Influência da irradiação com lasers de alta potência na microinfiltração bacteriana e fúngica em dentes humanos retrobturados com MTA / High power lasers Influence in bacterial and fungical microleakage in MTA retrofills

Este estudo experimental avaliou, in vitro, a microinfiltração de E. faecalis e C. albicans em raízes cujas superfícies apicais ressectadas e retropreparadas para obturação retrógrada sofreram irradiação dos lasers Nd:YAG, Diodo 810 nm e Er,Cr:YSGG, e posteriormente retrobturadas com MTA. Setenta raízes foram instrumentadas, obturadas endodonticamente, apicectomizadas e receberam retropreparos com pontas ultra-sônicas diamantadas e lisas na potência média. A divisão dos espécimes se deu de acordo com o laser utilizado e o tipo de microorganismo. Os grupos G1 e G5 não foram irradiados, G2 e G6 irradiados com Nd:YAG, G3 e G7 com Diodo e G4 e G8 com Er,Cr:YSGG (n = 8). Foram utilizados controles positivo (n = 3) e negativo (n = 3). Todos os procedimentos operatórios foram realizados com um dispositivo de simulação de dificuldades operatórias. Após a impermeabilização, colagem das raízes em tubos preparados, esterilização e retrobturação com MTA branco, o sistema de câmaras superior, contendo o inóculo de microorganismos, e inferior, contendo meio esterilizado foi montado em contato com o ápice. Os grupos G1 a G4 foram inoculados com E. faecalis, e os grupos G5 a G8 com C. albicans. A troca de meio na câmara superior se deu a cada 3 dias e a verificação da turvação do meio inferior era diária, por um período de 60 dias. Ao final, os resultados indicaram que: o controle positivo apresentou 100% de microinfiltração, o controle negativo não microinfiltrou e os grupos experimentais apresentaram padrões de microinfiltração que variaram entre 50% (G1) e 100% (G2) de microinfiltração, onde 68,75% do total de espécimes microinfiltraram. Não houve diferença estatística significativa entre os grupos aplicando-se os testes Exato de Fischer e Kruskal-Wallis (p > 0,05) em nenhuma interação. Pode-se concluir que o tratamento das retrocavidades e superfície apical ressectada, com os lasers utilizados neste estudo, não alterou o padrão de microinfiltração das retrobturações com MTA frente a E. faecalis e C. albicans. O tipo de microorganismo utilizado também não alterou a quantidade de espécimes microinfiltrados. / This in vitro experimental study evaluated the microleakage of E. faecalis and C. albicans in roots whose ressected apical surfaces and retrocavities had suffered irradiation from Nd:YAG, Diode 810 nm and Er,Cr:YSGG lasers, and later retrofiled with MTA. Seventy roots had been prepared, obturated, had suffered apicoectomy and were retroprepared with diamond and conventional ultra-sonic tips in the average power. The division of specimens was in accordance with the used laser and the type of microorganism. The groups G1 and G5 had not been irradiated, G2 and G6 irradiated with Nd:YAG, G3 and G7 with Diode and G4 and G8 with Er, Cr:YSGG (n = 8). Positive (n = 3) and negative (n = 3) controls had been used. All procedures executed until this point had been carried inside an access difficulties simulation device. After the waterproofing, attachment of the roots in prepared tubes and sterilization, the retrofillings were made using white MTA. The superior chambers were inoculated with the microorganisms and inferior chambers were completed with sterile culture broth in contact with the root tip. The G1, G2, G3 and G4 groups had been inoculated with E. faecalis and the rest of the groups with C. albicans. The contaminated broths of the upper chambers were exchanged each 3 days and for 60 days the verification of the turbidity in the inferior chambers was evaluated daily. At the end, the results had indicated that: the positive control had 100% microleakage, the negative control none and the experimental groups had presented microleakege patterns that range from 50% (G1) to 100% (G2), with a total of 68.75% of all specimens presenting microleakage. Applying the Exact Fischer test and Kruskal-Wallis test (p > 0,05), no significant statistical difference between the groups were observed in any interaction. It can be concluded that the treatment of the retrocavities and ressected apical surface, with the lasers used in this study, did not modify the rate of E. faecalis and C. albicans microleakage in MTA retrofillings. Also the type of used microorganism did not seem to modify the amount of microleakage in the specimens.

Orientador: Mario Tanomaru Filho / Banca: Luiz Geraldo Vaz / Banca: Marco Antonio Húngaro Duarte / Resumo: Novos materiais retrobturadores sao propostos, destacando-se os cimentos que contem hidroxido de calcio ou a base de trioxido mineral agregado (MTA). O cimento endodontico Sealer 26 e o MTA sao indicados em obturacoes retrogradas. Recentemente, o cimento a base de MTA denominado Endo CPM Sealer foi proposto. Assim, o objetivo deste estudo foi avaliar a liberacao de ions calcio e o pH proporcionado por alguns cimentos utilizados em obturacao retrograda: 1- Sealer 26, 2- MTA, 3- Endo CPM Sealer e 4- Endo CPM Sealer em maior proporcao po/liquido e 5- Oxido de zinco e eugenol (OZE). Para analise do pH e calcio, os materiais foram inseridos em tubos de polietileno medindo 1,5 mm de diametro e 10 mm de comprimento e imersos em frascos com 10 mL de agua destilada. As avaliacoes foram realizadas nos periodos de 3, 6, 12, 24 e 48 horas, 7, 14 e 28 dias. Os tubos foram transferidos apos cada periodo experimental em novos frascos contendo 10 mL de agua destilada. A mensuracao do pH das solucoes foi realizada por meio de pHmetro digital. A liberacao de calcio foi estudada por meio da espectrofotometria de absorcao atomica. Tambem foi avaliada a capacidade seladora dos mesmos materiais em obturações retrogradas. Foram utilizados dentes unirradiculados humanos, cujos canais radiculares foram instrumentados e obturados com cones de guta-percha e cimento de oxido de zinco e eugenol. Apos a seccao de 3 mm apicais, cavidades retrogradas com 3 mm de profundidade foram preparadas com auxilio de pontas de ultra-som. Os dentes foram divididos em cinco grupos experimentais, de acordo com o material utilizado para obturacao retrograda (n=12) e dois grupos controles (n=3), sendo um sem material retrobturador e outro com impermeabilizacao total incluindo a superficie apical. The most important are that containing calcium hydroxide, or based on mineral trioxide aggregate (MTA). The endodontic sealers Sealer 26 and MTA are used in retrogade obturation. Recently, the cement-based MTA called Endo CPM Sealer has been proposed. The aim of this study was to evaluate the release of calcium ions and pH provided by some materials used in retrograde filling: 1 - Sealer 26, 2 - MTA, 3 - Endo CPM Sealer and 4 - Endo CPM Sealer in greater proportion powder / liquid and 5 - Zinc oxide and eugenol (ZOE). For analysis of pH and calcium, the materials were put in tubes of polyethylene measuring 1.5 mm in diameter and 10 mm in length and immersed in bottles with 10 mL of distilled water. Evaluations were performed in periods of 3, 6, 12, 24 and 48 hours, 7, 14 and 28 days. The tubes were transferred after each experimental period in new bottles containing 10 mL of distilled water. The measurement of pH of the solutions was done through a digital pHmeter. The release of calcium was studied by means of atomic absorption spectrophotometry. It was also evaluated the sealing ability of the same materials in retrograde obturations. One rooted human teeth were used, whose root canals were instrumented and fillied with gutta-percha cones and zinc oxide and eugenol sealer. After the section of 3 mm apical, retrograde cavities with 3 mm depth were prepared using ultrasound tips. The teeth were divided into five experimental groups, according to the material used for retrograde filling (n = 12) and two control groups (n = 3), one without material and the other with total impermeabilization including apical surface.
