vez, na análise dos modelos isolados, foi encontrado relação estatística para situação previdenciária no modelo 2 (p=0,027), escolaridade no modelo 3 (p=0,0117) e situação conjugal no modelo 4 (p=0,023). A prevalência de envelhecimento bem-sucedido foi expressivamente menor entre os modelos biomédicos, sendo maior quando avaliado sob uma perspectiva focada nos modelos psicossociais e no modelo geral proposto pelo estudo, se aproximando dos elevados índices de EBS auto-referido e de satisfação com a vida encontrados na amostra. Acredita-se, assim, que os modelos que mais se relacionam ao envelhecimento bem-sucedido em idosos longevos são os psicossociais, devendo ser dada ênfase na sua aplicação em pesquisas nesse subgrupo. Espera-se que, a partir desse conhecimento, sejam adotadas medidas que promovam um envelhecimento baseado em medidas de bem-estar com participação social, independência e autonomia. / Faced with the increase in life expectancy and significant increase in the oldest old around the world, there is a need to reflect on the way of how these individuals have reached an older age. It is expected, with the advancement of the medical field and current technologies, the human race, in addition to exceeding the life expectancy, reaches a successful aging. Thus, it was proposed the current study to assess successful aging in the oldest old assisted by the Family Health Strategy, using four different models, composed of physical, psychological and social criteria. This is an analytical cross-sectional study, with data collected in the health units of micro Family 4.2 of the city of Recife, in Pernambuco. The characterization of successful aging was considered the presence of chronic health conditions, cognitive and functional independence, engagement and satisfaction with life and self-perceived health. The four different proposed models were compared with each other, with the model of Rowe and Kahn and with the successful aging self-reported. As associated factors were analyzed socio-demographic data, morbidity diagnosed in medical records and social support. The study included 172 oldest old, the average age was 85.51 years, with a predominance of females (80.2%), widowed (69.2%), low education (43.6%) and low levels income (76.7%). The most prevalent disease was hypertension (73.3%) followed by diabetes (33.7%) and most common social security situation was the retired (68.0%). The prevalence of successful aging was 39%, varying according models employed, as follows: model 1 (4.1%), model 2 (2.9%), model 3 (14.0%), model 4 (18.0%), according to the model of Rowe and Kahn (8.1%) and successful aging self-reported (87%). In evaluating factors related to successful aging, statistical significance was observed for age (p = 0.032), social security situation (p = 0.01639), depression (p = 0.00011), diabetes (p = 0.01203) and neurological diseases (p = 0.00001). In the analysis of separate models, statistical relationship was found for social security situation in the 2 model (p = 0.027), education at 3 model (p = 0.0117) and marital status at 4 model (p = 0.023). The prevalence of successful aging was significantly lower among biomedical models, being higher when assessed from a perspective focused on the psychosocial models and overall model proposed by the study, approaching the high levels of self-reported EBS found in the sample. It is believed therefore that the models most related to successful aging in the oldest old are the psychosocial and should be given emphasis on its application in research in this subgroup. It is expected that from this knowledge, measures are taken in order to promote an aging based on wellness measures with social participation, independence and autonomy.
