Instituto de Pesquisa Clínica Evandro Chagas, Rio de Janeiro, RJ, Brasil / A leishmaniose cutânea (LC) se caracteriza por lesões granulomatosas, comumente ulceradas, e a esporotricose é importante diagnóstico diferencial. O presente trabalho objetivou determinar alterações histopatológicas úteis para essa diferenciação e relatar um caso de LC em que houve hiperplasia pseudoepiteliomatosa exuberante que foi confundida com carcinoma de células escamosas. Foi realizado um estudo das alterações histopatológicas de 119 lesões cutâneas ativas de esporotricose em lâminas coradas pela hematoxilina e eosina e, posteriormente, uma comparação entre as alterações em 171 lesões ativas de LC e 97 de esporotricose em que não se encontrou o microorganismo ao exame histopatológico. Todos os casos foram diagnosticados pelo isolamento do agente em cultura A esporotricose se caracterizou histopatologicamente por uma dermatite difusa granulomatosa supurativa com necrose. A LC apresentou granulomas dos tipos \201Cclareira de Montenegro\201D e \201Ctuberculóide\201D e degeneração da matriz extracelular de forma significativamente mais freqüente do que a esporotricose. A esporotricose apresentou granulomas dos tipos \201Csupurativo\201D e \201Cestrelado\201D, células gigantes de diferentes tipos, granulomas associados a tecido de granulação e abscessos não associado aos granulomas de forma significativamente mais freqüente do que a LC. Uma análise múltipla dessas alterações por regressão logística demonstrou que elas podem distinguir as duas doenças corretamente em cerca de 90% dos casos. Assim, o exame histopatológico pode fornecer informações importantes na condução do diagnóstico diferencial entre a LC e a esporotricose. A hiperplasia pseudoepiteliomatosa na LC pode simular um carcinoma de células escamosas / Cutaneous leishmaniasis (CL) is characte rized by granulomatous lesions, often ulcerated, and sporotrichosis is an important differential diagnosis. This study aimed to determine histopathological changes that coul d assist in this differentiation and to report a case of CL which presented ex uberant pseudoepitheliomatous hyperplasia thas was mistaken for squamous cell carc inoma. A study of histopathological changes of 119 active cutaneous lesions of sporotrichosis in hematoxylin-eosin stained slides was conduct ed, and then a comparison between the changes in 171 CL and 97 sporotrichosis active lesions in which the microorgani sm was not found on histopathological examination. All cases were diagnosed by is olation of the agent in culture. Sporotrichosis was histopathologically characteri zed by a diffuse suppurative granulomatous dermatitis with necrosis. CL showed granuloma of "Montenegro clearing" and "tuberculoid" types and degeneration of extracellular matrix significantly more frequent than sporotrichosis. Sporotrichosis presented granulomas of "suppurative" and "stellate" types, giant cells of different types, granulomas associated with granulati on tissue, and abscesses not associated with granulomas significantly more frequent than CL. A multip le analysis of these changes by logistic regression demonstrated that they can correctly distinguish the two diseases in about 90% of cases. Thus, the histopathologic al examination can provide important information in guiding the differential diagnosis between CL and sporotrichosis.
