ComparaÃÃo da infiltraÃÃo apical entre canais obturados com guta-percha/AH Plus e o Sistema Resilon/Epiphany quando submetidos a duas tÃcnicas de obturaÃÃo / Comparison of apical leakage between canals filled with gutta-percha/AH Plus and the Resilon/Epiphany System, when submitted to two filling techniques

CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / O propÃsito deste estudo in vitro foi comparar o nÃvel de infiltraÃÃo apical entre guta-percha/AH Plus (GP) e Sistema Resilon/Epiphany (SRE) quando submetidos a duas tÃcnicas de obturaÃÃo, e observar, atravÃs do uso de Microscopia Ãptica (MO), a adaptaÃÃo destes materiais Ãs paredes dentinÃrias em cortes transversais realizados a 4mm do Ãpice. Para a avaliaÃÃo da infiltraÃÃo, 70 raÃzes de dentes humanos extraÃdos foram instrumentadas e divididas aleatoriamente em quatro grupos experimentais de acordo com o material e tÃcnica empregados (condensaÃÃo lateral e tÃcnica HÃbrida-TH) e dois grupos controles (positivo e negativo). ApÃs sete dias em estufa (37ÂC, umidade de 100%), as raÃzes foram imersas em tinta nanquim (7 dias) e diafanizadas. A infiltraÃÃo linear foi medida atravÃs do programa NIH imageJ. Utilizou-se o teste exato de Fisher, para a comparaÃÃo entre percentual de nÃo-infiltraÃÃo e teste nÃo-paramÃtrico de Mann-Whitney, para testar o comprimento de infiltraÃÃo, quando esta existiu. O valor de p<0,05 foi considerado ser estatisticamente significante. Na presenÃa de infiltraÃÃo, nÃo houve diferenÃa entre as tÃcnicas de obturaÃÃo empregadas (p>0,05), mas houve diferenÃa estatisticamente significante quando o SRE foi comparado à GP (p<0,05), que apresentou maior infiltraÃÃo. Concluiu-se que a infiltraÃÃo com o SRE ficou confinada ao terÃo apical e a TH pode ser utilizada para termoplastificaÃÃo do SRE com resultados satisfatÃrios. Observou-se atravÃs da anÃlise em MO dos 20 dentes restantes, obturados da mesma forma que os grupos experimentais (cinco dentes para cada grupo), que o SRE apresentou muitas Ãreas de desadaptaÃÃo entre o material e as paredes dentinÃrias, fato que justificaria a infiltraÃÃo ocorrida com esse material. / The purpose of this study was to compare the level of apical leakage between canals filled with gutta-percha/AH Plus (GP) and the Resilon/Epiphany System (RES), when submitted to two filling techniques, and to observe, through the use of Optic Microscopy (OM), the adaptation of these materials to the dentinal walls in carried through transversal cuts to 4 mm of the apex. For the evaluation of leakage, seventy extracted human teeth were instrumented and randomly divided into 4 experimental groups in accordance with the material and technique used (lateral condensation and Hybrid technique - HT), and two control groups (positive and negative). After 7 days in an oven (37 C, humidity of 100%), the teeth were immersed in Indian ink and cleared. Leakage was measured by the NIH imageJ program. The exact Fisher test was used to make the comparison between the non-leakage percentages and the non-parametric Mann-Whitney test to measure the leakage when there was any. The value p<0.05 was considered to be statistically significant. With regard to the presence of leakage, there was no difference between the filling techniques (p>0.05), but there was a statistically significant difference when RES was compared with GP (p<0.05), which leaked more than RES. With RES, leakage was confined to the apical third and HT could be used to thermo-plasticize RES with satisfactory results. It was observed through the analysis in OM of 20 remaining teeth, filled in the same way that the experimental groups (5 teeth for each group), that the RES presented many areas mismatch between the material and the dentinal walls, what it would justify occurred leaking with this material.

âInfluÃncia da soluÃÃo irrigadora e do material obturador na infiltraÃÃo apical de dentes tratados endodonticamente â avaliaÃÃo pelo mÃtodo de filtraÃÃo de fluidosâ / Irrigation Solutions and Root Canal Filling Material Influence in Apical Leakage of Endodontically Treated Teeth â Fluid Filtration Analysis.

CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / Um dos objetivos do tratamento endodÃntico à promover o reparo na regiÃo periapical, a partir do uso de materiais e tÃcnicas que buscam a assepsia do canal radicular. Assim, na Endodontia diversas soluÃÃes irrigadoras tÃm sido aplicadas com as principais funÃÃes de lubrificaÃÃo das paredes dentinÃrias, dissoluÃÃo de matÃria orgÃnica, remoÃÃo de microrganismos e de smear layer. O hipoclorito de sÃdio à a substÃncia mais usada, porÃm possui algumas desvantagens, como a alta toxicidade tecidual. Outras soluÃÃes irrigadoras alternativas tÃm sido propostas, como a clorexidina e o vinagre de maÃÃ, que possui adequada biocompatibilidade e capacidade de remoÃÃo de smear layer. A obturaÃÃo endodÃntica apresenta objetivos tÃcnicos, baseados no completo selamento do sistema de canais radiculares, assim como biolÃgicos, promovendo o fechamento do forame apical atravÃs da deposiÃÃo de tecido mineralizado. Por muito tempo, a guta-percha associada a um cimento tem sido usada, entretanto, com o advento dos materiais adesivos na clÃnica odontolÃgica, alguns surgiram no mercado com emprego em Endodontia, sendo o mais conhecido o sistema Resilon/Epiphany. Assim, o objetivo deste estudo foi avaliar , a partir do mÃtodo de filtraÃÃo de fluidos, a influÃncia da soluÃÃo irrigadora e das associaÃÃes entre materiais obturadores (cones e cimentos) na infiltraÃÃo apical. Foram instrumentados e obturados 112 prÃ-molares inferiores humanos extraÃdos, divididos inicialmente em 2 grupos controle (n=8) e 8 grupos experimentais (n=12) de acordo com a soluÃÃo irrigadora (H) hipoclorito de sÃdio, (V) vinagre de maÃÃ, e materiais obturadores guta-percha/AH Plus (GA), Resilon/Epiphany (RE), guta-percha/Epiphany (GE) e Resilon/AH Plus (RA). Os resultados nÃo mostraram diferenÃas estatÃsticas em relaÃÃo Ãs soluÃÃes irrigadoras usadas (p>0,05), porÃm foi observado que os grupos obturados com a associaÃÃo Resilon/AH Plus (H/RA e V/RA) apresentaram significantemente mais infiltraÃÃes do que os demais grupos. Concluiu-se que a infiltraÃÃo apical foi influenciada apenas pelo material obturador, independente da soluÃÃo irrigadora. / One of the aims of endodontic treatment is to promote the repair in the periapical region, from technical procedures including chemomechanical preparation, represented by the instrumentation associated to irrigation and aspiration, in addition to filling the root canal system. Thus, in Endodontics several irrigation solutions have been employed with the main functions of dentin walls lubrication, organic matter dissolution, removal of microorganisms and smear layer removal. Sodium hypochlorite is the most used substance, however, it possesses some disadvantages, such as high tissue toxicity. Others irrigation solution alternatives have been proposed, such as chlorhexidine and apple vinegar, which has suitable biocompatibility and ability to remove the smear layer. The root canal filling presents technical objectives, focused on the full sealing of the root canal system, as well as, biological, enabling closure of the apical foramen, through the deposition of mineralized tissue. For many years, gutta-percha associated with a sealer has been used, however, with the advent of adhesive materials in dental clinic, some hit the market with employment in Endodontics, the most known Resilon/Epiphany system. Thus, the objective of this study was to assess, by fluid filtration method, the influence of irrigation solution and the filling materials (cones and sealer) association in the apical root leakage. One hundred twelve lower pre-molars human teeth were instrumented and filled, being initially divided into two controls groups and 8 groups (n = 12), according to the irrigation solution, (H) sodium hypochlorite, (V) apple vinegar, and root canal filling materials, gutta-percha/AH Plus (GA), Resilon/Epiphany (RE), gutta-percha/Epiphany (GE) and Resilon/AH Plus (RA). The results have showed no statistical difference in relation to the irrigation solutions used (p> 0.05), nevertheless, it was found that the groups filled with Resilon/AH Plus (H/RA and V/RA) association had significantly more leakage than the other groups (p <0.05). It was concluded that the apical leakage were influenced only by the filling material regardless the irrigation solution.
