O objetivo dessa pesquisa foi analisar o conhecimento, atitude e prática do exame de Papanicolaou em mulheres com neoplasia cervical e os fatores sócio-demo gráficos, dificuldades para consultar e a qualidade do atendimento, para entender porquê algumas pacientes não realizavam periodicamente este exame. Esse estudo foi descritivo, associado a inquérito CAP (Conhecimento, Atitude e Prática) e realizado no Hospital e Maternidade Leonor Mendes de Barros, Secretaria de Estado da Saúde, SP; com pacientes diagnosticadas entre 1993 e 1995. O tamanho amostral foi 138 mulheres, P 10% e oc 0,05. Elas foram entrevistadas por um questionário. Os dados foram submetidos à análise bivariada e múltipla de regressão logística. As mulheres com neoplasia invasora tinham conhecimento, atitude e prática mais inadequados do que aquelas com neoplasia intra-epitelial, maior idade, residiam a menos tempo em São Paulo, não trabalhavam fora e tiveram dificuldades pessoais. O conhecimento inadequado associou-se à maior idade; a atitude inadequada à maior idade e menor escolaridade e, a prática inadequada às dificuldades pessoais e às que não trabalhavam. A prática do exame depende da iniciativa do médico e a periodicidade da coleta depende da mulher sentir-se doente / Abstract: Brazil presents high cervical cancer mortality rates, in spite of sreening programmes for control of this disease. The objective of the study was to investigate knowledge, attitude and practice of Pap smear among women with cervical neoplasia, socio demographic characteristics, difficultes to consult and quality of care, to understand why some do not have a Pap smear periodically. This was descriptive study with a KAP (knowledge, attitude and practice) component, carried out at the Hospital e Maternidade Leonor Mendes de Barros - São Paulo State Health Secretariat - in São Paulo city. Subjects were diagnosed between 1993 and 1995. The sample size was 138, P 10% and oc 0,05. Women were interviewed using a questionnaire. Bivariate and logistic regression was used for the analysis. Inadequate knowledge, attitude and practice were higher among women with invasive neoplasia. They were older, had lived in São Paulo city during fewer years, were homemakers and had personal difficulties to consult. Inadequate knowledge was associated to older age inadequate attitude to older age and little schooling, and inadequate practice to personal difficulties and being a homemaker. Having a Pap smear depended on a physicians' request, and interval between tests was determined by the women' s need to consult because they were feeling unwell / Mestrado / Tocoginecologia / Mestre em Tocoginecologia
