Análise qualitativa e quantitativa das fibras colágenas na hérnia inguinal indireta

Os autores apresentam um estudo de caso-controle, no qual foram colhidas amostras de fáscia transversal; na região inguinal, em 26 pacientes, entre 20 e 60 anos de idade, masculinos, com hérnia inguinal indireta, coletadas no momento da correção cirúrgica, em pacientes operados no Hospital Geral de Caxias do Sul, e comparadas com amostras do mesmo tecido, retiradas de 26 cadáveres sem hérnia, masculinos, entre 20 e 60 anos de idade, não formolizados, no Instituto Médico-Legal de Caxias do Sul. Utilizaram-se as técnicas de coloração com hematoxilina-eosina e com picrosirius para determinar os elementos constitutivos e a quantificação do percentual de colágeno total e percentual de fibras colágenas tipos I e III nas amostras de fáscia transversal. As lâminas de picrosirius foram submetidas à observação no microscópio ótico com lentes de polarização, e foram realizadas leituras em cinco campos aleatórios de médio aumento (100x). A média de proporção do tecido polarizado e as características de birrefringência em cada campo foram anotadas. Os resultados mostraram que as quantidades de colágeno total eram 17,3% menores nos pacientes com hérnia inguinal indireta, quando comparados com o grupo-controle de cadáveres sem hérnia (p<0,01). Assim, como o percentual de fibras colágenas tipo I nos pacientes com hérnia inguinal indireta apresentou resultado 23,7% menor que o grupo- controle (p<0,01); e com relação ao percentual de fibras colágenas tipo III, a diferença entre as médias foi 6,4% menor no grupo-controle (p<0,01). Os autores sugerem que os percentuais menores de colágeno total e colágeno tipo I na fáscia transversal de pacientes com hérnia inguinal indireta poderiam ser um dos fatores na formação dessas hérnias. / The authors present a case-control study where samples of transversal fascia from 26 male patients with indirect inguinal hernia and from 26 unformalized male cadavers without hernias were compared. No. of bitstreams: 0 Previous issue date: 2005 / Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) / Com o objetivo de investigar os efeitos produzidos pela prótese de polipropileno, implantada na parede posterior do canal inguinal, 14 cães, adultos e sem raça definida, pesando entre 12 e 23 Kg, foram igualmente distribuídos em dois grupos de 07 e submetidos a inguinotomia bilateral, porém com colocação de prótese em somente um dos lados (grupo A = tela à esquerda; grupo B = tela à direita). A região contra-lateral com o mesmo tipo de dissecção e sem a colocação da prótese, serviu para comparação. Após 60 dias os animais foram reoperados pela mesma via de acesso, sendo realizada uma avaliação macroscópica bilateral concomitante, seguida da retirada dos funículos espermáticos, epidídimos e testículos para estudo histopatológico. Por meio de microscopia óptica estudou-se a presença de reação inflamatória agudo/crônica no 1/3 proximal do funículo espermático, no corpo do epidídimo e testículo. Nestas mesmas localizações mediu-se, respectivamente, os diâmetros da luz do ducto deferente, ducto do epidídimo e túbulo seminífero. Realizouse a análise morfológica quantitativa dos espermatozóides no epidídimo, e no testículo, das células intersticiais e espermatogênicas. Outros quatro animais, sem qualquer tipo de manipulação cirúrgica prévia, serviram como controle (grupo C). Macroscopicamente havia aderências acentuadas entre a tela e a parede posterior do canal inguinal, e moderada entre o funículo espermático e a tela. Nenhuma aderência foi observada no lado contra-lateral (sem tela), assim como no grupo controle. Ainda no lado com tela, três cães apresentaram uma congestão leve do plexo pampiniforme, sendo que um deles também apresentou uma hidrocele. Microscopicamente, no lado xx que foi implantada a tela, além de uma reação inflamatória do tipo corpo estranho, observou-se também uma reação inflamatória crônica no funículo espermático e ducto deferente em todos os animais. No lado contra-lateral havia uma reação inflamatória crônica em menor grau que do lado com tela em 86% dos funículos espermáticos e em 36% dos ductos deferentes. Três cães apresentaram uma reação inflamatória crônica no epidídimo do lado com tela, sendo que um deles também o apresentou do lado sem tela. Destes três cães, dois apresentaram uma redução dos espermatozóides no epidídimo, sendo que um deles também apresentou uma reação inflamatória crônica focal interessando a face interna da albugínea e um outro, redução acentuada da espermatogênese. No grupo controle um cão apresentou uma reação inflamatória crônica focal e um outro, um micro-abscesso no epidídimo. O cão que apresentou a reação inflamatória crônica focal no epidídimo também apresentou redução dos espermatozóides em ambos os lados. Os diâmetros médios da luz do ducto deferente, do ducto do epidídimo e do túbulo seminífero, do lado com prótese, sem prótese e grupo controle foram, respectivamente, 0,31-0,36-0,37 mm, 0,14-0,15-0,16 mm e 77- 76-78 µ. As conclusões são de que a tela provoca uma maior reação inflamatória no funículo espermático e uma redução na luz do ducto deferente de forma significante, bem como redução acentuada da espermatogênese numa minoria dos animais. / With the aim of investigating the affects produced by the prosthesis of polypropylene implanted in the back wall of the inguinal canal, 14 adult cross-bread dogs, weighing between 12 and 23 kg, were equally divided in two groups of 07 and submitted to the bilateral inguinotomy, however the placement of prosthesis in only one of the side (group A = mesh to the left; group B = mesh to the right). The contralateral region with the same type of the dissection and without the placement of the prosthesis, served for comparison. After 60 days the animals were new operated by the same initial skin incision, being carried out a bilateral macroscopic evaluation by the same time, followed by the retreated of the spermatic funiculus, epididimus and testicle for histological analysis. By means of optical microscope the presence of inflammatory reaction acute/ chronic in the 1/3 proximal of the spermatic funiculus was studied, in the body of the epididimus and testicle. In these same location, the diameters of the luminal of the deferens ductucs, epididimus ductus and seminiferous tubules was measured respectively. The quantitative morphological analysis of the spermatozoom in the epididimus, and in the testicle, of the intersticial and spermatogenic cells was achieved. Four others animals, without any type of the prior surgical manipulation, served as a control (group C). Macroscopically there were marked adhesions between the mesh and posterior wall of the inguinal canal, and moderate between the spermatic funiculus and the mesh. No adhesion was observed in contralateral side (without mesh), as well as in the control group. Still in mesh side, three dogs presented a light congestion of the plexus pampiniform, being that one of them also presented a hydrocele. Microscopically, in the side that was implanted the mesh, beyond of the an foreign body type inflammatory reaction, a chronic inflammatory reaction in the spermatic funiculus and ductus deferens in all the animals. Was also observed in contralateral side there was an inespecific chronic inflammatory reaction in lower grade that by the side with mesh in 86% of the spermatic funiculus and in 36% of the ductus deferens. Three dogs presented a chronic inflammatory reaction in the epididimus in the side with mesh, being that of them also presented in the side without mesh. Two among these three dogs, two presented a reduction of the spermatozoon in the epididimus, being that one of them also presented a focal chronic inflammatory reaction involving the internal side of the albuginea and the other, one micro-abscess in the epididimus. The dog that presented a focal chronic inflammatory reaction in the epididimus also presented an espematozoom reduction in the both side. In one of these sides mesh, there was also marked reduction of the spermatogenesis. The means diameters of the luminal of the ductus deferens, epididimus ductus and of the seminiferous tubules, of prosthesis side, without prosthesis and control group were, respectively, 0,31-0,36-0,37 mm, 0,14-0,15- 0,16 mm and 77-76-78 µ .The conclusions are that the mesh, provokes also an inflammatory reaction in the spermatic funiculus and a reduction in the light of the ductus deferens, significative, as well as accented reduction in the spermatogenesis in the minority of the animals. / BV UNIFESP: Teses e dissertações

Porém o tratamento cirúrgico ainda tem controvérsias: a exploração do lado contralateral assintomático, a incidência de complicações relacionadas às lesões dos vasos espermáticos e ducto deferente e complicações relacionadas à técnica cirúrgica, como as recidivas da hérnia ou criptorquia iatrogênica. A questão sobre qual é a melhor maneira de detectar patência de um processo peritônio- vaginal contralateral em crianças com hérnia inguinal unilateral tem sido debatida por mais de 50 anos. O ideal seria eliminar a desnecessária exploração inguinal e um futuro reparo cirúrgico contralateral. Com o advento da videocirurgia e suas alternativas terapêuticas, objetivou este estudo analisar os resultados do tratamento das hérnias inguinais por videolaparoscopia em meninos acima de 6 meses de idade, avaliando a efetividade da técnica empregada, a presença de hérnia contralateral, a correlação do espessamento do cordão espermático com os achados videolaparoscópicos e a análise da ocorrência de complicações pós-operatórias. Foram incluídos em um estudo prospectivo não randomizado 51 pacientes masculinos com hérnia inguinal não recidivada, tendo sido efetuadas 68 herniorrafias inguinais. Concluiu-se que a técnica empregada neste estudo mostrou-se efetiva, segura e com baixa incidência de complicações, apresentando-se como um excelente método para avaliação diagnóstica do lado contralateral da hérnia e como um método mais eficaz para detectar a presença de hérnia inguinal ou patência peritônio-vaginal contralateral assintomática, quando comparado com o sinal clínico de espessamento do cordão espermático. O presente estudo evidenciou recidiva de 2 hérnias. Não houve alterações do tamanho dos testículos nem criptorquia iatrogênica. / The conventional inguinal herniorrhaphy represents one of the most frequent surgical interventions in children, presenting high success rates and low rates of complications. However, controversies regarding the treatment of inguinal hernia still remain: the exploration of the assintomatic contralateral side, the incidence of complications related to injury of spermatic vessels and vas deferens, and complications related to the surgical technique such as the hernia recurrence or iatrogenic cryptorchidism. What is the best way to detect a contralateral patent processus vaginalis in a child presenting a unilateral inguinal hernia is an issue that has been debated for over 50 years. The ideal tool would be the one that eliminated the unnecessary inguinal exploration and future contralateral surgical repair.The advent of videosurgery and its therapeutic alternatives motivated this study to analyze the results of the treatment of inguinal hernias through videolaparoscopy in boys over six months of age, assessing the effectiveness of the technique, the presence of contralateral hernia, the correlation of the spermatic cord thickening with the videolaparoscopic findings and the analysis of the occurrence of post-surgical complications. Fifty one male patients with non recurrent inguinal hernia were included in a prospective study and sixty eight herniorrhaphy were performed. The conclusion was that the technique used in this study was effective and safe with a low incidence of complications, being an excellent method for diagnostic assessment of the contralateral side of the hernia and as a better and more efficient method to detect the presence of an inguinal hernia or an assintomatic contralateral patent processus vaginalis as compared to the clinical sign of spermatic cord thickening. Concluiu-se que a técnica empregada neste estudo mostrou-se efetiva, segura e com baixa incidência de complicações, apresentando-se como um excelente método para avaliação diagnóstica do lado contralateral da hérnia e como um método mais eficaz para detectar a presença de hérnia inguinal ou patência peritônio-vaginal contralateral assintomática, quando comparado com o sinal clínico de espessamento do cordão espermático. O presente estudo evidenciou recidiva de 2 hérnias. Não houve alterações do tamanho dos testículos nem criptorquia iatrogênica. / The conventional inguinal herniorrhaphy represents one of the most frequent surgical interventions in children, presenting high success rates and low rates of complications. However, controversies regarding the treatment of inguinal hernia still remain: the exploration of the assintomatic contralateral side, the incidence of complications related to injury of spermatic vessels and vas deferens, and complications related to the surgical technique such as the hernia recurrence or iatrogenic cryptorchidism. What is the best way to detect a contralateral patent processus vaginalis in a child presenting a unilateral inguinal hernia is an issue that has been debated for over 50 years. The ideal tool would be the one that eliminated the unnecessary inguinal exploration and future contralateral surgical repair.The advent of videosurgery and its therapeutic alternatives motivated this study to analyze the results of the treatment of inguinal hernias through videolaparoscopy in boys over six months of age, assessing the effectiveness of the technique, the presence of contralateral hernia, the correlation of the spermatic cord thickening with the videolaparoscopic findings and the analysis of the occurrence of post-surgical complications. Fifty one male patients with non recurrent inguinal hernia were included in a prospective study and sixty eight herniorrhaphy were performed. The conclusion was that the technique used in this study was effective and safe with a low incidence of complications, being an excellent method for diagnostic assessment of the contralateral side of the hernia and as a better and more efficient method to detect the presence of an inguinal hernia or an assintomatic contralateral patent processus vaginalis as compared to the clinical sign of spermatic cord thickening. The present study has shown the recurrence of two hernias. There were no alterations in the size of the testicles or iatrogenic cryptorchidism.
