Edentulismo funcional e seus fatores de risco em adultos

They were all selected from a previous dental loss prevalence study thus being complemented by an active research. The study was a case-control based and data collection was made though a survey as well as with a clinical examination. The cases were identified through individuals with 20 or more teeth. Age and gender were used as variables of pairing off. The data was analyzed through chi-square, significant level of 95% to the checking of its force associations. The independent variables were grouped in three levels. The first one, more macro, is related to the region where the individual lives, which was also related to the second level, the family-based one, linked with a third level, at this low socio-economical level, where the domiciliary density was favorable, living in capital cities, regions with prime sanitary condicitions, with predominant possibility of accessing the public dental service, but in despite of this, only looking for this service when tooth ache is felt, where preferentially an aid dressing treatment is executed in detriment of preventive procedures. From all the samples, less than 25% of the individuals make use of tobacco and alcoholic beverages for quite a number of years. There was an association of functional edentulism with all the studied variables in a regional method. In the family-based with Crit?rio Brasil (OR=4,45) and monthly wages (OR=9,62) and to an to an individual level, the associations took place with the current use of kind of attendance (OR=1,78), looks for dressing treatment (OR=2,51), does not look for preventive treatment (OR=3,31), pain as the main cause of demand (OR=1,92), previous treatment as the demanding reason for dental service (OR+0,28), interval of the last visit to the dental service (OR=1,35) and when advise was received (OR=1,66). It was noticed from the results that the functional edentulism is much more expressive in those families which live in environments with precarious social economical sanitary conditions. Such conditions seem to have a direct influence upon the family social economical conditions which are also shown in detriment to functional edentulism. In the same way, the collection of variants influence the social economical conditions of the individual, as well as the kind of dental service searched by them, the reason of the search and the interval of the last visit to the dental service were strong determiners to the functional edentulism. Beyond that, individual habits like the use of tobacco and its frequency on its previous use influenced in a significant way the existence of functional edentulism in the studied population / O prop?sito desse estudo foi investigar os fatores de risco para o edentulismo funcional em adultos de 35 a 44 anos na cidade do Natal-RN. Participaram do estudo 278 adultos, selecionados a partir de um estudo de preval?ncia sobre perdas dent?rias e complementados por busca ativa. O estudo foi do tipo caso-controle e a coleta de dados foi realizada atrav?s de um question?rio e exame cl?nico. Os casos foram identificados por indiv?duos com 13 ou menos dentes e os controles indiv?duos com 20 ou mais dentes. Idade e g?nero foram utilizados como vari?veis de emparelhamento. Os dados foram analisados atrav?s do qui-quadrado, Mann-Whitney ou teste t de Student para amostras independentes e Kruskall-Wallis para um n?vel de signific?ncia de 95%. Ademais, lan?ou-se m?o da raz?o de chances (odds ratio) e o seu intervalo de confian?a (95%) para verifica??o da for?a das associa??es. As vari?veis independentes foram agrupadas em 3 n?veis. O primeiro, mais macro, relacionado ? regi?o que o individuo habita, que se relaciona com um segundo n?vel, familiar e com um terceiro n?vel, desta vez mais micro, o individual. A amostra foi composta na maior parte por mulheres, com baixo n?vel socioecon?mico, densidade domiciliar favor?vel, morando a maior parte da vida em capitais, em regi?es com boas condi??es sanit?rias, com acesso predominantemente ao servi?o odontol?gico p?blico, que procura esse servi?o apenas quando sente dor, onde preferencialmente se executa tratamento curativo em detrimento dos procedimentos preventivos. Desses menos de 25% dos indiv?duos fazem uso de tabaco e ?lcool h? muitos anos. Houve associa??o do edentulismo funcional com todas as vari?veis estudadas em n?vel regional. Em n?vel familiar, com Crit?rio Brasil (OR=4,45) e renda mensal (OR=9,62) e em n?vel individual, a associa??o se deu com o uso de tabaco atual (OR=2,64), uso de tabaco no passado (OR=2,70) e freq??ncia de uso de tabaco no passado, tipo de servi?o atendido (OR=1,78), procura por tratamento curativo (OR=2,51), n?o procura por tratamento preventivo (OR=3,31), dor como motivo da procura (1,92), tratamentos anteriores como motivo da procura pelo servi?o odontol?gico (OR=0,28), tempo da ?ltima visita ao servi?o odontol?gico (OR=1,35) e se recebeu orienta??o (OR=1,66). Observou-se a partir dos resultados que o edentulismo funcional ? mais expressivo naquelas pessoas que vivem em ambientes com piores situa??es socioecon?mico sanit?rias. Tais condi??es parecem ter influ?ncia direta sobre as condi??es socioecon?micas das fam?lias que tamb?m se mostraram como determinantes do edentulismo funcional. Do mesmo modo, este conjunto de vari?veis influenciam as condi??es socioecon?micas dos indiv?duos, assim como o tipo de servi?o odontol?gico procurado, o motivo da procura e o tempo da ?ltima visita ao servi?o odontol?gico, que foram fortes determinantes do agravo estudado. Ademais, h?bitos individuais como o caso do uso do tabaco e sua freq??ncia de uso no passado influenciaram significativamente a exist?ncia de edentulismo funcional na popula??o estudada
