Evaluating Surgical Outcomes: A Systematic Comparison of Evidence from Randomized Trials and Observational Studies in Laparoscopic Colorectal Cancer Surgery

Background: Laparoscopic surgery for colorectal cancer is a novel healthcare technology, for which much research evidence has been published. The objectives of this work were to compare the oncologic outcomes of this technology across different study types, and to define patterns of adoption on the basis of the literature. Methods: A comprehensive systematic review of the literature was conducted using 1) existing systematic reviews, 2) randomized controlled trials (RCTs), and 3) observational studies. Outcomes of interest were overall survival, and total lymph node harvest. Outcomes were compared for congruence. Adoption was evaluated by means of summary expert opinions in the literature. Results: 1) Existing systematic reviews were of low to moderate quality and displayed evidence of overlap and duplication. 2) Laparoscopy was not inferior to open surgery in terms of oncologic outcomes in any study type. 3) Oncologic outcomes from RCTs and observational studies were congruent. 4) Expert opinion in the literature has been supportive of this technology, paralleling the publication of large RCTs. Conclusions: The evaluation of laparoscopic surgery for colorectal cancer in RCTs and observational studies suggests that it is not inferior to open surgery. Adoption of this technology has paralleled RCT evidence.

Alkohol a násilná kriminalita - případová studie / Alcohol and violent crime - case study

The topic of this thesis is the effect of alcohol consumption on committing violent crime. The aim of this work is to describe the relationship between alcohol use and perpetration of violent crime using a description of selected violent crimes. The theoretical part deals firstly with the individual areas such as alcohol and violent crime. In the following paragraphs work connects these two concepts - thus dealing with alcohol as a criminogenic factor. Finally, the work is devoted to the theoretical models that describe link between alcohol and crime. The practical part contains a description and analysis of two selected violent crimes - murder and robbery, which resulted from a qualitative analysis of court files. Within it are described the circumstances in detail that led to the commission of a crime in connection with the use of alcohol. The work aims to address three main issues - whether the use of alcohol has a role in the criminal career of the offender; on what circumstances related to the commission of the offense is the expertise pointing, as to the issue of alcohol use and express that it is possible in these cases find the influence of alcohol applied to the offense.

The Diploma thesis deals with general issues in the environment of Czech expertise from the view of ordinary experts. The study fosuses on a critical analysis of expert activities with special regards to remuneration issues, timing of expert fees, expertise, experts‘ inauguration and dismissal from a post and cooperation with government authorities. The methodology used is an array of reactions by experts to individual and specific points, which are then evaluated and possible methods of solution are proposed. The end of the thesis statistically summarises all problems in a graph and highlights the most critical ones.

The aim of this thesis is to define the position of an expert on an expropriating proceedings and creating an expert opinion for these uses. In the theoretical part the definitions connected to expropriation and expropriating proceedings are explained. An expert activity and an expert opinion are defined in other chapters in the theoretical part. Last two chapters deal with transportation infrastructure and evaluating of the immovable assets where there are the established price, the common price and the ways of evaluating. The practical part of the thesis is devoted to the expert opinion in expropriating proceedings. The established price, the standart price of expropriated immovable assests and compensation for moving are set in a comparing way within the expert opinion.

The second part of the thesis deals with the professional insurance of experts. Diploma thesis deals with oficial causes of road accidents and their consequences. In the first part are defined basic terms connected with traffic accidents, their classification and statistical records. The second part is focused on analysis of long – term statistics of traffic accidents in the the Czech Republic for the years 1996-2015, which aim has been assed the total number of accidents, their causes and consequences. The results of the analysis have been compared to other European countries or states associated in OECD. In the context of theoretical part have been characterized legal and also technical interpretation of selected terms of act no 361/200 coll. concerning road traffic. Conclusion of the thesis is based on application of legal and also technical interpretation in case studies with real road accident.

Sebevražednost v ČR jako veřejně-politický problém a možnosti prevence / Suicidality in CZ as public policy problem and prevention options

Suicide rate is above EU average in Czechia. At the same time, no initiative trying to solve this problem exist. Aim of this thesis is suggest content of suicide prevention policy suitable for Czech environment. Author adopts optics of evidence-based policy approach and in line with this, meta- review (systematic review of systematic reviews; MR) and group discussion were conducted. MR identified 73 potentially effective interventions, 47 (64%) of these were recommended for assessment for Czech context. These interventions were critically reviewed by seven experts and discussion resulted in set of comments and other relevant outputs. Results of MR and group discussion were then put together in relation to broader context and in this sense they represent optimal basis for complex suicide prevention policy in Czechia.
