A psychotherapy clinic in a township : exploring the concept of community

This study is about the exploration of the concept of community, using the Mamelodi Counselling Clinic as the context for the exploration. The members who got involved in different phases in the running of the clinic, went through the process of defining and redefining the concept. This process was based on their experiences and their coevolved reality of what the concept means. These experiences will be discussed and in the end, a punctuated end product of the coevolved meanings will given. Furthermore, an account will be given as to how the running of the clinic evolved with the changes in meaning. CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / Esta pesquisa teve como objetivo geral conhecer as representaÃÃes sociais do profissional de saÃde sobre a prÃtica nos novos serviÃos de saÃde mental de um municÃpio da ParaÃba. Buscou-se caracterizar o grupo pesquisado quanto aos aspectos pessoais, sociodemogrÃficos e profissionais; apreender as representaÃÃes sociais da equipe tÃcnica sobre a atuaÃÃo profissional no novo modelo de atenÃÃo em saÃde mental; interpretar as representaÃÃes sociais da equipe tÃcnica com base nos pressupostos da Reforma PsiquiÃtrica. Trata-se de uma pesquisa com abordagem qualitativa e tipologia descritiva e exploratÃria, fundamentada no suporte teÃrico-metodolÃgico da Teoria das RepresentaÃÃes Sociais. O estudo foi realizado em trÃs Centros de AtenÃÃo Psicossocial, um infantil, um geral e um Ãlcool e outras drogas, da cidade de Campina Grande-PB. Integraram a pesquisa sujeitos que compunham a equipe tÃcnica de nÃvel superior. O material empÃrico foi obtido por meio roteiro de entrevista estruturada de dados sociodemogrÃficos, do Teste de AssociaÃÃo Livre de Palavras (TALP) e do grupo focal e analisado com base no processo de categorizaÃÃo temÃtica. O projeto foi aprovado pelo Comità de Ãtica em Pesquisa e seguiu os preceitos Ãticos e legais da ResoluÃÃo 196/96 do Conselho Nacional de SaÃde. Os resultados apontam similaridades quanto aos aspectos de formaÃÃo e estÃmulo à consecuÃÃo de conhecimentos relacionados Ãs transformaÃÃes na saÃde mental e discussÃo das prÃticas existentes; por outro lado, à notÃria a precariedade dos vÃnculos empregatÃcios, a falta de apoio dos gestores, inclusive quanto ao aspecto financeiro. Com relaÃÃo à dinÃmica de funcionamento, os resultados apontam que os serviÃos estÃo acolhendo as pessoas em sofrimento mental, a partir da especificidade de cada demanda. Mas, simultaneamente, pÃem em evidÃncia as implicaÃÃes relativas Ãs condiÃÃes de trabalho na Ãrea e ao frÃgil suporte social fornecido à demanda de saÃde mental. Com base nas representaÃÃes sociais sobre a prÃtica nos novos serviÃos de saÃde mental, percebe-se que alguns dos preceitos da Reforma PsiquiÃtrica estÃo sendo contemplados no cotidiano dos serviÃos, destacando-se: o trabalho em equipe interdisciplinar; o acolhimento à clientela, respeitando-se sua singularidade e individualidade; oferta de outras formas de abordagens, que nÃo a medicamentosa. ConcluÃmos que as representaÃÃes sociais dos profissionais aparentam ter incorporado termos que enfocam mudanÃas epistemolÃgicas na produÃÃo das prÃticas, alÃm de estarem pautadas em saberes que fomentam novas formas de abordagem, ampliando as intervenÃÃes, de modo que o indivÃduo possa se apropriar de mecanismos que permitam a conduÃÃo com mais autonomia de sua prÃpria vida no cotidiano. Este estudo nÃo esgota a possibilidade de futuras pesquisas sobre a temÃtica, e espera-se que possa auxiliar em estudos acerca da prÃtica profissional em saÃde mental dentro de novo paradigma definido nas polÃticas pÃblicas de saÃde e saÃde mental. / This research was aimed to know the social representations of health professionals about the practice in the new mental health services in a ParaibaÂs city. We sought to characterize the group surveyed about the personal aspects, socio-demographic and professional; apprehend the social representations of the technical team on the professional activities in the new model of mental health care; interpret the social representations of the technical team based on the assumptions of the Psychiatric Reform . It is a qualitative research approach and type descriptive and exploratory, based on theoretical-methodological support of the Theory of Social Representations. The study was conducted in three Psychosocial Care Center, for children, general and alcohol and other drugs, in the city of Campina Grande-PB. Integrated the study professional with the university level.The empirical material was obtained through Structured interview for demographic data, Testing of Free Association of Words and focus group. The project was approved by the Research Ethics Committee, and followed the ethical and legal resolution 196/96 of National Health Council. The results show similarities on the issues oF training and encouraging the pursuit of knowledge related to changes in mental health and discussion practices; the other hand, it is clear that the precariousness of employment contracts, including the financial aspect. With respect to dynamic operation, the results indicate that services are welcoming people in mental distress, from the specificity of each demand. But at the same time highlight the implications for working conditions in the area and the fragile social support provided to the demand for mental health. Based on the social representations about the practice in the new mental health services, we realized that some of the principles of psychiatric reform are being put into practice in everyday services, including: teamwork, interdisciplinary; welcoming customers, respecting uniqueness and individuality; offering other forms of approaches than drugs. We concluded that the social representations of the professionals seem to have incorporated terms that focus on epistemological changes in production practices, and are guided into knowledge that promote new approaches, expanding the interventions, so that the individual can appropriate mechanisms for the management of your everyday life. This study doesn`t exhaust the possibility of future research on the subject, and it is expected to assist in studies of profession practice in mental health set into new paradigm in public health policies and mental health.

An evaluation of NISAA as a community service for battered woman

M.A. / NISAA was established in 1994. It is one of the first organisations dealing with women abuse in Lenasia and the surrounding areas. The organisation has great potential in becoming a recognised institution in South Africa. Women who are in distress, have no where to go and are left abandoned have a shelter with NISAA. In the short period of its existence NISAA has strived hard to uplift and assist women. Counselling and treatment is offered on a daily basis by professionals and trained personnel. However, many needs still have to be met and research has shown us that not nearly enough is being done for those who are being abused, be it wife abuse or child abuse. The processs of research is presented in five stages and covers, the problem statement, the literature survey, research methodology, analysis, conclusions and recommendation. The survey of the literature is very important in this study. South Africa has become epitomised by a rapidly changing social order, swiftly unfolding political development and an unprecedented fluidity in the thinking of future opinions. The attitudes and perceptions of domestic violence and violence in general needs to be changed. The chapter on research methodology stresses the importance of feminist organisations. Feminist organisations' role is vital in any study on woman abuse, as research on woman abuse began with these organisations. Methodology in this study also focuses on the qualitative nature of battering, research and the relevant themes surrounding battering. These themes include woman abuse, intervention. treatment and shelters. Other important aspects discussed is coding of data, research design, sampling and interviewing procedures. The analysis and results shows that NISAA has made a significant contribution to battering of women. From the results of this study, NISAA has become an important structure and service in the South African society. The services NISAA should expand on include, a safe and positive crisis help, as well as promoting the rights of all victims. Important to the growth of NISAA is alleviating the funding problem. Other relevant findings discussed are attitudes and perceptions of women abuse and services that encourage and discourage the use of NISAA. These findings compliments. Gishen's (1993) study on the Power Organisation. Recommendations made to NISAA are based on the findings of this research. This research hopefully will expose abuse, help women to achieve recognition in its fight against abuse and in its quest for the empowerment of women. The research was also intended as an educative process. This study entails the physical and psychological abuse of women which has become a global problem. The problem needs to be addressed, understood, analysed and treated. An effective solution must be found.

A psychiatric service at a community hospital

The purpose of this dissertation is to evaluate an attempt to use the facilities of a general hospital with minimal psychiatric cover to deal with acute psychiatric conditions. This is in keeping with current psychiatric opinion in South Africa as expressed by Dr. C.W. Allwood and Dr G.A.D. Hart. This dissertation will examine the needs structure, difficulties and results of the provision of a service dealing with acute psychiatric disorders in a peripheral General Hospital and make suqqestions should similar satellite facilities be established at other peripheral hospitals. Although in principle. the concept of part-time specialist cover at a peripheral general hospital is not new in medical. surgical and gynaecological departments. the previous involvement of private psychiatrists within the teaching department has been in the form of out-patient department sessions only at academic and state psychiatric hospitals as is practiced in Cape Town. This project however allowed for the evaluation of emergency cases with subsequent referral to appropriate treatment centres if needed from a peripheral hospital. A consultation-liaison service within the general wards and the opportunity to conduct seminars, lectures and in-house training for all level of professional staff were also encompassed.

An Evaluative Study of Client Satisfaction at a Mental Health Clinic

Evaluative research in the field of mental health is carried out pursuant to several goals. One is to study a very particular intervention on a very particular client (or client population) in a controlled way with the aim to test a theory of intervention. This form of research requires basically an experimental research design. It also requires rigorously defined and measured intervention and a good control for factors other than intervention. The requirements for this form of research are stringent and the number of such projects reported is, therefore, rare. The present study was done as a pilot study for the Elahan Center for Mental Health and Family Living (formerly Clark County Mental Health) in Vancouver, Washington. This agency has recently undergone much change. About eighteen months ago there was a change of Directors. At about the same time, though unrelated, the agency was involved in a public scandal around the drug program. As a result of much inter-agency strife and the change in administration, few employes from the old staff remain. The new administration is dedicatedly behaviorist in therapeutic orientation, as are some of the therapists. Use of para-professionals in professional capacities is high and most of tile para-professionals follow the behavioristic bent of the administration.
