Elastas+ neutrofila granulocyter och CD20+ B-celler i parodontit- respektive periimplantitvävnad

Målet med denna studie var att undersöka förekomst och lokalisering av neutrofila granulocyter (Neutrophil Elastase+) och B-celler (CD20+) hos patienter med kronisk parodontit och/eller periimplantit. Dessa celler utgör viktiga delar av det specifika respektive ospecifika försvaret och kan ses som representanter för inflammatoriska svar av olika karaktär. Inflammerad vävnad innefattande epitel och bindväv insamlades från tjugo lokaler (tretton med parodontit och sju med periimplantit) på sexton individer som behandlades kirurgiskt. Vävnaden preparerades immunohistokemiskt (Dako’s Two-Step Polymer Method) och analyserades i mikroskop. På vävnadsnivå iakttogs i samband med periimplantit ett längre fickepitel samt ett kraftigare inflammatoriskt cellinfiltrat längst apikalt. Den apikala hälften av periimplantitvävnaden uppvisade som regel mer lokalt koncentrerade inflammatoriska infiltrat vid kontaktytan än parodontitvävnaden, både gällande B-celler som neutrofila granulocyter. Graden av vaskularisering var även högre. B-celler dominerade i förekomst vid båda tillstånden och hade ofta en generell spridning i vävnaden. Neutrofila granulocyter förekom i mindre utsträckning än B-celler, framförallt vid parodontit. / The purpose of the current study was to investigate the presence and localization of neutrophil granulocytes (Neutrophil Elastase+) and B-cells (CD20+) in patients with chronic periodontitis and/or peri-implantitis. These cells play an important role in the adaptive as well as the innate immune system, and can be considered as representative for responses of different character. Inflamed tissue consisting of epithelia and connective tissue was collected from twenty sites (thirteen with periodontitis and seven with peri-implantitis) on sixteen individuals undergoing surgical treatment. The biopsies were prepared immunohistochemically (Dako’s Two-Step Polymer Method) and analyzed in microscopes. On tissue-level a longer pocket epithelium and a greater inflammatory infiltrate in the apical portion was observed in peri-implantatis. The apical half of the peri-implant tissue generally indicated a more local inflammatory infiltrate in the contact area than in periodontitis, both concerning B-cells and neutrophil granulocytes. The level of vascularisation was also higher. B-cells were predominant in both conditions and had a general distribution in most cases. Neutrophil granulocytes occurred in a smaller extent than B-cells, above all in periodontitis.
