Older people's perceptions of health services and their health seeking behaviour in the era of HIV/AIDs : a case study of the eThekwini Municipality in KwaZulu-Natal.

In sub-Saharan Africa - the region with some of the world's highest levels of HIV prevalence - AIDS has reached pandemic proportions. At present, the highest rates of infection are in the region where the main route of transmission is heterosexual intercourse. Although HIV/AIDS is a major concern for all age groups, to date there has been very little exploratory research conducted on the population aged 50 and above. This dissertation seeks to analyse older people's perceptions of health services and their health seeking behaviour in the era of HIV/AIDS. This study utilized both qualitative and quantitative methodologies for data collection. The qualitative data relies on focus group discussions and the quantitative data from the exit interviews. In total, two hundred interviews were conducted and two focus group discussions were conducted: one with men only and one with women only. For the purpose of the study, older people referred to those persons aged 50 years and above. The study found that awareness of HIV/AIDS was relatively high. The majority of older people were aware of the main routes of HIV transmission and also the measures that they can use to protect themselves. An important source of information is public health facilities. Few older men and women perceived a medium or high risk of HIV infection. This is likely to have influenced their attendance at HIV/AIDS services. Few respondents reported ever using HIV/AIDS services. There are a number of factors inhibiting use of HIV/AIDS services including perceptions of health services, stigma and discrimination, high transportation costs and poor interpersonal relationships with providers. The Health Systems Trust and the integrated nutrition programme : a case study of policy implementation.

The South African government has a constitutional obligation to provide health care services to all South Africans. The Department of Health (DOH) has been tasked with delivering health care services to a country which is still recovering from the inequities of the past where unemployment, lack of education, and poverty rates are high. Poverty contributes to food insecurity in many households. Household food insecurity contributes to malnutrition, morbidity, and mortality, particularly in children. Policies are made to tackle a particular identified social problem. In 1995, the DOH introduced the Integrated Nutrition Program (INP) to deal with malnutrition in this country. The Health Systems Trust (HST) is an independent NGO which was established in 1992 to support the transformation of the South African healthcare system. The HST implements the community component of the INP with the aim to contribute to household food security and health status of children under the age of 5 years (INP Progress Report 2002/3: 3). The purpose of the study is to identify policy implementation issues as identified by the literature, particularly the literature on policy networks, while analyzing the implementation of the INP. The key focus of this study is policy implementation. It particularly looks at policy networks as forums for policy making and implementation. It looks at interactions between government and non-governmental organisations, more particularly the DOH and HST and their networking with other organisations. A qualitative methodology was used because, as Marlow (1993:67) argues, a qualitative approach may be more effective because the answers can provide a detailed description of the program. Primary and secondary data was collected from the DOH as well as the HST. Purposive sampling was applied where participants were selected on the basis of their relevance to the study. Interviews were held with key informants. A structured questionnaire was designed for the key participants at the HST as well as the key participants of the DOH. The implementation of the INP by the DOH and HST is an illustration of a policy network in action. The study on the HST's implementation of the INP emphasizes that government cannot afford to ignore the contribution NGOs have made and continue to make in service delivery 'because of their cost effectiveness and ability to engage people at the grassroots level, especially in remote areas' (Taylor, cited in Camay and Gordon, 2002:37). Factors which contribute to network failure, according to Kickert et al (1997:9) include: a lack of incentives to cooperate and the existence of blockades to collective action; proposed goals may be vague; important actors may be absent, while the presence of other actors may discourage the participation of necessary actors; crucial information about goals, means and actors may be lacking; discretionary power may be absent; and the absence of commitment of actors to the common purpose. All the above were evident in the implementation of the INP. The implementation of the INP is for the most part successful. An examination of the role of antenatal care attendance in preventing adverse birth outcomes in South Africa.

Increasing adverse birth outcome are a major concern in South African maternal care and globally In South Africa, perinatal mortality rate of 40/1000 and maternal mortality ratio of 150/100 000 are poor considering the fact that 95.1 percent of women attend antenatal care and 83.7 percent of women deliver in a medical facility. This study focuses on the effect of lack of adequate antenatal care on adverse birth outcome using data from the 1998 South African Demographic and Health Survey. The analysis is based on univariate and bivariate analysis to examine the effect of socio- demographic characteristics on adverse birth outcomes. In addition, binary logistic regression is used to examine the impact of antenatal and sociodemographic characteristics on adverse birth outcome. The results show majority of women reporting adverse birth outcome are those who delivered though caesarean section (53 percent) and that most of these women are likely to be educated and have better socio economic status and that they are likely to be Non African. It is also shows that the proportion of women attending antenatal care adequately is very high in South Africa. The results show that the use of antenatal care is determined by a range of socio demographic factors including the level of education and the economic status of the mother. Only timing of antenatal care, place of delivery and race had a significant effect on adverse birth outcome. All other variables were not significant. Results from the binary regression analysis show that women who started their antenatal care during second and third trimester (95%CI: 0.211-0.975), were Non African (95%CI: 1.082 to 2.098) and delivered in a private health facilities (95%CI: 0.28 to 0.73) were more likely to have adverse birth outcome compared to their counterparts. Most women choosing caesarean section do so without adequate information on the disadvantages of delivering through caesarean section. The evolution of decentralisation policy in developing countries : a policy analysis of devolution in Zimbabwe.

The COPAC-driven constitution-making process in Zimbabwe was largely focused on revamping local governance and ensuring a return to democracy. The attempts were mainly focused on checking the power of the executive in a bid to institutionalise separation of powers by empowering the legislative and judicial arms of the state. However the most critical power-sharing objective was the reform of intergovernmental balance of power between the central government and subnational government structures; the provincial, urban and rural local authorities. This was captured in the ideology of devolution of power, which was set as fundamental principle of good governance repealing the erstwhile centralised system of government. The hope was that this shift of preference will be enshrined in the 2013 Constitution. However this study notes that this public optimism has not been sufficiently met in the new constitution, which is officially dubbed ‘Constitution of Zimbabwe Amendment (No. 20) Act, 2013’. The study argues that the provisions for devolution in the 2013 Constitution are inconsequential and betray a lack of political will by the central government to devolve authority and resources to local governments. The extensive reliance on impending Acts of Parliament to clarify and give effect to devolution, such as the administrative, political and fiscal competencies of different tiers of government, creates a weak framework for decentralisation which is tantamount to the continuation of the existing status quo, in which subnational governments are de facto deconcentrated appendages of central government. Notwithstanding this major setback in the aspirations of devolution in Zimbabwe, this study recommends that extensive civil society engagement with the central government which has been evidenced over the last couple of years can still reclaim the reforms initially agreed to by the ruling elite during the negotiations thereby ensuring the institutionalisation of devolution in Zimbabwe. / Thesis (M.Soc.Sc.)-University of KwaZulu-Natal, Pietermaritzburg, 2013.

Unruly Nature and Technological Authority: Governing Locust Swarms in the Sahel

This dissertation examines how states and international organizations respond to complex ecological problems that are mismatched to their management capacity. The study concentrates on effort by scientific advisors, technicians, and bureaucrats to manage the population dynamics of the desert locust, Schistocerca gegaria in Western and Northern Africa. Desert locusts periodically invade crops and pastures, where they cause massive depredations that undermine agricultural productivity and food security, often in extremely impoverished regions. The immensely complex and bio-geographically stochastic breeding and gregarization dynamics of the desert locust put the insect at odds with the conventional spatiality of the state. This make it difficult for managers to precisely predict and effectively control locust outbreaks and invasions. To better understand the factors shaping institutional responses to this insect, I address three interrelated questions primarily informed by political ecology, political geography, and critical development studies: (1) What historical trajectory yielded the contemporary configuration of locust control? (2) Why do some approaches to locust management become selected over others amongst experts and organizations? (3) What is the relationship between the spatial dynamics of locust outbreaks and invasions, on the one hand, and the spatial logic and imperatives of the state? Analysis of interviews, field observations, and archival records indicates that the ability of the desert locust to evade and exceed the conventional spatiality of the state has made this pest problem an appealing field to innovate and enact new regimes of governance that operate transnationally. This has embedded locust control in the historical arc spanning from formal colonialism to the current configuration of independent states supported by international programs of foreign aid and technical assistance. In this context, concerns for the professional viability of locust expertise within state agencies and international organizations favor the selection of strategies that best fit the modalities of access to development aid and resources. This motivates state-mandated locust managers to favor the adoption of locust control strategies that are best aligned with capacity building goals of these programs, and that incorporate locust management in broader interventions of social and environmental improvement.

The implementation of housing policy in Msunduzi municipality.

This research project aims to explore the implementation process of the low cost housing policy, over the period from 1994 to 2001 in the, Msunduzi Municipality area. There are problems with the implementation of the housing policy by local government and this is causing delays in the delivery of the low cost housing for the poor. The findings of this research project will show that these problems have many sources. These include: firstly, the funding and the budget allocated for the housing projects. Secondly, the interference of the provincial government and the power play to control the housing policy. Thirdly, the uniformity of the houses in the projects; these houses can be related to the social and township houses of the late apartheid era. The housing policy in Msunduzi is facing many problems in the implementation stages. In order to identify the problems one must first find out what makes a problem free and an efficient policy. The information provided in the background study was gathered from a collection of sources, namely, journal articles, newspaper articles and government papers including the housing White Paper. A major part of the information gathered was through interviews conducted with a handful of members of the housing sector in the Msunduzi area. My interviews were conducted with a member of the Msunduzi Housing Association, a member of the National Home Builders Registration Council, and three members of the local Department of Housing. An assessment of the housing strategy during the post-apartheid era : the case of Waterloo in Durban.

This research attempted to assess the housing strategy at Waterloo as well as the services such as electricity, sanitation, water, transport, adequate shelter and employment opportunities that are available for residents. The information was gathered through interviews and the use of questionnaires. There were some problems encountered during the gathering of information. The questionnaires were completed by the household head and in some cases the head was not available, making it difficult for the researcher to continue smoothly, in such cases extra visits were made. In some cases the household head was unable to read, so the researcher had to read the questionnaire for him/her and then write down an answer. Another problem worth noting was that of acceptance of a researcher by the respondents, but the researcher was able to finally convince them and assured them of confidentiality of the information. There were lots of problems raised by beneficiaries of Waterloo about the services rendered to them. Most of them are not satisfied with the type of houses built for them, they are small, some have leakages and no ventilators. Beneficiaries also complained about the basic services which are not available for them. The other services needed at Waterloo are health clinics, schools and transport. People have to travel long distances for other services and traveling to those places is very costly. It is recommended that Government looks at the problems raised by the beneficiaries and improve on them. It would also help them when they are building in other areas not to make the same mistake again. From this study it shows that the strategy in Waterloo partially meets the requirements of the beneficiaries and with the recommendations given, Waterloo will be a better place to live in . / Thesis (M.A.)-University of Durban-Westville, 2003.

Behind Closed Doors: Aboriginal Women's Experiences with Intimate Partner Violence

This study provides a critical analysis of Aboriginal women's experiences with intimate partner violence, and how this experience is affected by their lives on a reserve and their access to resources while there. By taking a social ecological perspective—looking at individual, interpersonal, community, institutional/organizational, and society/policy levels of the ecosystem—a comprehensive analysis can be done. The study explores the role of colonization in the development of today’s circumstances, and its associated factors. It analyzes the role of the government, both past and present, in perpetrating and enabling the problem. This study concludes by arguing that Aboriginal women's experiences are much more complex, needing more innovative and community-based initiatives in order to deal with its intracies. The Canadian government's attention and efforts thus far have fallen short of what is needed within many of Canada's Aboriginal communities.
