This diploma thesis deals with an experimental analysis of lubrication of total hip joint replacement using fluorescence microscopy, focusing on so-called soft bearing pairs. Experiments were realized using a hip joint simulator, which was modified and refined for fluorescence microscopy. The aim of this diploma thesis was to determine the effect of the individual components of the synovial fluid on the lubrication mechanisms. For this purpose, different model fluids were designed to represent the composition of proteins and other synovial fluid constituents to elucidate the effect of albumin, -globulin and hyaluronic acid on the resulting lubrication performance. The experiments were separate into two categories different in type of load, dynamic (representative walking) and combined (representing standing with short walk). The experimental results were time-dependent fluorescence intensity values, representing dimensionless film thickness parameters. The values were recorded on the graphs and supplemented with fluorescence images taken during the characteristic sections of the measurements. The measured data was subsequently confronted with previously published articles. It was found that the dominant constituent responsible form film thickness is albumin, while -globulin and hyaluronic acid forms thin stable layer enhancing adsorption ability of albumin which contributes to increase of film thickness eventually.

The diploma thesis is focused on the run-in phase of total hip replacement. Mutual adjustment of contact surfaces takes place in this phase and polyethylene acetabular cup shows notable creep behaviour. The magnitude of creep deformations is assessed experimentally in relation to acetabular cups orientation in human body after its implantation. The condition of articulating surfaces is also assessed as well as resulting coefficient of friction in run-in phase. Physiological load and kinematics of the artificial joint as well as physiological conditioning is achieved by new module of existing joint simulator, which is created as part of this thesis. Deformations and condition of articulating surfaces are assessed via optical methods and SEM. Findings about the run-in phase are compared with existing FEM analyses to evaluate simplifications used in these analyses. Measurement of creep deformations has also potential to improve the accuracy of acetabular wear diagnosis in clinical practice.

Behavior of coefficient of friction is observed on Mini Traction Machine. Results are plotted at graphs representing dependence of coefficient of friction on time. There are described effects of several kinematic conditions, the influence of used material and the effect of the method of contact lubrication. The experiments analyze how the formation of lubricant film with a layer of adsorbed protein affects coefficient of friction.

This diploma thesis deals with an experimental analysis of lubricant film formation in hip joint replacements. The main objective is to clarify the effect of mean speed, slide-to-roll ratio, and material of femoral head on the development of film thickness, focusing on the role of particular constituents contained in model synovial fluid, such as albumin and -globulin. For this purpose, a model ball-on-disc configuration was applied, while the development of lubricant film was evaluated using the combination of fluorescent microscopy and optical interferometry. To better understand the process, coefficient of friction between implant surfaces was later investigated as well. The effect of material, slide-to-roll ratio, mean speed and model fluid composition was analysed. Results indicate that the main parameter, influencing the character of film formation, is slide-to-roll ratio. Under most conditions, the dominant constituent responsible for the film thickness development was albumin. Coefficient of friction is affected mainly by the material of implant. The thesis contains original scientific results extending the knowledge in the area of hip joint biotribology.

Einleitung 2015 belegte die Implantation einer Endoprothese am Hüftgelenk mit 219.325 Operationen Platz 8 der 50 häufigsten Operationen der vollstationären Patienten in Krankenhäusern (DRG-Statistik 2015). Bei diesen Eingriffen kommt eine Vielzahl von verschiedenen Prothesenmodellen zum Einsatz (Kirschner 2005). Aktuell geht der Trend in Richtung zementfreie Verankerung, Verkleinerung der Implantate und den Erhalt von möglichst viel Knochensubstanz (Jerosch 2013). So werden in den letzten Jahren immer häufiger Kurzschaftendoprothesen implantiert (Jerosch 2013). Sie stellen ein knochensparendes System dar und erlauben gleichzeitig verschiedene Gleitpaarungen (Jerosch 2013). Allerdings liegen für die meisten Kurzschaftendoprothesentypen noch keine Langzeittestergebnisse vor (Jerosch 2011), so dass das Langzeitverhalten dieser Prothesen nicht vorhergesagt werden kann. Speziell bei Kurzschaftendoprothesen ist der Einfluss der verkürzten Schaftlänge auf das Einwachsverhalten und die Lebensdauer des Implantats noch nicht ausreichend geklärt. Am Lehrstuhl für Konstruktionslehre und CAD der Universität Bayreuth finden deshalb Untersuchungen zur computergestützten Analyse von Kurzschaftendoprothesen statt. Ziel hierbei ist es, durch eine Variation der Schaftlänge eine Prothesenform mit optimalen Spannungsverläufen und einer verbesserten Krafteinleitung in den Knochen zu entwickeln.

A qualitative holographic study of hemipelvic and acetabular deformation caused by different hip prostheses

Aseptic loosening of the components is probably the most common long-term complication resulting in failure of Total Hip Arthroplasty. The mechanical behaviour of bone under load is one of the contributory causes of loosening encountered at the prosthesis/cement/bone interface. The present study dealt with a series of invitro experiments conducted on epoxy resin models of human hemi-pelves with different commercially available acetabular components implanted in them. These are used for the construction of simplified models of the artificial hip joint (three-dimensional) and of the prosthesis/cement/bone acetabular interface (two-dimensional). Loading conditions for the models included tensioning of the simulated abductor muscles for the hemi-pelvic and femoral loading for the prosthesis/cement/bone interface study. The experimental method employed was real-time holographic interferometry, a stress analysis technique recently used in the biomechanical field, which permitted whole-field simultaneously inspection of deformation patterns. The holographic interferograms were interpreted in a qualitative rather than a quantitative manner. The models do not exactly represent the in-vivo situation. Since this study identified high stresses both in the hip bone as well as in the interface (prosthesis/bone) it is suggested that these stresses are implicated in the mechanical pathogenesis of loosening. The observed changes in stress levels detected in our models could serve as a guide for future designs of acetabular prostheses as well as guide a in surgical techniques.

Submitted Diploma thesis deals with stress-strain analysis of deformation proximal end of femur with applied total hip joint endoprosthesis (replacement) – shortcut type. To identify deformation and tensity (stress) was used computational simulation by method of final elements. Have been created two computational models TEP- type Santori and type DePuy Proxima. Geometry model Santori was created on low level model geometry through the use of X-ray photograph. Principle of geometry model type DePuy Proxima was real Femoral stem endoprosthesis which was scanned on scanner ATOS. Geometry of both these replacements were set up in program Rhinoceros 4.0 and then execute in program CatiaV5R17. Data for geometry model of femur were gained from CT chains. Material model of femur have been crated in two variants. The first one looks at structure bone tissues and the second one were created by Gruen´s zones. Femoral Stem was weighted by static equivalent resultant force acting in hip joint. Computational model of system and self solution, including depiction results, was done by ANSYS Workbench 11.0 for four model variants.

Investigation of hip kinematics in adult sports participants during single leg drop landing with chronic groin pain

Thesis (MScPhysio)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: Introduction-Groin injuries are among the top six most cited injuries in soccer and account for 10-18 per cent of all injuries reported in contact sport. Groin pain can result from a variety of pathologies, but according to literature, 63 per cent of groin pain is due to adductor pathology. Objective-The objective of this study was to explore if there are kinematic differences in the hip joint in sports participants with groin pain compared to matched healthy controls. Study design A cross sectional, descriptive study was conducted. Study setting-The study was conducted at the FNB -3D motion analysis laboratory at the University of Stellenbosch, South Africa. Outcome variables-The dependent variables included hip kinematics in the sagittal, frontal and transverse planes at foot strike, lowest vertical point of the pelvis and total range of hip motion during a single leg drop landing. Methodology-The study sample comprised 20 male club level soccer-and, rugby players, running and cycling participants between the ages of 18-55 years of age. Ten of the subjects had chronic groin pain and the other ten were healthy matched controls. An eight-camera Vicon system was used to analyse the kinematics of the hip joint during single leg drop landing. For the purpose of comparison, the data was analysed for participants with unilateral groin pain and matched controls (n=14) and participants with bilateral groin pain and controls (n=6). The full set of data was subdivided for analysis into three distinct sub-groups. Unilaterally injured groin cases (n=7) were matched with seven healthy controls for analysis. Bilaterally injured groin cases (n=3) were matched with three healthy controls. Results-Cases with unilateral groin pain at initial contact had significantly more abduction of the hip joint when compared to controls (p<0.05). The effect size of this difference was large (0.94). Cases with unilateral groin pain also demonstrated greater hip internal rotation while the controls had external rotation (p<0.05) during a drop landing activity. Bilaterally injured groin cases landed with significantly (p=?) greater ranges of hip flexion as well as in significantly (p=?) more hip abduction during a drop landing activity. They also demonstrated greater total range of motion in the frontal plan when compared to controls. Groin pain cases overall demonstrated greater ranges of motion and tended to land in more abduction compared to controls. Conclusion-This study found that during a single leg drop landing, sports participants with unilateral chronic groin pain landed with significantly greater hip abduction and exhibited larger total range of motion in the transverse plane, which may indicate impaired stability of the hip complex when compared to controls. / AFRIKAANSE OPSOMMING: Inleiding-Liesbeserings is een van die top ses mees prominente sokker beserings. Die agt kamera Vicon sisteem was gebruik om die kinematika van die heupgewrig te analseer tydens een been landing. Vir die doel om ‘n vergelyking te kan maak, was die data geanaliseer van deelnemers met unilaterale liespyn en die vergelykende groep sonder liespyn (n=14) en deelnemers met bilaterale liespyn en hulle vergelykende groep sonder liespyn (n=6).. Die volledige stel data was onderverdeel in drie afsonderlike sub groepe. Vir die analiese was unilaterale liesbeserings (n=7) vergelyk met sewe deelnemers sonder liespyn in die kontrolegroep. Deelnemers met bilaterale liesbeserings (n=3) was vergelyk met drie in die kontrolegroep. Resultate-Die deelnemers met unilaterale liespyn het met eerste kontak beduidend meer abduksie van die heupgewrig gehad in vergelyking met die kontrolegroep (p<0.05). Die effek van hierdie verskil was groot (0.94). Die deelnemers met unilaterale liespyn het ook ‘n grooter interne rotasie getoon, terwyl die kontrole groep meer eksterne rotasie gedemonstreer het (p<0.05) met landing. Deelnemers met bilaterale liespyn het beduidend (p=?) meer heup fleksie en abduksie omvang van beweging tydens landing. Hulle het ook ‘n groter totale heup omvang van beweging in die frontale vlak gehad in vergelyking met die kontrolegroep. Deelnemers met liespyn het oor die algemeen ‘n grooter omvang van beweging getoon, en was geneig om met meer abduksie van die heup te land as die kontrolegroep. Gevolgtrekking-Die studie toon dat deelnemers met kroniese unilaterale liespyn, tydens een been landing, beduidende meerheup abduksie toon en dat die heup in die transverse vlak meer totale omvang van beweging gebruik wat kan dui op onstabiliteit in die heupkompleks in vergelyking met die kontrolegroep.

Total hip joint prostheses made of CoCrMo heads versus ultra high molecular weight polyethylene (UHMWPE) cups have a limited lifetime, mainly due to the wear of the UHMWPE cups as a result of high friction between the articulating surfaces leading to osteolysis and implant loosening with revision surgery becoming inevitable in more active patients. Tribology plays an important role in developing the design, minimizing wear and reducing friction of hip joint prostheses in order to improve their long-term performance, with good lubricating properties. Metal-on-metal hip resurfacing prostheses have shown significantly lower wear rates compared with conventional metal-on-polyethylene implants and thus osteolysis is potentially reduced leading to increased lifetime of the prosthesis. Nevertheless, excessive wear of metal-on-metal joints leads to metal ion release, causing pseudo-tumours and osteolysis. An alternative approach to such bearings is the use of newly developed carbon fiber-reinforced poly-ether-ether-ketone (CFR PEEK) acetabular cups articulating against ceramic femoral heads due to their better wear resistance compared to UHMWPE. In this study, therefore, friction and lubrication properties of large diameter, as cast, Co-Cr-Mo metal-on-metal hip resurfacing implants with various diameters and clearances have been investigated and compared to those of the newly developed zirconia toughened alumina (ZTA) ceramic femoral heads articulating against carbon fiber reinforced poly-ether-ether-ketone (CFR PEEK) acetabular cups with different diameters and clearances. Friction hip simulator was used to measure frictional torque and then friction factors were calculated along with Sommerfeld numbers leading to Stribeck analysis and hence the lubricating mode was also investigated. This involved using lubricants based on pure bovine serum (BS) and diluted bovine serum (25 vol. %BS+75 vol. %distilled water) with and without carboxymethyl cellulose (CMC) (as gelling agent). Standard Rheometer was used to measure lubricant viscosity ranged from 0.0014 to 0.236 Pas at a shear rate of 3000 . Pure bovine serum, diluted bovine serum without CMC and with CMC (25BS+75DW+0.5gCMC and +1gCMC) showed pseudoplastic flow behaviour up to shear rate of ~139 s⁻¹ above which a Newtonian flow with significant increase in shear stress was observed. The viscosity flow curves for the 25BS+75DW+2gCMC, +3.5gCMC and +5gCMC showed only shear thinning up to a shear rate of 3000 . The shear rate application modified the flow behaviour of bovine serum from a pseudoplastic to a Newtonian flow depending on its purity and CMC content. This will cause a different frictional behaviour depending on joint diameter and clearance, as seen in this work. The experimental data were compared with theoretical iv predictions of the lubricating regimes by calculating theoretical film thickness and lambda ratio. The metal-on-metal Biomet ReCaps showed similar trends of Stribeck curves, i.e. friction factors decreased from ~0.12 to ~0.05 as Sommerfeld numbers increased in the range of viscosities ~0.001-0.04Pas indicating mixed lubrication regimes above which the friction factor increased to ~0.13 at a viscosity of 0.236Pas. The Stribeck analyses suggested mixed lubrication as the dominant mode with the lowest friction factor in the range ~0.09 - ~0.05 at the physiological viscosities of ~0.01 to ~0.04 Pas and that such joints can be used for more active patients as compared to the conventional total hip replacement joints with 28mm diameter. The Stribeck curves for all ZTA ceramic-on-CFR PEEK components illustrated a similar trend with BS fluids showing higher friction factors (in the range 0.22-0.13) than the diluted BS+CMC fluids (in the range 0.24-0.05). The friction tests revealed boundary-mixed lubrication regimes for the ZTA ceramic-on-CFR-PEEK joints. The results, so far, are promising and suggest clearly that the newly developed ZTA ceramic femoral heads articulating against CFR PEEK cups have similar friction and lubrication behaviour at optimum clearances to those of currently used metal-onmetal hip resurfacing implants at the range of viscosities 0.00612 to 0.155Pas. These results clearly suggest that the ZTA ceramic-on-CFR-PEEK joints showed low friction at the physiological viscosities of ~0.01Pas in the range ~0.1-0.05, suggesting that these novel joints may be used as an alternative material choice for the reduction of osteolysis. The result of this investigation has suggested that the optimum clearance for the 52mm diameter MOM Biomet ReCaps could be ~170μm. However, 48 and 54mm joints showed lower friction due to clearances to be >200μm. For the 52mm ZTA ceramic-on-CFR-PEEK joints the optimum clearance seems to be ≥ 630μm radial clearance. These results suggested that increased clearance bearings have the potential to generate low friction and hence no risk of micro- or even macro-motion for the ceramic-on-CFR-PEEK joints. This study found no correlation between theoretical predictions and experimental data for all metal-onmetal and ZTA ceramic-on-CFR PEEK bearings at the physiological viscosity (0.0127Pas). However, at lubricant viscosity of 0.00157Pas, the theoretical prediction of lubrication regime correlated well with the experimental data, both illustrating boundary lubrication. As expected, a decrease in viscosity resulted decrease in the film thickness.
