Análise de citocinas, quimiocinas e fatores de crescimento em endoftalmite infecciosas aguda pós-facectomia

Analisar a participação de citocinas, quimiocinas e fatores de crescimento em endoftalmite infecciosa aguda pós-facectomia. MÉTODOS. Amostras de humor vítreo de 18 pacientes com endoftalmite foram coletadas e armazenadas a -80oC. IL-1ra, IL-2, IL-4, IL-5, IL-6, IL-7, IL-8, IL-9, IL-10, IL-12 (p70), IL-13, IL-15, IL-17, FGF básico, Eotaxina, G-CSF, GM-CSF, IFN-γ, MCP-1, MIP-1a, MIP-1b, PDGF-BB, RANTES, TNF-α e VEGF foram dosadas pela técnica de Bioplex e comparadas com 39 pacientes submetidos a vitrectomia por doença não-infecciosa (controle). RESULTADOS. As concentrações de IL-1ra, IL-2, IL-4, IL-6, IL-8, IL-9, IL-10, IL-15, IL-17, eotaxina, FGF Básico, G-CSF, GM-CSF, IFN-γ, MCP-1, MIP-1a, MIP-1b, PDGF-bb, RANTES e TNF-α foram significantemente maiores (p<0.05) em olhos com endoftalmite. Não houve diferença significante entre os grupos em relação à concentração de IL-5, IL-12 (p70), IL-13 e VEGF. A concentração de IL-7 foi superior no grupo controle (p=0.0229). Considerando-se os diferentes diagnósticos do grupo controle, apenas IL-5 não apresentou diferença significante entre os grupos (p=0.1764). Foi observada correlação positiva entre TNF-α e eotaxina, IL-8, MIP-1a, MIP1b e RANTES. Observou-se correlação positiva moderada entre TNF-α e acuidade visual inicial (r=0.59, p=0.0096). Não se encontrou correlação entre acuidade visual inicial ou final entre os demais fatores dosados. CONCLUSÕES. A endoftalmite infecciosa pós-facectomia estimula a produção de citocinas, quimiocinas e fatores de crescimento. Os níves de TNF-α podem estar relacionados com a gravidade do processo inflamatório inicial. Não foi encontrado fator inflamatório preditivo para a acuidade visual final. / Purpose. To analyze the involvement of cytokines, chemokines and growth factors in acute infectious endophthalmitis following cataract extraction surgery. Methods. Vitreous humor samples were collected from 18 patients with infectious endophthalmitis. The Bio-Plex technique was used to measure IL-1ra, IL-2, IL-4, IL-5, IL-6, IL-7, IL-8, IL-9, IL-10, IL-12 (p70), IL-13, IL-15, IL-17, basic FGF, eotaxin, GCSF, GM-CSF, IFN-γ, MCP-1, MIP-1a, MIP-1b, PDGF-BB, RANTES, TNF-α and VEGF for comparison with levels in 39 patients undergoing vitrectomy for a noninfectious disease (control). Results. IL-1ra, IL-2, IL-4, IL-6, IL-8, IL-9, IL-10, IL-15, IL-17, eotaxin, bFGF, G-CSF, GM-CSF, IFN-γ, MCP-1, MIP-1a, MIP-1b, PDGF-BB, RANTES and TNF-α levels were significantly higher (p <0.05) in eyes with endophthalmitis. There were no significant differences between groups for IL-5, IL-12 (p70), IL-13 and VEGF levels. IL-7 levels were higher in the control group (p = 0.0229). Considering the different diagnosis in the control group, only IL-5 was not significantly different between groups (p = 0.1764). There were positive correlations between TNF-α and eotaxin, IL-8, MIP-1a, MIP1b and RANTES, and a moderate positive correlation between TNF-α and the initial visual acuity (r = 0.59, p = 0.0096). There was no correlation with the initial or final visual acuity for the other factors measured. Conclusions. Infectious endophthalmitis following cataract extraction stimulates the production of cytokines, chemokines and growth factors. Levels of TNF-α may be related to the severity of the initial inflammatory processes. No inflammatory factor that could predict the final visual acuity was evident.
