EINLEITUNG: Der apikal repositionierte Verschiebelappen (ARF) ist eine chirurgische Technik zur Augmentation der periimplantären keratinisierten Gingiva (KG). Ziel der Studie war die Quantifizierung des Gewinnes an KG sowie die Abhängigkeit des Behandlungsergebnisses von verschiedenen patientenbezogenen Faktoren. MATERIAL UND METHODEN: Die Studienpopulation bestand aus 13 Patienten mit insgesamt 31 Implantaten, bei allen wurde eine Augmentation der KG mittels des ARFs durchgeführt. Es wurden vier verschiedene Messungen der Breite der KG durchgeführt: präoperativ, postoperativ, zum Zeitpunkt der endgültigen prothetischen Versorgung und die durch die orale Inzision mobilisierte Breite der KG. Neben den absoluten Werten wurde die Abhängigkeit des Ergebnisses von verschiedenen patientenbezogenen Faktoren (Alter, Geschlecht, Einheildauer; Position des Implantates) ausgewertet. ERGEBNISSE: Die Breite der KG postoperativ (+3,94 mm, p < 0,0001) und zum Zeitpunkt der prothetischen Restauration (+3,55 mm, p < 0,0001) haben sich signifikant im Vergleich zu den Werten präoperativ erhöht. In der Einheilungsphase kam es zu einer signifikanten Schrumpfung der KG (-0,39 mm, p < 0,0421). Beim Vergleich der verschiedenen Gruppen konnten keine statistisch signifikanten Zusammenhänge festgestellt werden. ZUSAMMENFASSUNG: Der apikal repositionierte Verschiebelappen ist eine zuverlässige Methode zur Augmentation der KG. / OBJECTIVE: The apically relocated flap (ARF) is a surgical procedure to augment keratinized gingiva (KG) around dental implants. The purpose of this study was the quantification of the expected gain of KG as well as the reliance between different patient specific values and the gain of KG. METHODS AND MATERIALS: The study population included 13 patients with a total of 31 dental implants, all of which were uncovered by the technique of the ARF. Four different measurements were performed in order to take the width of the KG: preoperative, postoperative, at the time of the incorporation of the prosthetic restauration as well as at the time of the oral incision. RESULTS: A statistically significant gain of KG postoperative (+3,94 mm, p < 0,0001) and at the time of the incorporation of the prosthetic restauration (+3,55 mm, p < 0,0001) compared to the width of KG preoperative could be found. During healing time, a shrinkage of the KG (- 0,39 mm, p < 0,0421) could be proven. No statistically significant reliance between groups of patient specific values and the gain of KG could be found. CONCLUSIONS: The apically relocated flap is a reliable surgical procedure to augment KG around dental implants.
