Research, Design and Validation of a Cognitive Aid to Support the Reprocessing of Flexible Endoscopes

abstract: The objective of this project was to evaluate human factors based cognitive aids on endoscope reprocessing. The project stems from recent failures in reprocessing (cleaning) endoscopes, contributing to the spread of harmful bacterial and viral agents between patients. Three themes were found to represent a majority of problems: 1) lack of visibility (parts and tools were difficult to identify), 2) high memory demands, and 3) insufficient user feedback. In an effort to improve completion rate and eliminate error, cognitive aids were designed utilizing human factors principles that would replace existing manufacturer visual aids. Then, a usability test was conducted, which compared the endoscope reprocessing performance of novices using the standard manufacturer-provided visual aids and the new cognitive aids. Participants successfully completed 87.1% of the reprocessing procedure in the experimental condition with the use of the cognitive aids, compared to 46.3% in the control condition using only existing support materials. Twenty-five of sixty subtasks showed significant improvement in completion rates. When given a cognitive aid designed with human factors principles, participants were able to more successfully complete the reprocessing task. A human factors systems approach to exploring vehicle rollaway

Failure of a parked unattended vehicle to remain stationary, otherwise known as vehicle rollaway, can result in property damage, injury or even fatality. Although the incidence of vehicle rollaway may be under reported, around 8% of drivers and 13% of Approved Driving Instructors surveyed indicated they had experienced a vehicle rollaway event. Unlike previous studies which focused only on the mechanical factors that may contribute to this phenomenon, the research presented in this thesis employed a more comprehensive, systems approach to explore additional factors related to the driver s interaction with the parking brake system at various interface levels. A mixed methods strategy collated data through two online surveys and three observational studies to explore the organisational, mechanical and driver related factors identified in a fault tree framework. The results indicated that current driver practice and interaction with the parking brake system may be contrary to legislative requirements and manufacturer s instruction. The findings suggested that a past experience of vehicle rollaway or parking brake system failure, had a statistically significant influence on whether the driver complied with the recommended practice. Driver interaction and the holding capability of the parking brake system were observed in 53 vehicles parked on three test gradients. The observations indicated that drivers were able to apply sufficient force to the parking brake lever to hold the vehicle stationary and that an additional degree of confidence in the system was provided by parking in gear. But, after driving a short commuting route, when the vehicle was parked with the parking brake lever applied to the lowest position to hold the vehicle and a gear was not selected, 63% of vehicles fitted with disc brakes rolled as the temperature returned to ambient. Discussion relates to the organisational, driver related and mechanical components of the parking brake system and in reference to Reason s Swiss Cheese model, considers how latent failures within the defensive layers of the system can contribute to rollaway. The research findings contributed to a change in UK driving standards and since 2015, drivers are recommended to park in gear at all times to reduce the risk of rollaway. This recommendation is likely to require a change in practice for up to 80% of Approved Driving Instructors who would not normally instruct new drivers in this way. Although this research focused on the manually operated parking brake system, the studies have uncovered results that can contribute to knowledge and are applicable to interaction with electronic parking brake systems. As parking brake systems develop, the Human Factors systems approach can be applied retrospectively and proactively to explore that interaction and prevent passenger vehicle rollaway.

An Android and Visual C-based controller for a Delta Parallel Robot for use as a classroom training tool

This report will show the development of a Delta Parallel robot, to aid in teaching the basics of robotic motion programming. The platform developed will be created at a fraction of the cost of conventional commercial training systems. This report will therefore show the development procedure as well as the development of some of the example training material. The system will use wireless serial data communication in the form of a Bluetooth connection. This connection will allow an Android tablet, functioning as the human-machine interface (HMI) for the system, to communicate with the motion controller. The motion controller is based in the C environment. This will allow future development of the machine, and allow the system to be used on an integral level, should the trainers require an in depth approach. The motion control software will be implemented on a RoBoard, a development board specifically designed for low- to mid-range robotics. The conclusion of this report will show an example task being completed on the training platform. This will demonstrate some of the basic robotic motion programming aspects which include point to point, linear, and circular motion types but will also include setting and resetting outputs. Performance parameters such as repeatability and reproducibility are important, as it will indirectly show the level of ease with which the system can be manipulated from the software. Finally, the results will be briefly discussed and some recommendations for improvements on the training system and suggestions for future development will be given.
