The responsibilization of aging under neoliberal health regimes: A case study of Masters athleticism

With amateur athleticism on the rise in Canada, older Masters athletes have been promoted as exemplars of “successful aging” in governmental population health campaigns that encourage all seniors to be physically active. This study investigates the life experiences of a group of ‘successfully aging’ Masters athletes to better situate their circumstances against the backdrop of a discourse of health responsibilization enacted by the state in its efforts to improve the health of aging citizens. Data were obtained from 15 in-depth interviews with Masters athletes age 60 and over. The findings revealed Masters athletes to have had exceptional life-long involvement in athleticism with intense physical training debuting early in adult life with several participants having been high-ranking amateur athletes prior to their involvement in Masters athleticism. Belonging for the most part to a high socioeconomic status, these participants were able to afford the costs associated with participation in high calibre athletic training and events. While these athletes might be held as exemplars of successful aging, they did not perceive themselves as such nor are their lifestyles and athletic achievements typical of the older seniors population that is targeted by state funded population health promotion efforts. This study offers insight into the socially constructed nature of successful aging under neoliberalism. The niche audience targeted people aging with disabilities (or chronic health conditions). Evolving Abilities supported connectedness and full inclusion by addressing ageism and ableism.</p>

Decision Making Among Older Adults with a Limited Prognosis

<p> Decisions near the end of life must be made at a time of great emotional upheaval and rely on knowledge that few people possess outside of healthcare. Complicating this is that choices appear to change as older adults become more ill and approach death. Thus, healthcare providers must help older patients navigate this complexity to make decisions that are consistent with patients' priorities and wishes. Research examining end-of-life decisions has been descriptive in nature, looked at decisions from among younger adults, or examined decision making among those with cancer diagnoses. In addition, the research often focused on prospective decisions among healthy elders. Much less is known about decision-making processes used by older adults with a limited prognosis. </p><p> The aim of this dissertation was to examine decision-making processes among older adults with a limited prognosis who were in the midst of making significant healthcare decisions. The goal was to explore the phenomenon from within the context of the end of life to bring forth the missing voice of these elders. </p><p> Twenty community-based adults (13 men, 7 women, ages 67-97) with a prognosis of less than 12 months were recruited through the San Francisco Veterans' Administration Medical Center and the University of California, San Francisco, Division of Geriatrics. One-on-one interviews were conducted in participants' homes using a semi-structured interview guide. Interviews were recorded and transcribed verbatim. Grounded theory was used to analyze the interviews to allow themes and concepts to emerge organically from the data. </p><p> Two main themes emerged: <i>maintaining a sense of control </i> and <i>decision making in the context of ambiguity.</i> Participants rarely discussed actual decisions and preferred to delegate end-of-life decisions. By using different approaches to express their priorities and goals, they maintained the <i>sense</i> of control without being <i> in</i> control. The actual context of decisions could not be understood a priori, which resulted in uncertainty and ambivalence that influenced how decisions were made. </p><p> By understanding how older adults approach late-life decisions, healthcare providers can better support their patients as they make different choices and help guide patients to choices that are consistent with their stated preferences and priorities.</p>

Fatigue Behavior in Hygrothermally Degraded toughened epoxy Adhesives

A method to measure the mixed-mode fatigue behavior of environmentally degraded adhesive joints was developed. Firstly, the absorption and desorption of water in two different rubber-toughened epoxy adhesives was measured gravimetrically. The water absorption in both adhesives showed anomalous behavior that was fitted to a new “sequential dual Fickian” (SDF) model. The water desorption in both adhesives was modelled accurately using Fick’s law, and there was a significant difference in the amount of retained water after drying in the two adhesives. The effects of long-term aging were studied using open-faced specimens made with two different rubber-toughened epoxy adhesives. The contrasting results illustrated the effects of environmental degradation on the matrix and toughener. Furthermore, the differences in the degradation behavior of both adhesives, combined with gravimetric and dynamic mechanical thermal analysis (DMTA) results, were used to illustrate the role of retained water in degrading the toughening mechanisms. The measured fatigue results invalidated the environmental index (EI) hypothesis for fatigue behavior, at least for the relatively short aging times studied here. Compared with aging under constant humidity, the fatigue performance of joints was found to be superior after aging in a cyclic salt-spray environment due to the lower water concentrations in the adhesive. The effects of test environment humidity and temperature on the fatigue behavior were also studied using closed, un-aged specimens. Both individual and combined effects of temperature and humidity on fatigue behavior were studied. In elevated temperature and humidity environment, joint performance at higher crack growth rates was degraded solely due to the effect of the increased temperature, whereas fatigue performance at low crack growth rates degraded predominantly because of elevated moisture. Finally, to generalise the techniques developed to automotive aluminum sheets, a reinforced specimen was developed that avoids yielding of thin aluminum sheet adherends while loading. Fatigue testing with these reinforced specimens revealed that the fatigue behavior was sensitive to the loading phase angle and the orientation of rolling lines on the sheet. These reinforced specimens were also used to study the effects of long-term aging and the effects of test environment.

Age-related Changes In Emotion Regulation Using A Startle Modulation Paradigm

Lifespan theories of emotion suggest that the ability to regulate emotion improves with age. The supporting evidence, however, is indirect: older adults pay less attention to negative events, remember less negative information, and report fewer experiences of negative emotion. Few studies directly measure emotion regulation by explicitly instructing older adults to modulate their feelings while exposed to emotion-evoking stimuli. The purpose of this thesis was to directly compare younger and older adults in their ability to modulate feelings to investigate whether aging results in decline, stability, or improvement in emotion regulation and also to examine potential mechanisms underlying regulation skills. The study employed a startle modulation paradigm to measure both emotional reactivity and regulation. Two experimental tasks (Stroop colour-word interference, reversal learning) were also administered to explore the relationship between emotion regulation and two theoretically-relevant processes: (a) cognitive control and (b) modification of learned emotional associations. There were three main findings: (1) emotional reactivity was preserved in older adults. Both age groups showed emotion-modulated startle (negative > neutral) during the pre-regulation viewing period; (2) age-related decline in emotion regulation was evident on an objective measure of emotion regulation (startle eyeblink reflex) but not on a subjective measure (self-ratings). Specifically, for older adults, startle eyeblink was not enhanced or attenuated following increase and decrease instructions, respectively. In contrast, both groups showed similar modulation of valence and arousal ratings by regulation instruction (increase > look > decrease); (3) for older adults, reversal learning performance correlated positively with the degree of reappraisal-related startle attenuation in the decrease condition, suggesting a possible mechanism for impaired down-regulation. These findings suggest that even when emotional reactivity is similar, older adults are less effective at modulating their physiological responses.

Emotion in Speech: Recognition by Younger and Older Adults and Effects on Intelligibility

Spoken language conveys two forms of information: transactional (content, what is said) and interactional (how it is said). The transactional message shared during spoken communication has been studied extensively in different listening conditions and in people of all ages using standardized tests of speech intelligibility. However, research into interactional aspects of speech has been more limited. One specific aspect of interactional communication that warrants further investigation is the communication of emotion in speech, also called affective prosody. A series of experiments examined how younger and older adults produce affective prosody, recognize emotion in speech, and understand emotional speech in noise. The emotional valence and arousal properties of target words from an existing speech intelligibility test were rated by younger and older adults. New stimuli based on those words were recorded by a younger female and an older female using affective prosody to portray seven emotions (anger, disgust, fear, happiness, pleasant surprise, sadness, neutral). Similar to previous studies, the acoustical parameter that best differentiated the emotions was fundamental frequency (F0). Specifically, discriminant analysis indicated that emotional category membership was best predicted by the mean and range of F0. Overall, recognition of emotion and intelligibility were high. While older listeners made more recognition errors and had poorer intelligibility overall, their patterns of responding did not differ significantly from those of the younger listeners on either measure. Of note, angry and sad emotions were recognized with the highest degree of accuracy, but intelligibility was highest for items spoken to portray fear or pleasant surprise. These results may suggest that there is a complementarity between the acoustic cues used to recognize emotions (how words are said) and those used to understand words (what is said). Alternatively, the effect of emotion on intelligibility may be modulated primarily by attentional rather than acoustical factors, with higher performance associated with alerting emotions.

Subcortical Hyperintensities in Alzheimer's Disease and the Elderly: An MRI-based Study Examining Signs of Cerebrovascular Disease and Dementia

Subcortical hyperintensities (SH) are believed to be observable signs of cerebrovascular disease, indicating some form of subcortical vasculopathy. Also commonly referred to as leukoariosis, these hyperintense signals on proton density, T2-weighted and fluid attenuated inversion recovery magnetic resonance images, are commonly observed phenomena in Alzheimer’s disease patients and elderly persons. Several SH sub-types with differential brain-behavior associations have been proposed in the scientific literature: periventricular, deep white, cystic fluid filled lacunar-like infarcts and perivascular Virchow-Robin spaces. This study will present Lesion Explorer (LE): a comprehensive tri-feature MRI-based processing pipeline that effectively and reliably quantifies SH sub-types in the context of additional brain tissues volumetrics in a regionalized manner. The LE pipeline was validated using a scan-rescan procedure. Finally, the LE pipeline was applied in a cross-sectional study of Alzheimer’s disease patients and normal elderly controls. Brain-behavior relationships were demonstrated with regional SH volumes and executive functioning, speed of mental processing, and verbal memory.

The Effect of Acute Eccentric Treadmill Running on NF-κB Activation and HSP72 Content in Skeletal Muscle from Late Middle-aged Rats

Eccentric exercise causes skeletal muscle damage, yet the acute cellular responses post-exercise have yet to be fully elucidated. To better understand the post-exercise response, heat shock protein (HSP) 72 content and nuclear factor-κB (NF-κB) activation where examined in Adult (A; 6 month) and Late middle-aged (LMA; 24 month) Fischer 344xBrown Norway rats. Animals were randomly divided into five groups (n=6): non-exercising controls (C), level (L) or eccentric (ECC) (-16°) running at 16m.m-1 and killed immediately post-exercise (0), 48 hours post-exercise (48). Following ECC, vastus intermedius (VI) from A and LMA showed more damage compared to L exercise. Neither age-group had significantly increased VI HSP72 content compared to C. Pooled results founded increased HSP72 content in ECC-48 compared to C (p<0.02). NF-κB activation in the VI was lower in LMA (p<0.001) and unchanged in WG when compared to AC. These findings suggest HSP72 is increased following eccentric exercise in the VI.

Left Atrial Phasic Function during Exercise: The Role of Atrioventricular Coupling

Left ventricular (LV) filling increases during exercise, but left atrial (LA) phasic function and its contribution to LV filling is poorly understood. Sixteen endurance-trained middle-aged males were studied at rest and during light (LE) and moderate (ME) intensity cycle-ergometry. Atrioventricular-plane displacement (AVPD) increased from rest to LE (from 14±2 mm to 18±2 mm, p<0.01), but did not increase further at ME. LA reservoir volume increased from rest to LE (from 32±8 mL to 40±10 mL, p<0.01). LA passive contribution increased at LE (from 21±5 mL to 27±8 mL, p<0.01), while LA active contribution increased from rest only at ME (from 12±5 mL to 23±9 mL, p<0.01). AVPD, and thus the longitudinal shortening of LV systole, contributes to LA filling primarily during LE, but is a limited mechanism beyond LE. These data suggest that LV filling appears to shift to a reliance on conduit function to increase LV filling at ME.
