Exploration of needs, problems and living experiences of older persons in Uganda: implications for policy and decision making

Philosophiae Doctor - PhD / The purpose of the study was to explore the needs, problems and living experiences of the older persons in Uganda and their implications for decision making and policy. The study was an exploratory descriptive cross-sectional study that involved triangulation of both qualitative and quantitative approaches. It was guided by five objectives, which included: exploring the living experiences of Uganda' s older persons living in both rural and urban settings; identifying the barriers to these people's effective participation in society; establishing the mechanisms that they use to cope with the barriers; evaluating the extent to which their needs are addressed by policy; and proposing policy strategies needed to address their plight. The study sample consisted of two purposively selected sub samples of 165 older persons for qualitative data and 50 key informants for quantitative data. The key informants were selected from Ministries of Gender, Labour and Social Development; Urban, Housing and Physical Planning; Agriculture; Health; Education and Sports, Public service, and nongovernmental organizations dealing with the older persons in Uganda. Data were collected using in depth interviews with older persons, self administered questionnaire to key informants and documentary review. The qualitative data was analyzed using content analysis including documentary review, while the quantitative data was analysed using the frequency option of the descriptive method of SPSS, and graphical method of the Excel programme. Results show that the living experiences of most of Uganda's older persons are characterised by pathetic economic, health, social, housing and accommodation, water supply, and sanitation conditions. The barriers to the effective participation of these people in society take the form of age-related prejudices, problems, and limitations faced at personal, household and community levels. They basically include constraints to the physical fitness, healthcare, economic status, food security and nutrition, and realization of accommodation and housing conditions desired by these people to live a life by which they can effectively participate in society. Other barriers include: large numbers of grandchildren most of whom are orphans left behind by the older person's children claimed by the HIV/AIDS pandemic and community members ignoring them as helpless people who have outlived their usefulness. Accordingly, the coping mechanisms used by these people were established as the psychological, physical, healthcare, and economic ways by which they deal with each of the aforementioned barriers. A review of development policies such as PEAP, PMA, NAADS, HIV/AIDS policies among others revealed that a number of policies and programs in Uganda exclude the older persons from active participation. Findings show further that though the extent to which the Constitution of Uganda government recognizes the plight of the older persons is appreciable, it is largely insufficiently translated into policy action by which the plight can be effectively addressed. A number of strategies were therefore proposed to help address this insufficiency, thereby solving the needs and attending to the barriers facing these people in an effective policy manner. The strategies focused on improving older persons' physical fitness, access to healthcare, economic capacity, food security and nutrition, and housing and accommodation. Other strategies focused on dealing with the impacts of HIV/AIDS and community prejudices held against older persons. The study was concluded by observing that although the quality of the living experiences of Uganda's older persons can be improved by dealing with the barriers faced by these people, the mechanisms that they use to do so are not adequate. It was also noted that there was need to translate the constitutionally recognised plight of the older persons into effective policy action. Consequently, it was recommended that the proposed policy strategies should be adopted and translated into the needed policy action. Further research was also recommended into the legal implications and funding implications of the proposed policy strategies.

Memórias autobiográficas e cartas de Claire Lange: uma análise fenomenológica / Autobiographical memories and letters from Claire Lange: a phenomenological analysis

Esta pesquisa se refere a documentos autobiográficos (cartas e memórias) acerca da experiência de imigração escritos por Claire Reuge Lange. Claire chegou ao Brasil em Agosto de 1887 e, enquanto aqui viveu, escreveu várias cartas para seus pais adotivos, que permaneceram na Alemanha. As memó-rias de Claire Reuge Lange tratam da instauração da colônia de Brüderthal no município de Joinville, em Santa Catarina. O objetivo desta pesquisa é o es-tudo das 13 cartas escritas por Claire e de suas memórias, nas quais procu-ramos analisar os impactos da mudança de contexto nas vivências da autora. Para tanto, realizamos uma análise das vivências por ela narradas nas cartas, levando em conta a função psicológica da correspondência epistolar enquanto vínculo do imigrante com o seu mundo de origem e seu passado. Focamos a dinâmica psicológica e as vivências relatadas pela autora, à luz do texto O Estrangeiro (2010) de Alfred Schutz, e à luz da fenomenologia de Edmund Husserl e de Edith Stein. Nos relatos de Claire foram encontradas vivências perceptivas, afetivas, vivências de temporalidade e vivências espirituais de fé, concomitante às categorias de análise do texto O Estrangeiro de Schutz. No que diz respeito a estas categorias, evidenciamos três grupos de vivências: as primeiras vivências do estrangeiro como recém-chegado; a crise: as vivên-cias de choque de confiança no pensar habitual e os mecanismos de supera-ção da crise - emergindo a vivência espiritual como recurso para superação desta. Conclui-se pela possibilidade de superação da crise através do com-partilhamento de vivências com a alteridade, que ocorre nas cartas ou nas memórias, seja ela a alteridade das relações sociais representadas pelos destinatários das cartas, seja ela a dimensão transcendente do divino. / This research refers to autobiographical documents (letters and memories) about immigration experiences, written by Claire Reuge Lange. Claire arrived in Brazil in August 1887. While she lived here, she wrote many letters to her adoptive parents, who stayed in Germany. Claire Reuge Langes memories are about the Brüderthals colony settling in the city of Joinville, Santa Catari-na. The main objective of this research is the studying of the 13 letters written by Claire and her memories, in which we sought to analyze the impacts of the changes of context, lived by the author. In order to achieve it, we performed an analysis of her living experiences portrayed on her letters, taking into ac-count the psychological function of the epistolary correspondence as immi-grant bound with their original world and past. We focused on the authors psychological dynamics and living experiences based on the text The For-eigner (2010) by Alfred Schutz, and also on the phenomenology by Edmund Husserl and Edith Stein. On Claires stories, perceptive, affective, temporality and faith-spiritual living experiences were found, according to the categories of the analysis over The Foreigner by Schutz. Concerning these categories, we evidenced three groups of experiences: the first living experiences as a new-comer; the crisis: the living experiences of trust shock over the habitual think-ing and the tools of the crisis overcoming arising the spiritual experience as a resource to overcome it. It is concluded through the possibility of overcom-ing the crisis through the sharing of the living experiences with the otherness, which occurs on the letters or memories, either from the social relations rep-resented by the receivers of these letters, or the divine transcending dimen-sion.

THE MIGRATION, WORKING, LIVING AND INTEGRATION EXPERIENCES OF IMMIGRANT LIVE-IN CAREGIVERS IN ONTARIO, CANADA / THE EXERIENCES OF IMMIGRANT LIVE-IN CAREGIVERS IN ONTARIO

This thesis aims to fill in the gaps in our knowledge of the experiences of immigrant live-in caregivers in Canada as a means to better understand the role of social networks and how this is informed by intersectionality theory. More specifically, based on 34 qualitative interviews with current and former live-in caregivers, this dissertation explores the migration, working, living and integration experiences of immigrant live-in caregivers in Ontario, Canada. In particular, I focus on: 1) their experiences with processes of decision making, migration and finding a job with a particular focus on role of social networks in these processes; 2) the impact of the type of care on live-in caregivers’ working and living experiences as well as the role of networks in the process of their adaptation to life in Canada; and 3) their integration experiences (with a particular focus on role of social networks in these processes). Although some researchers relied on some sociological perspectives in their studies focusing on live-in caregivers in Canada, none of the previous studies used these in conjunction with social network theory. My research shows that while social network theory is useful in considering the role of social networks in migration, living and integration experiences of LCP workers, it is not sufficient to come to a complete understanding of these issues. On the basis of my findings, I conclude that one should combine social network theory and intersectionality when exploring such issues with regards to not only LCP workers in particular, but also immigrant workers in general. By bringing such new empirical and theoretical insights, my dissertation contributes to the body of previous research on experiences of LCP workers in Canada and more broadly to the literature on domestic workers in an international context, immigration research focusing on social networks and sociological theory. / Dissertation / Doctor of Philosophy (PhD)

Physical contact between therapist and patient and the frequency of the assistance involves the development of a peculiar type of interpersonal relationship. This research aimed to grasp, phenomenologically, the experience of the physical therapy trainees regarding their relationship with outpatients at a university clinic. It is an exploratory qualitative research, phenomenologically inspired. Eight individual encounters were carried out with undergraduates who are in their seventh, ninth and tenth semesters. After each of the encounters, the researcher wrote a first-person narrative, based on her impressions of the meaning of the lived experience of the participants. A narrative synthesis was then developed in order to capture the meaning of the experience as such. The analysis made it possible to conclude that trainees strive to offer a treatment that includes, not only technique but also attitudes of respect, warmth and welcome. Caring for patients with neurological problems, especially those involving progressive degeneration, is signified by the interns as unrewarding since recovery is impossible and verbal communication is difficult. They also feel distressed, as they perceive themselves emotionally involved with the patients, despite the attempt to maintain a strictly professional approach. On the other hand, in a broad way, this relationship is referred to as an opportunity for new learnings and psychological maturity with a potential for growth both in personal and professional life. / Inspirada no modelo biom?dico de sa?de, a pr?tica da fisioterapia possui caracter?sticas espec?ficas no que se refere ao atendimento aos pacientes do ponto de vista psicol?gico. O contato f?sico e a frequ?ncia das sess?es acarretam o desenvolvimento de um relacionamento interpessoal que esta pesquisa objetivou apreender fenomenologicamente a partir da experi?ncia de alunos de uma faculdade de Fisioterapia. Foram realizados encontros individuais com oito graduandos que cursavam o s?timo, nono e d?cimo per?odos do Curso com foco em suas atividades como estagi?rios no ambulat?rio de uma cl?nica universit?ria. Ap?s cada um desses encontros, a pesquisadora construiu uma narrativa a partir de sua compreens?o sobre os significados da experi?ncia para o participante. Ap?s concluir esta fase, elaborou uma narrativa-s?ntese contendo o conjunto de significados relatados pelos participantes a fim de interpretar o sentido da experi?ncia como tal. Este processo de an?lise possibilitou concluir que os alunos empenham-se em oferecer um tratamento que inclua, al?m das t?cnicas, atitudes de respeito, cordialidade e acolhimento. Desenvolvem uma vis?o do paciente como pessoa e n?o apenas como caso cl?nico, embora tenham sido treinados para um desempenho t?cnico. Por outro lado, sentem-se angustiados ao se perceberem envolvidos emocionalmente, a despeito da inten??o inicial de manterem uma postura estritamente profissional. Atender pessoas que apresentam problemas neurol?gicos, em especial aqueles que implicam progressiva degenera??o, ? significado pelos participantes como pouco gratificante, em fun??o da impossibilidade de recupera??o dos movimentos e da dificuldade de comunica??o verbal. Consideram que os atendimentos possibilitam uma aprendizagem necess?ria e importante que promove amadurecimento psicol?gico com potencial transformador, tanto para a vida pessoal quanto profissional.
