Outcomes of secondary alveolar bone grafts in cleft patients

A research report submitted to the Faculty of Health Sciences, University of the Witwatersrand, Johannesburg, in partial fulfilment of the requirements for the degree of Master of Dentistry in the branch of Maxillofacial and Oral Surgery 31 May 2018. / Aim The aim of this retrospective study was to determine the outcomes of secondary alveolar bone grafts (SABG) in cleft patients treated at the Wits Dental Hospital at the Charlotte Maxeke Johannesburg Academic Hospital. Objectives To record the demographic data of patients who had secondary alveolar bone grafts, and to assess the outcomes of SABG by evaluating clinical variables and the quantity of bone post SABG. Methods Records of 19 patients with a total of 23 clefts were examined to evaluate the amount of bone at the cleft site following secondary alveolar bone graft. Socio-demographic information was collected as well as the clinical variables of the type of graft used, canine eruption, keratinised tissue around teeth close to the cleft, and closure or persistence of any oronasal fistula. The amount of bone at the graft site was measured on CBCT images using the Chelsea scale. Comparisons of Chelsea scale scores and CBCT findings were carried out. SPSS ® 24 was used to analyse the data. All statistical tests were conducted at 5% significance level. Results Most (52%) patients were male, with 57% having left unilateral cleft. The majority of them received autogenous bone grafts from the chin. Most (65.2%) of the patients showed good clinical outcomes, whilst showing evidence of bone resorption in and around the graft site on CBCT images. Fifteen of the patients were considered to have partial alveolar graft resorption. Conclusion In the majority of patients, 3D CBCT images revealed bone resorption in areas that 2D images gave an impression of presence of bone. However, the majority of patients indicated good clinical outcomes despite the poor radiological findings. / LG2018

A presente investigação propôs-se a realizar uma análise radiográfica de pacientes em manutenção periodontal do Programa de Pós-Graduação da Disciplina de Periodontia da Faculdade de Odontologia da Universidade de São Paulo. A porcentagem da perda óssea alveolar foi correlacionada com a idade, o fumo , o gênero, os arcos, os grupos de dentes e faces proximais por meio de exames radiográficos completos periapicais. A amostra deste trabalho foi composta por 27 sujeitos da pesquisa do gênero masculino e 53 do gênero feminino, com idade de 16 a 84 anos (média de 48,3 anos). Foram digitalizadas 1120 radiografias periapicais (1970 dentes) e posteriormente, analisadas com o software Image Tool ® (University of Texas Health and Science Center). O critério utilizado para se considerar perda óssea foi que a distância junção esmalte cemento (JEC) à crista óssea alveolar (COA) fosse > que 2 mm. Dois examinadores calibrados (p < 0,0001), realizaram as medidas da perda óssea. A média da perda óssea da amostra foi 20,61% (± 12,12). A prevalência foi maior em fumantes. No entanto, não houve diferença entre homens e mulheres (p > 0,05). A superfície distal e a arco superior apresentaram maiores perdas, bem como o grupo de incisivos e molares superiores. O grupo etário que apresentou maior perda óssea foi o de 45 a 54 anos. O fumo e a faixa etária de 45 a 54 anos são as variáveis que, isoladamente, apresentaram indivíduos com maior susceptibilidade à perda óssea alveolar, quando avaliada radiograficamente. / The aim of the present investigation was to perform a radiographic analysis of patients in Periodontal Maintenance from the Department of Periodontology Graduation - Dentistry College University of São Paulo. Complete sets of periapical radiographs provided data for alveolar bone loss percentage which was correlated to age, smoking status, gender, arches, teeth group and proximal sites. The sample was consisted of 27 male and 53 female subjects with age from 16 to 84 years old (mean 48,3 years). 1120 radiographs (1970 teeth) were digitalized and analyzed with the software Image Tool ® (University of Texas - Health and Science Center). The criteria used to consider bone loss was when the distance from the cement enamel junction to the apex was greater than 2 mm. Two examiners, intra and inter calibrated (p < 0,0001) were responsible for taking the radiographic measurements of bone loss. The mean bone loss of the sample was 20,61%. The prevalence of bone loss was greater in smokers. Nevertheless, there was no difference between male and female (p < 0,0005). The distal surface and the superior arch had the greatest bone loss, as well as the group of incisors and superior molars. The age group of 45- 54 had the highest scores for bone loss. Smoking and group age from 45 to 54 years are two independent variables that may characterize those individuals most prone to alveolar bone loss.

Influência de três hemostáticos tópicos no processo de reparo em feridas de extração dental : análise histológica e histométrica em ratos /

Devido à dificuldade para se exercer manobras como pinçamentos e ligaduras de vasos neste tipo de hemorragias, existe a possibilidade do preenchimento do alvéolo com materiais hemostáticos de ação tópica. O propósito deste estudo foi analisar a influência de três hemostáticos tópicos (Hemostop®, Pro Tape® e ViscoStat®) na cronologia do processo de reparo em feridas de extração dental por meio de uma análise histológica e histométrica. Para tanto foram utilizados 60 ratos (Wistar) machos divididos em quatro grupos de 15 animais cada. Todos os animais foram submetidos à extração do incisivo central superior direito, seguido ou não da colocação de agente hemostático entre o terço médio e apical do alvéolo dental, e posterior sutura com fio reabsorvível. Em grupos de cinco, os animais foram sacrificados aos 7, 14 e 28 dias pós-operatórios por inalação excessiva de éter sulfúrico. Suas maxilas foram separadas, fixadas em formalina, descalcificadas em EDTA e incluídas em parafina. Foram realizados cortes de 6 æm de espessura e corados com Hematoxilina e Eosina, e Tricrômio de Masson. Após a análise histológica, em microscópio óptico; e histometria óssea, com o software ImageLab, os resultados foram submetidos ao teste não-paramétrico de Kruskal-Wallis. Pela metodologia aplicada foi possível concluir que: 1) houve atraso na cronologia do processo de reparo alveolar em todos os grupos tratados; 2) os materiais analisados provocaram intensa reação inflamatória no tecido adjacente; 3) em todos os períodos estudados (7, 14 e 28 dias), a análise histométrica revelou maior neoformação óssea nos animais do grupo 1 (controle). Due to the difficulty to exercise maneuvers as arrestty and ligature of blood vessel in this type of hemorrhages, exists the possibility of the filling of the alveolus with hemostatics materials of topical action. The purpose of this study was to analyze the influence of three topical hemostatics (Hemostop®, Pro Tape® and ViscoStat®) in the chronology of the repair process in wounds of dental extraction through a histologic and histometric analysis. For such, 60 male's rats (Wistar) were used divided in four groups of 15 animals each. All the animals were submitted to the extraction of the right superior central incisor, followed or not of the placement of hemostatics agent among the medium and apical third of the dental alveolus, and subsequent suture with reabsorble filament. In groups of five, the animals were sacrificed to the 7, 14 and 28 postoperative days by excessive inhalation of sulfuric ether. Your maxillaries were separate, fixed in formalin solution, decalcified in EDTA and included in paraffin. Cuts of 6 æm of thickness were accomplished and stained with hematoxylin and eosin, and Masson's trichromic. After the histologic analysis, in optical microscope; and histometric bony, with the software ImageLab, the results were submitted to the no-parametric test of Kruskal-Wallis. For the applied methodology it was possible conclude that: 1) there was delay in the chronology of the alveolar repair process in all the treated groups; 2) the analyzed materials instigated a intense inflammatory reaction in the adjacent tissue; 3) in all the studied periods (7, 14 and 28 days), the histometric analysis revealed larger bony neoformation in the animals of the group 1 (control group)...(Complete abstract click electronic access below) / Mestre
