Recognizing the Impact of Obstructive Sleep Apnea in Patients With Asthma

Krishnaswamy, Guha, Girish, Mirle, Guha, Bhuvana, Ballard, Robert, Vyas, Harsha, Chi, David S. 01 October 2005 (has links)
The coexistence of asthma and obstructive sleep apnea (OSA) in a given patient presents a number of diagnostic and treatment challenges. Although the relationship between these 2 diseases is complex, it is clear that risk factors such as obesity, rhinosinusitis, and gastroesophageal reflux disease (GERD) can complicate both asthma and OSA. In the evaluation of a patient with poorly controlled asthma, it is important to consider the possibility of OSA. The most obvious clues are daytime sleepiness and snoring, but the definitive diagnosis is made by polysomnography. Management of OSA may include weight loss and continuous positive airway pressure (CPAP). Surgical intervention, such as uvulopalatopharyngoplasty, may be an option for patients who cannot tolerate CPAP. Management may include specific therapies directed at GERD or upper airway disease as well as modification of the patient's asthma regimen.

Hepatitis C and Dilated Cardiomyopathy

Iskandar, Said B., Loyd, Steve, Downs, Christopher J., Roy, Thomas M. 01 January 2004 (has links)
In North America, viral infection is the most common cause of myocarditis. A number of common viral infections have been implicated. With the increased identification of patients with hepatitis C, there is recent evidence derived from myocardial molecular biology studies suggesting an association between HCV infection and the development of myocarditis. This may be followed by dilated cardiomyopathy. Primarily researchers overseas have supported the recognition of this association. We recently cared for a patient who appears to fulfill the clinical criteria of hepatitis C-induced cardiomyopathy.

Cranial Nerve VIII Involvement in a Patient With Progressive Systemic Sclerosis

Iskandar, Said B., Loyd, Stephen, Roy, Thomas M. 01 January 2004 (has links)
Systemic sclerosis is a multisystem disorder characterized by abundant fibrosis of the skin, blood vessels, and visceral organs. Cranial nerve involvement is an uncommon feature of this connective tissue disorder, and when it occurs it is the trigeminal nerve that is primarily affected. We report an elderly woman who presented with sensorineural hearing loss and was then diagnosed with the CREST syndrome of progressive systemic sclerosis (calcinosis cutis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasia). Involvement of the eighth cranial nerve with scleroderma and CREST syndrome is rare, but appears to be the cause of sensorineural hearing loss in our patient.
