The case for the development of an online intervention designed to support midwives in work-related psychological distress

Midwives experience both occupational and organisation episodes of work-related psychologicl distress. As the wellbeing of health professionals is linked with the safety and quality of care, these episodes of distress should be met with adequate support. Midwives can be reluctant to speak openly about episodes of work-related distress. Additionally, they may not be able, or prefer not to access, face-to-face support. As such, an online intervention may be one option that midwives turn to when seeking support, as it can provide confidential and flexible access to support. This research makes a case for the development of an online interevention, designed to effectively support midwives in work-related psychological distress. Firstly, a narrative literature review integrates contemporary research to build an overview of the nature, prevalence, and origin, of work-related psychological distress in midwifery populations. A critical literature review then explores some of the ethical considerations in relation to providing midwives with anonymous and confidential online support. This review concludes that the provision of anonymity and confidentiality online would ensure the greatest benefit overall to the greatest number of people using and working within maternity services. A systematic mixed-methods literature review then concludes that there are currently very few targeted interventions designed to support midwives in work-related distress, none of which are currently delivered online. Moreover, this review identifies insufficient high-quality research to comprehensively understand which particular interventions or techniques could deliver effective support to midwives in work-related psychological distress. Lastly, a multi-stakeholder Delphi study is presented to establish consensus in relation to the content development, design and delivery of an online intervention to support midwives and/or student midwives in work-related psychological distress. In this case, an expert panel prioritised confidentiality and anonymity, along with 24-hour mobile access, effective moderation, an online discussion forum, and additional legal, educational, and therapeutic components. Consensus also supported the inclusion of a simple user assessment to identify people at risk of either causing harm to others or experiencing harm themselves, in order to direct them to appropriate support. The impact of any future intervention of this type will be optimised by utilising the findings from this Delphi study throughout the intervention development process. Furthermore, as the ethical, practical and evidence based arguments for the development of an online intervention designed to support midwives in work-related psychological distress have now been formed, it will be important to build and rigorously test this intervention in response to the identified gaps in research. This thesis demonstrates that there is a case for the development of an online intervention designed to support midwives in work-related psychological distress. Future research will require feasibility studies, pilot studies and adequately powered randomised controlled trials in order to sercure the evidence base for any new online support for this professional population.

As demandas e pressões da situação de trabalho agregadas ás características pessoais podem contribuir para o adoecimento físico e mental de trabalhadores levando ao esgotamento profissional e esse pode estar associado à ocorrência de acidentes do trabalho. Objetivo: verificar a associação entre indicadores da síndrome de burnout, presenteísmo e a ocorrência de acidentes do trabalho com exposição ao material biológico entre profissionais de enfermagem. Método: estudo transversal descritivo realizado com enfermeiros, auxiliares e técnicos de enfermagem, de um hospital de ensino do Estado de São Paulo, acometidos por acidentes do trabalho com material biológico no período 2015- 2017 e que foram cadastrados na Rede de Prevenção de Acidentes do Trabalho com Exposição ao Material Biológico (REPATUSP). A amostra foi composta por 100 trabalhadores de enfermagem. As variáveis estudadas foram: síndrome de burnout, acidentes do trabalho, características sóciodemográficas e o presenteísmo. Os instrumentos utilizados para coleta de dados foram: protocolo de registros de acidentes da REPAT- USP; questionário de dados sociodemográficos e ocupacionais; Malash Burnout Inventory; escala de presenteismo? de Stanford (SPS-6). Os dados foram coletados de abril a outubro de 2017. Foram analisados por estatística descritiva, testes de associação qui-quadrado de Pearson e os testes não paramétricos Mann-Whitney e Kruskal-Wallis. O nível de significância adotado foi ?=0,05. O projeto foi aprovado pelo Comitê de Ética em Pesquisa da Escola de Enfermagem de Ribeirão Preto da Universidade de São Paulo (protocolo CAAE: 67510017.5.0000.5393). Resultados: Com base na análise das respostas dos 71 questionários, observou-se entre os profissionais que sofreram acidente do trabalho com material biológico, que a prevalência de burnout foi de 52,11% (37). Foram analisadas associações da síndrome de burnout e o presenteísmo em que se observou que 45,5% dos profissionais apresentaram burnout e presenteísmo. Conclusão: Este estudo evidenciou uma alta prevalência da SB nos profissionais de enfermagem, revelando assim a necessidade de intervenções em relação às condições de trabalho dos enfermeiros. Foram encontradas associações significativas entre a dimensão despersonalização da síndrome de burnout e o presenteísmo. Destaca-se a importância de se avaliar a prevalência da síndrome entre os diferentes profissionais, pois o seu desenvolvimento pode acarretar resultados negativos para os profissionais, pacientes e instituições. Os esforços aqui empreendidos pretende encorajar as instituições a promoverem iniciativas para mudanças / The demands and pressures from work aggregated to personal characteristics can contribute for workers physical and mental illness leading to professional exhaustion and can be associated with the occurrence of occupational accidents. Objective: to verify the association between indicators of burnout syndrome, presenteeism and occurency of occupational accidents with exposure to biological agents between nursing professionals. Method: This is a cross-sectional study carried out with nurses, auxiliaries and technicians of a Hospital Teaching from Sao Paulo state who were affected for accidents at work with biological material between 2015-2017 and were records at the Work Accidents Prevention Network with exposure to biological agents. The sample consisted of 100 nursing workers. The variables studied were the Burnout Syndrome, occupational accidents, socio-demographic characteristics and the presenteeism. The tools used for data collection were: protocolo de registros de acidentes da REPAT- USP; questionário de dados sociodemográficos e ocupacionais; Malash Burnout Inventory; escala de presenteismo? de Stanford (SPS-6). The data were colected from april to june 2017. Data were analyzed using descriptive statistic Pearson\'s chi-square association tests and non-parametric Mann-Whitney and Kruskal-Wallis tests. The level of significance was ? = 0.05. The project was approved by the Research Ethics Committee of the University of São Paulo at Ribeirão Preto College of Nursing (protocolo CAAE: 67510017.5.0000.5393). Results: Based on the analysis of the results of the 71 questionnaires, the prevalence of burnout was 52.11% (37) among the professionals who suffered from work accident with biological material. Associations of the burnout syndrome and presenteism were analyzed in which 45.5% of the professionals presented burnout and presenteeism. Conclusion: This study evidenced a high prevalence of Burnout Syndrome in nursing professionals, thus revealing the need for interventions in relation to nurses\' working conditions. Significant associations were found between the depersonalization dimension of burnout syndrome and presenteeism. It is important to evaluate the prevalence of the syndrome among the different professionals, as their development can lead to negative results for professionals, patients and institutions. The efforts made here are intended to encourage institutions to promote initiatives for change

Self-reported burnout among secondary Indian school teachers : role- related variables and locus of control.

The main aim of this study was to gain an overall estimate of the incidence of self-reported burnout among Indian secondary school teachers. Other secondary, though closely related aims were: a) to determine whether teachers differ, with respect to selected background variables, in their perceptions of burnout; b) to investigate the differences, if any, in the responses of burnoutees and non-burnoutees to 25 selected role-related variables with a view to examining the relationship between such variables and burnout; and c) to examine the distribution of burnoutees and nonburnoutees on an internal - external locus of control dichotomy with a view to examining the relationship between this personality construct and burnout. The sample of Level One secondary school teachers in this study (N = 690) was randomly selected from a list of all Indian secondary schools in the greater Durban area (N = 59). Teachers responded to a four-part questionnaire designed to gather data relating to demographic characteristics, role-related stressors, locus of control and degree of burnout. The Maslach Burnout Inventory was used to determine whether a respondent was a "burnoutee" or a "non-burnoutee". It included 3 subscales relating to Emotional Exhaustion, Depersonalisation and Personal Accomplishment. The major findings of this study were as follows: a) Approximately 1 in 4 teachers in the sample saw himself/herself as a "burnoutee". b) When teachers were grouped according to background variables, the incidence of burnout was found to be greater among female, married, younger, less experienced, graduate and low-salaried teachers. c) The majority of both "burnoutees" and "non-burnoutees" in this study were unhappy with teaching and found it to be only "moderately satisfying". Many also declared that teaching had not lived up to their expectations and that they would readily change to an entirely new kind of occupation if they had to start their careers afresh. The low degree of satisfaction was found to correlate with perceived feelings of burnout. Older teachers, however, enjoyed greater job satisfaction than their younger counterparts. d) No relationship was found between the personality construct of Locus of Control and burnout. / Thesis (M.Ed.)-University of Durban-Westville, 1992.
