Exploring risk factors associated with potential hearing loss in Namibian Class A mines

Thesis (MAud)--Stellenbosch University, 2015. / ENGLISH ABSTRACT: In developing countries, like Namibia, there is limited data pertaining to the number of individuals with hearing loss and its associated factors. This study aimed to determine the prevalence of potential hearing loss in Namibian Class A mines and to describe the extrinsic and intrinsic factors associated with hearing loss. A cross-sectional design was utilised and data were collected from 132 respondents (mining employees) from five different Class A mines throughout the country. A questionnaire and a retrospective review of respondents’ medical records were utilised to determine the risk factors. The most recent audiogram found in the respondents’ records was used to determine the presence of potential hearing loss. Three definitions of potential hearing loss were used in this study and included all major frequency hearing loss (AFHL), high frequency hearing loss (HFHL) and low frequency hearing loss (LFHL). Potential hearing loss was identified when the pure tone average (PTA) of 0.5, 1, 2, & 4kHz, 0.5, 1 & 2kHz and 4 & 8kHz respectively was greater than 25dBHL in either one or both ears. Chi-square measurements or, where necessary, Fisher’s exact tests, as well as Odds Ratios were used for the analysis of data. In general a significance level of 5% was applied for all analyses. Results indicated the prevalence of potential hearing loss in Namibian mining employees to be 27% and that both extrinsic and intrinsic factors were associated with hearing loss. The extrinsic factors significantly associated with potential hearing loss were both occupational and medical. The occupational factors found to be significant were the number of years employed in whole life >10 years (p=0.012; OR=3.1, 95% CI=1.3-7.9), the number of years employed in current job > 10 years (p=0.01; OR=3.9, 95% CI1.7-8.8) and the non-availability of formal training in prevention of hearing loss (p=0.022; OR=0.3, 95% CI (0.1-0.9). Diabetes was the sole significant extrinsic medical factor (p=0.035, OR=5, 95% CI 1.1-22.1). The only intrinsic factor which was found to be significantly associated with hearing loss was Age, specifically being older than 40 years (p=0.002; OR=3.5, 95% CI 1.6-7.8) and 50 years (p=0.001, OR=5.5, 95% CI1.9-15.8). A multiple logistic regression model of all significant factors found that only no formal training of prevention of hearing loss was found to be significant in the presence of all other factors (p=0.036, OR=0.036, 95% 0.1-0.92). Findings from this study suggest that multiple factors may be associated with potential hearing loss and not just the exposure to hazardous occupational conditions. Recommendations for future research and clinical practice should, therefore, include thorough investigations into the aetiology of hearing loss. As this study focused on Class A mines, it is recommended that future research be conducted in other mines that are not categorised as Class A mines. Hierdie studie het gepoog om die voorkoms van gehoorverlies in Namibiese Klas A myne te bepaal en die ekstrinsieke en intrinsieke faktore wat verband hou met potensiale gehoorverlies te beskryf. 'n Deursnee-ontwerp is gebruik en data is ingesamel uit 132 respondente (mynbou werknemers), uit vyf verskillende Klas A myne regdeur die land. 'n Vraelys en 'n retrospektiewe oorsig van die respondente se mediese rekords is gebruik om die risiko faktore te bepaal. Die mees onlangse oudiogram wat in die respondente se rekords gevind is, is gebruik om die teenwoordigheid van potensiale gehoorverlies te bepaal. Drie definisies van potensiale gehoorverlies is gebruik in hierdie studie, ingesluit al die groot frekwensie gehoorverliese (AFHL), hoë frekwensie gehoorverlies (HFHL) en 'n lae frekwensie gehoorverlies (LFHL). ‘n Gehoorverlies was teenwoordig wanneer die suiwer toon gemiddelde (PTA van 0.5 , 1 , 2, & 4kHz , 0.5, 1 & 2kHz en 4 & 8kHz onderskeidelik , groter was as 25dBHL in een of albei ore. Resultate het aangedui die voorkoms van gehoorverlies in Namibiese mynbouwerknemers tot 27 % was en dat beide ekstrinsieke en intrinsieke faktore ‘n verband toon met potensiaal gehoorverlies. Die ekstrinsieke faktore wat ‘n beduidende verband getoon het met gehoorverlies was albei beroeps- en mediese faktore. Die beroepsfaktore wat betekenisvol was, was die aantal jare diens in hele lewe > 10 jaar ( p = 0,012 ; OR = 3.1 , 95 % CI = 1.3-7.9) , die aantal jare in huidige pos> 10 jaar diens (p = 0,01 ; OF = 3.9 , 95 % CI1.7-8.8 ) en die onbeskikbaarheid van formele opleiding in die voorkoming van potensiaal gehoorverlies (p = 0,022 ; OF = 0,3 , 95 % CI ( 0,1-0,9 ). Diabetes was die enigste beduidende ekstrinsieke mediese faktor (p = 0,035 , OR = 5 , 95 % CI 1,1-22,1 ). Die enigste intrinsieke faktor watbeduidend was en verband hou met gehoorverlies was ouderdom, spesifiek om ouer as 40 jaar ( p = 0,002 ; OF = 3.5 , 95 % CI 1,6-7,8 ) en 50 jaar ( p = 0.001 , OR = 5.5 , 95 % CI1.9-15.8 ) te wees. 'n Veelvuldige regressie model van alle beduidende faktore het bevind dat slegs geen formele opleiding in die voorkoming van gerhoor verlies beduidende was in die teenwoordigheid van al die ander faktore ( p = 0,036 , OR = 0,036 , 95 % 0,1-0,92 ) . Bevindinge van hierdie studie dui daarop dat verskeie faktore geassosieer kan word met gehoorverlies en nie net die blootstelling aan gevaarlike beroepstoestande nie. Aanbevelings vir toekomstige navorsing en kliniese praktyk moet dus 'n grondige ondersoek na die etiologie van gehoorverlies uitvoer. Aangesien hierdie studie gefokus het op die Klas A- myne , word dit aanbeveel dat toekomstige navorsing gedoen word in ander myne wat nie gekategoriseer is as Klas A myne nie. Essays in Development, Environment and Health

This dissertation examines topics related to development, environment and health in developing countries using empirical methods. In the first chapter, I study how developing countries can increase enforcement to reduce subsidy leakage in public programs, by investing in the state capacity to target program beneficiaries. This chapter further attempts to understand how the formal sector and black market respond to a policy that reduces diversion of a subsidized commodity. I explore these questions using the case of a Unique ID-based direct fuel subsidy transfer policy in India. Second chapter focuses on the health and wealth trade off near mineral mining operations in developing countries. Using extensive data on mining, health outcomes and assets from 44 developing countries, this study quantifies the wealth gain and adverse health impact of mineral mining. With a number of empirical strategies, this study shows that, despite high wealth gains, how heavy metal mining significantly increases the level of anemia in women and stunting in children living near mines. In the third chapter, I estimate demand for a water quality diagnostic product -- arsenic testing, when it is offered at a price. I further look into various aspects related to selection, learning and households behavioral response to the information. This study is based on a field experiment in Bihar, India.

The effects of corporate social responsibility on community dispute resolutions in the South African mining sector

Submitted in fulfillment of the requirements for the degree of Doctor of Philosophy: Business Administration, Durban University of Technology, Durban, South Africa, 2017. / The South African mining sector has over the years been perceived to be exploitative and not responsive to the investment interest of immediate stakeholders where they extract (local communities and labour). The perception of non-investment has resulted in amongst others, extensive community and labour unrests leading up to mass killings of mine workers at Marikana by the South African Police. The South African government has enacted policies to guide Corporate Social Responsibility (CSR) for the sector but the historic problems associated with this sector persists. As a result of community concerns around mining CSR, communities are mobilising around civic organisations, with the aim of brining the mining sector to account and fostering community development in their localities. This research explores and analyses if the mining sector adheres to CSR in their ventures and how communities perceive the sector’s activities and CSR approaches. The epistemological foundations of this research are mainly positivist adopting theoretical assumptions of the stakeholder theory. The research methods are however those of mixed methods – quantitative sequential qualitative methods. The subsequent research designs are a survey in the quantitative methods and exploratory interviews in the qualitative methods. Data was gathered using a survey questionnaire in the quantitative methods and recorded phenomenological interviews in the qualitative methods. Data was gathered from a sample of 200 community members in the two sampled mining areas of Marikana (100) and Carletonville (100). The former is a platinum belt and the latter is an old gold mining area. To augment quantitative data from the field survey sequential qualitative data was collected through semi-structured interviews (phenomenological interviews) with mining company executives, government leaders, trade unions leadership as well as community members in Makhado and Tshikondeni mining sites. The researcher encountered various limitations including travels to remote areas with lengthy distances on gravel roads, and respondents’ “over-researched” attitudes especially in Marikana and Makhado. The findings suggest that the mine workers’ location, living and working conditions influence their understanding and therefore definition of CSR. Although numerous CSR definitions emerged in the case of Marikana CSR was equated to basic service provision similar to those provided for by the municipality like houses, schools, roads and clinics. In the case of Carletonville which is a well-developed mining town, CSR meant skills development, local enterprise development and provision of bursaries. It emerged that the majority of community members are not aware of the CSR programmes implemented in their localities. Respondents further cited collusive corrupt activities between the mining companies, traditional authorities and municipal leadership as the main deterrent to CSR and to local economic development. Overall findings suggest that the mining sector is adherent to CSR legislation at the minimal level but this does not meet the needs and expectations of community members. The mismatch between community expectations and their understanding of CSR does not align with government policy on CSR requirement of the mining sector. The study recommends, among others, that bold and decisive government enforcement of penalties, which include consistent revoking of mining licences for non-CSR compliance by mining companies. The thesis also highlights the implications for managers as the rise of civic organisations propels a new engagement approach between mining companies and communities. This new approach would have inherent challenges such as delays in reaching consensus and exposing mining companies to internal community politics. Additionally, the study recommends future research be focused on evaluating constitutional powers of traditional leaders in relation to community interests in mining CSR. Furthermore, research could be conducted to establish previous compliance records on post-mining social commitments and environmental rehabilitation of mining companies in South Africa. This thesis brings to the fore, an illustration of the emergent bargaining power communities has and how it compels mining corporates to engage with communities more consultatively. Failure to do so, result in situations such as Coal of Africa impasse in the Makhado area where losses amounting to billions of Rands are incurred. / D

Developing and evaluating a coaching program to improve safety leadership

Legislators are placing increased pressure on mining companies to improve their safety performance. The importance of safety leadership is highlighted by its role in safety culture and improving safety performance. The aim of this study was to develop and evaluate the impact of a coaching program on safety leadership. The main constructs namely safety culture, safety leadership and coaching was conceptualised along the humanistic paradigm, with theoretical definitions and models. In this study, safety culture is employees’ shared attitudes, beliefs, perceptions and values about safety that affect their behaviour in the workplace. Safety leadership is the interpersonal influence that a leader exercises to achieve the organisation’s safety performance goals. Coaching is an interpersonal interaction that aims to improve individual performance through increased selfawareness and action plans. A theoretical model was developed to explain the elements that constitute effective safety leadership. A coaching program was developed based on executive coaching and leadership development principles. The empirical investigation was conducted in an organisation in the South African mining industry. A nested mixed methods design was followed. In the quantitative study, a 360 degree survey was employed to assess the ratings of a purposive sample (n=54) along eight dimensions before and after the coaching. Data was analysed with descriptive and inferential analysis. Results showed statistically significant improvements on accountability, collaboration, and feedback and recognition after the coaching. The results reflected differences in 360 degree ratings according to gender, race, job level, age and geographical location. The most significant improvements were for females, Africans, management, age 51-60 years, and site 2. In the qualitative study, a semi-structured interview was employed to study four cases to investigate managers’ personal experiences and changes in attitude toward safety. Data was analysed utilising thematic analysis. The findings revealed that coaching was a positive experience and contributed to changing managers’ attitudes toward safety. The research added to the field of organisational behaviour by presenting a theoretical model that enhances the understanding of safety leadership, the development of a coaching program and providing empirical evidence that the principles of coaching and leadership development can be applied to improve safety leadership. / Industrial & Organisational Psychology / D.
