Orientador: Fausto Viterbo de Oliveira Neto / Resumo: A proposição da presente pesquisa consiste em avaliar subjetivamente e objetivamente a estética do lábio de 38 pacientes, portadores de fissura pré-forame incisivo unilateral completa, sendo 21 do sexo masculino e 17 do sexo feminino com idade entre quatro e 14 anos, com média de oito anos e oito meses, submetidos à queiloplastia primária com a técnica de Spina, e correlacionar a avaliação estética subjetiva com as mensurações objetivas. A avaliação subjetiva foi realizada por 20 avaliadores provenientes de quatro grupos constituídos por: fonoaudiólogas; pais e pacientes portadores de fissura de lábio e/ou palato; cirurgiões dentistas, fisiologistas e geneticista; cirurgiões plásticos do Hospital de Reabilitação de Anomalias Craniofaciais, da Universidade de São Paulo. A avaliação objetiva consistiu de mensurações no lábio superior dos lados fissurado e não fissurado realizadas pelo autor. Os resultados mostraram que as mensurações dos lados fissurado e não fissurado apresentaram-se diferentes e 70% das avaliações dos examinadores foram satisfatórias. Não houve correlação significante entre o julgamento dos avaliadores e as diferenças mensuradas entre os lados fissurado e não fissurado. / Abstract: The aim of the present study was to subjectivelly and objectivelly evaluate the aesthetics of the lip of 38 cleft lip patients, 21 males and 17 females, aging 4 to 14 years (mean eight years and eight months) who have undergone lip surgery by the SPINA'S technique, and to correlate both kinds of evalutions. Subjective evaluation was performed by 20 individuals among speech/language pathologistis, cleft lip and palate patients, cleft lip and palate patients parents, dentists, physiologists, geneticist and plastic surgeons from the Hospital of Rehabilitation of Craniofacial Anomalies, University of São Paulo. Objective evaluation consisted of measurements in the cleft and noncleft sides of the upper lip, which were performed by the author. The results have shown that there were differences in the measurements in both sides of the lip, and 70% of the subjective evaluations were considered satisfactoriy. No significant correlation was found between the subjective evaluation and the measurements obtained in the cleft and noncleft sides of the patients lips. / Mestre

Search the library catalog for the location and call number. / Towards a more complete description of cleft constructions, this thesis comprises an investigation of the prosody, syntax, and information structure of IT clefts, REVERSE WH clefts, and existential THERE clefts in Spoken English. Cleft constructions were extracted from the Santa Barbara Corpus of Spoken American English on the basis of syntactic characteristics, and empirical methods were developed for evaluating clefts with respect to prosody and information structure factors. Native speaker-hearer judgments about cleft constructions in authentic spoken language were gathered to provide a basis for operational definitions of PROSODIC PROMINENCE, GIVENNESS, NEWNESS, CONTRASTIVENESS, and levels of contextual RELEVANCE. While cleft constructions have conventionally been discussed as contrastive focusing devices, the current study provides empirical evidence for a more complex view of clefts. Added to past corpus studies, this thesis shows that English cleft constructions exhibit a broader range of subtypes and functions than captured by traditional accounts. / Committee in Charge: Dr. Doris L. A amostra foi constituída por 67 adolescentes portadores de fissura labiopalatal (G1), matriculados no Hospital de Reabilitação de Anomalias Craniofaciais da Universidade de São Paulo, HRAC-USP, sendo os dados coletados no período de 16 de novembro a 22 de dezembro de 1999; e de 67 adolescentes estudantes sem fissuras (G2), no período entre 22 de agosto a 15 de setembro de 2000. Os grupos tinham representação de ambos os sexos, na faixa etária de 10 a 20 anos incompletos e residentes na cidade de Bauru. Com a intenção de verificar o cotidiano, foi aplicado um questionário de questões estruturadas e algumas questões abertas, enfocando as suas percepções. Apesar do estigma que a deformidade congênita representa, os adolescentes portadores de fissura labiopalatal superam os limites da deficiência, evento fundamental para que se integrem à sociedade, apresentando satisfação com a vida que levam, através da autorealização, saúde e bem-estar. / The term quality of life has been employed by all professionals concerned with human welfare, which is often associated to psychosocial indicators. This study aimed at identifying, describing and evaluating the quality of life verbalized and noticed by teenagers with cleft lip and palate in relation to a control group presenting similar sociodemographic features, as well as the possible similarities and differences regarding quality of life. The sample comprised 67 teenagers with cleft lip and palate (G1) regularly enrolled for treatment at the Hospital for Rehabilitation of Craniofacial Anomalies of the University of São Paulo (HRAC/USP), the data of which were collected during the period November 16th to December 22nd, 1999. The control group consisted of 67 adolescent students without clefts (G2) who were surveyed in the period August 22nd up to September 15th, 2000. The groups included teenagers of both genders, aged 10 to 20 incomplete years old and living in the city of Bauru. A questionnaire comprising structured and some open questions was applied in an attempt to analyze the subjects’ quotidian according to their perceptions. Despite of the stigma yielded by the congenital deformity, the teenagers with cleft lip and palate overcome the limits imposed by their deficiency, which is fundamental for their integration into society, and demonstrate satisfaction with the life they live, through self-realization, health and welfare.

Genetic defects of collagen XI:the role of a minor cartilage collagen in chondrodysplasias, oral cleft defects and osteoarthrosis

Abstract Collagen XI is a minor component of articular cartilage collagen fibrils together with collagen IX. They are in close functional relationship with the major cartilage collagen II. Collagen XI has been suggested to play a role in regulating the diameter of collagen II fibrils. Together these collagens form a supportive framework in the extracellular matrix. Besides articular cartilage, these three collagens can also be found in the vitreous body of the eye, the intervertebral disc, the inner ear and in various tissues during embryonic development. As the major cartilage collagen, collagen II has been studied quite extensively. Several syndromes ranging from lethal to milder ones have been shown to result from collagen II gene defects. Far less is known about defects in genes coding for the minor cartilage collagens, IX and XI. By identifying mutations in the coding genes and observing the resulting phenotypes, the function and importance of these genes start to unravel. The goal of this study was to provide more information about collagen XI. As a quantatively minor cartilage component, it is a good candidate for mild disease phenotypes. Collagen XI gene mutations have been shown to cause relatively mild phenotypes, such as Stickler and Marshall syndromes and non-syndromic hearing loss. Seven families with a recessive chondrodysplasia, otospondylomegaepiphyseal dysplasia (OSMED), were analysed for mutations in COL11A2. This study showed that OSMED is typically caused by the absence of the α2(XI) chains. Sixty-two patients with isolated Robin sequence, cleft palate or micrognathia were analysed for COL11A2 gene mutations. Six unique nucleotide changes were found that are likely to associate with the phenotype. The results showed that collagen XI gene defects can play a role in the etiology of oral clefting, but are not common causes of these phenotypes. Altogether 72 unrelated osteoarthrosis (OA) patients and one family with OA were analysed for mutations in genes coding for collagens II, IX and XI. Eighteen percent of them were found to have a unique sequence variation. An association analysis of OA patients failed to reveal any common predisposing alleles in these genes.

Language and Play Development in Toddlers with Cleft Lip and/or Palate

The relationship between play gesture performance and language milestones was examined for 6 children with cleft lip and/or palate at 20, 24, and 30 months of age. Standardized measures of language development, language samples, and a play gesture protocol were administered and analyzed at each assessment. Results showed language delays for several of the children with cleft palate and displayed a complex relationship between play and language development. The results show parallels between single object use and vocabulary development as well as sequential play gestures and structural aspects of language use. Data from this study demonstrated slow play gesture and language development for children with cleft palate only.

Longitudinal Comparison of Early Speech and Language Milestones in Children With Cleft Palate: A Comparison of Us and Slovak Children

The purpose of this study was to compare early speech and language development of children with and without cleft lip and/or palate (CLP) in the US and Slovakia from 6 to 24 months of age. Thirty-two children from the US (eight with CLP and eight noncleft) and Slovakia (eight with CLP and eight noncleft) participated in this study. The children were videotaped at four time points for 30 minutes during mother-child interaction with play sets controlled for early-developing sounds in each language. Mean Babbling Level, consonant inventories, number of different words and mean length of utterance were calculated for 6-to 24-month samples. Results indicated that the US and Slovak groups showed similar performance across the ages. Cleft and noncleft groups showed significant differences in acquisition of all of the speech and language measures. High-pressure consonants, particularly alveolar place of articulation, were problematic for children with CLP.
