Precisión diagnóstica de los Scores Brighton y Coast para la admisión al hospital de emergencias pediátricas en el periodo 2018

Objetivo: Identificar las características de nuestra población y la precisión diagnóstica de los scores Brighton y COAST para predecir la admisión hospitalaria en el servicio de Emergencia de un hospital pediátrico Materiales y métodos: Estudio de cohorte tipo retrospectivo en población pediátrica de 1 mes a 17 años, que acudieron al servicio de emergencia del Hospital de Emergencias Pediátricas en los meses de febrero y agosto del 2018 en Lima, Perú. Se recabó datos para los scores Brighton y COAST a través de revisión de historias clínicas, las cuales se verificaron hasta el momento de admisión hospitalaria o alta del servicio de emergencia del paciente. Se calculó sensibilidad, especificidad, valores predictivos positivos y negativos, cociente de verosimilitud y el área bajo la curva (AUC). Resultados: Con una población de 822 pacientes. Se admitieron al servicio de hospitalización 86 pacientes y 736 fueron no hospitalizados. Con dichos datos se obtuvo para los scores Brighton y COAST con un punto de corte ≥3 una sensibilidad de 69,8% y 63,3%, respectivamente y una especificidad de 87,8 %y 87,6% para ambos scores. Así, como una AUC de 0,82 para Brighton y 0,80 para COAST. Conclusiones: Los puntajes ≥3 para los scores Brighton y COAST es específico y poco sensible para predecir admisión hospitalaria. No obstante, el valor predictivo negativo de estos scores es alto (mayor a 90%), por lo cual el número de pacientes con puntaje menor a 3 que requieran hospitalización será mínimo. / Objective: The objective of this study is to identify the characteristics of our population and the diagnostic precision of the Brighton and COAST scores to predict hospital admission to the Emergency service of a pediatric hospital. Materials and methods: A retrospective cohort study in the pediatric population from 1 month to 17 years, who attended the emergency service of Hospital de Emergencias Pediatricas from February and August of 2018 in Lima, Peru. Data were collected by the Brighton and COAST scores through a review of physical medical records, which were followed up until the time of hospital admission or discharge from the patient’s emergency service. Were calculated sensitivity, specificity, positive and negative predictive values, likelihood ratio and area under curve (AUC). Results: 822 patients were analyzed. Of these, 86 were admitted and 736 discharged. Data were collected Brighton and COAST scores with a cutoff point ≥3. The sensitivity obtained was 69,8% and 66,3% for the Brighton and COAST scores, respectively. The specificity was 87,8% and 87,6% for the Brighton and COAST scores, respectively. Thus, as an AUC of 0.82 for Brighton and 0.80 for COAST. Conclusions: A score of ≥3 has good specificity but poor sensitivity for predicting hospital admission. However, the negative predictive value of these scores is high (<90%), which is why the number of patients with a score lower than three who require hospitalization will be minimal. / Tesis
