Previous issue date: 2015-11-13 / Coordena??o de Aperfei?oamento de Pessoal de N?vel Superior - CAPES / O uso abusivo de ?lcool est? intimamente relacionado ? depend?ncia e a preju?zos sociais ou do trabalho. Esta tese tem como objetivo geral elaborar um instrumento sobre o abuso do ?lcool, com a finalidade de diferenciar o grau de comprometimento do quadro sintomatol?gico, considerando seus fatores psicossociais de predi??o. Como objetivos espec?ficos: I) caracterizar o estado da arte sobre avalia??o relacionada ao abuso e depend?ncia de ?lcool; II) investigar e sistematizar aspectos referentes aos fatores psicossociais preditivos para a depend?ncia de ?lcool; III) construir um instrumento para avalia??o de abuso de ?lcool e fatores de prote??o e risco para desenvolvimento de depend?ncia alco?lica; e IV) verificar evid?ncias de validade do instrumento constru?do para a popula??o brasileira. No Estudo I, observou-se que a preval?ncia de artigos referentes ao uso de ?lcool de forma problem?tica, sem classifica??o de depend?ncia, ? menor que a de artigos que investigam a doen?a j? manifestada, al?m de escassos estudos sistem?ticos sobre o tema de abuso de ?lcool no meio cient?fico. No Estudo II, foram realizados grupos focais (GFs), as an?lises acerca dos discursos dos GFs foram feitas atrav?s do Software ALCESTE e p?de-se observar um padr?o de respostas existente entre os participantes em diferentes grupos, com a gera??o de cinco classes. No Estudo III, foi desenvolvido um instrumento contemplando aspectos da S?ndrome de Depend?ncia de ?lcool do Millon Clinical Multiaxial Inventory-III, al?m das caracter?sticas definidas no Estudo I e no Estudo II. A vers?o final do instrumento contou com 59 itens avaliados atrav?s de escala likert de cinco pontos. No estudo IV, a administra??o do mesmo foi realizada no formato on line em estudantes universit?rios entre 18 e 24 anos, residentes em cidades metropolitanas brasileiras. Os resultados evidenciaram que a consist?ncia interna do instrumento ? considerada como satisfat?ria e no que se referem as classes, os dados mais significativos foram os com rela??o a Perda Financeira e Crit?rios para diagn?stico de uso abusivo de ?lcool. ? importante considerar o potencial avaliador de fatores protetivos e de risco para o desenvolvimento de depend?ncia de ?lcool do instrumento como um todo. Uma vez que os indicadores de abuso e o perfil dos abusadores tenha sido identificado, o paciente pode a ter seu tratamento/interven??o voltado para o transtorno e/ou s?ndrome espec?fica, tendo assim uma melhora r?pida e aparente. / The abusive use of alcohol is closely related to dependence and to social and work damages. The main focus of this thesis is to create an instrument about alcohol abuse, in order to differentiate the degree of commitment of the symptomatology, considering its psychosocial factors of prediction. As specific goals: I) characterize the state of the art about assessment related to the abuse and dependence to alcohol; II) investigate and systematize aspects related to the predictive psychosocial factors for alcohol dependence; III) build an instrument for the assessment of alcohol abuse and protection and risk factors for the development of an alcohol dependence; and IV) verify validity evidence of the instrument built for the Brazilian population. In Study I, it was possible to observe the prevalence of articles related to the use of alcohol in a problematic way, without a classification dependence, it is lower than the one of articles that investigate the disease when it is already manifested, not to mention a few systematic studies about the theme of alcohol abuse in the scientific environment. In Study II, focus groups (FGs) were conducted, the analysis about the discourses of the focus groups were made through the ALCESTE software and it was possible to observe a response pattern that existed among the participants in different groups, with the generation of five classes. In Study III, we developed an instrument that contemplated aspects of the Alcohol Dependence Syndrome of the Millon Clinical Multiaxial Inventory-III, in addition to the characteristics defined in Study I and in Study II. The final version of the instrument had 59 items assessed through the likert scale of five points. In Study IV, the administration of the instrument was performed in an online format with university students ranging from 18 to 24 years old, residents in Brazilian metropolitan cities. The results evidenced that the internal consistency of the instrument is considered satisfactory (? = 0,882) and in what it refers to classes, the most significant data was the one related to financial loss and criteria for the diagnosis of alcohol abuse. It is important to consider the evaluative potential of risk and protective factors for the development of alcohol dependence of the instrument as a whole. Once the indicators of abuse and the profile of the abusers has been modified, the patient may have his/her treatment/intervention focused on the trouble and/or specific syndrome, thus having a clear and fast improvement.
