Interdentala hjälpmedels effekt på dental plack och inflammation kring orala implantat : En systematisk litteraturstudie / Interdental aids effect on plaque and inflammation around oral implants : A systematic review

Bakgrund: Implantat kan användas för att fylla ut en tandlucka men kan påverkas av mucosit och peri-implantit som har samma etiologi, plack. Interdentala hjälpmedels effekt på plack och inflammation är baserat på klinisk erfarenhet och inte vetenskaplig evidens. Syfte: Syftet med studien var att granska publicerade vetenskapliga studier, för att identifiera interdentala hjälpmedels effekt på dentalt plack och inflammation kring orala implantat. Metod: En systematisk litteraturstudie utfördes i Pubmed, Medline och Scopus efter artiklar som studerade interdentalborstar, tandtråd, tandsticka eller water floss avseende förekomsten av plack, bleeding on probing (BOP), mucosit eller peri-implantit på vuxna människor. Totalt fanns 2174 titlar och fyra artiklar inkluderades. Studien följde PRISMA checklistan och SBU-granskningsmallar användes för kvalitetsgranskning samt bedöma evidensstyrka enligt GRADE. Resultat: Tandtråd minskar BOP med 33,3% men utgör risk för mucosit/peri-implantit ifall fibrer fastnar men avlägsnas dessa reducerar fickdjupet 2,2 mm. Water floss minskar BOP med 81,8% och är 2,45 gånger effektivare än tandtråd samt kan avlägsna 40,56% av plack. Detta är baserat på få studier med delvis bristande studiedesign och enligt GRADE bedöms evidensen som otillräcklig. Konklusion: Fler kliniska studier inom ämnet behövs för att stärka evidensen. / Background: Implants can fill tooth gaps but can be affected by mucosit and peri-implantitis cussed by plaque. Interdental aids effect on plaque and inflammation is based on clinical expertise and not scientific evidence. Objective: The objective was to review published studies, to identify interdental aids effect on plaque and inflammation around oral implants. Method: A systematic review was conducted in Pubmed, Medline and Scopus after articles that studied interdental brushes, dental floss, toothpick or water floss for presence of plaque bleeding on probing (BOP), mucosit or peri-implantitis on adults. Totally 2172 articles where found, four where included. The study followed PRISAM checklist. SBU templates where used for quality assessment and determine evidence according to GRADE. Result: Dental floss reduces BOP by 33,3% but can be risk for mucosit/peri-implantitis if fibers get stuck but if removed pocket depth reduces 2,2 mm. Water floss reduces BOP by 81,8% and is 2,45 times more efficient then dental floss and removes 40,56% of plaque. This is based on few studies with deficient study design and the evidence is insufficient. Conclusion: More clinical studies are needed to elevate evidence.

A Boot Camp Approach to Remediating Interdental /s/ in a School-Aged Child

The relationship between speech articulation therapy dose (frequency, intensity, duration) and treatment outcomes are poorly understood. Identifying optimal therapy doses for specific articulation disorders is essential to providing efficient clinical care. Recent research indicates that intensive speech therapy doses-known as boot camps-may promote rapid improvement and generalization for certain speech disorders. Therefore the present investigation examined the effects of a boot camp speech therapy approach to remediate interdental /s/ production in an 8-year-old male. The boot camp included two days of speech therapy involving visual, tactile, and auditory feedback approaches. Therapy was administered 5.5 hours per day across morning and afternoon sessions. Treatment outcomes were evaluated using auditory-perceptual ratings of pre- and post-treatment word pairs. The results indicated that /s/ production improved significantly immediately following the boot camp and improvements were sustained at one week post-treatment (p < 0.05). Medial and final /s/ productions improved more so than the initial /s/ productions. These findings suggest that the speech therapy boot camp approach may be effective for certain individuals with speech sound disorders. Future research should explore dose-response relationships among speech articulation therapy dose in other children with speech sound disorders.

O estudo epidemiológico transversal randomizado objetivou avaliar condições de saúde bucal nos competidores dos XV Jogos Pan-Americanos (JPA) e III Jogos Parapan-Americanos (JPPA), 2007. Foram enviados convites para 5.662 atletas (JPA) e 1.300 (JPPA). Radiografias panorâmicas digitais (RPD) foram utilizadas para o exame de triagem nos 2 eventos, e nos JPPA, os atletas também foram submetidos à avaliação do sangramento gengival interdental (SI) através de uma versão modificada do Índice de Sangramento Interdental de Eastman (EIBI). Foram obtidas RPDs de 410 atletas dos JPA, média de idade 24,38 (dp5,35), 55% homens; e de 118 dos atletas dos JPPA, média de idade de 32,3 (dp9,53), 77,97% homens. 121 competidores (JPPA) foram avaliados para SI: 78,51% homens, média de idade 32,6(dp9,6), e foram separados em grupos (G), conforme sua deficiência física: GI c/ deficiência visual (DV), com 2 subgrupos: GI-a: DV tardia e GI-c-: DV congênita/precoce; GII- deficiência de membro superior; com 1 subgrupo: GII-t: deficiência/ausência bilateral; GIII- deficiência de membro inferior (grupo controle). As RPDs foram examinadas por 1 examinador com o Kodak Dental Imaging(v6.7). A frequência e a distribuição do SI foram calculadas, e os grupos foram comparados. Conclusões: Os dados das 2 populações de atletas mostraram que há uma grande variação na saúde bucal entre os indivíduos avaliados. Diversas condições com potencial de influenciar o desempenho esportivo dos atletas foram detectadas através de radiografias panorâmicas digitais, sugerindo que um programa de saúde bucal deve ser incluído como parte da preparação destes indivíduos.A avaliação da frequência e distribuição de sangramento gengival interdental em uma população de atletas que competiu nos III Jogos Parapan-Americanos, revelou que o tipo de deficiência ou limitação física dos competidores é um fator que influencia na saúde gengival desses indivíduos.
