"Necessidades de familiares em unidades de terapia intensiva: análise comparativa entre hospital público e privado" / Family members’ needs at Intensive Care Units: comparative analysis between a public and a private hospital

Trata-se de um estudo comparativo, observacional e transversal, que teve como proposta analisar comparativamente as necessidades de familiares de pacientes adultos internados em UTIs de hospital público e privado, buscando evidenciar possíveis diferenças entre os dois tipos de instituição. A coleta dos dados foi realizada no período de novembro de 2004 a fevereiro de 2005. Um total de 91 familiares foram entrevistados, sendo 47 da instituição pública e 44 da particular. As necessidades dos familiares foram mensuradas quanto ao grau de importância e de satisfação, por meio do Inventário de Necessidades e Estressores de Familiares em Terapia Intensiva (INEFTI). Para análise estatística foram utilizados os testes Qui-quadrado, Mann Whitney e t-Student e a Regressão Linear Múltipla (RLM). Cerca de 89% das 43 necessidades do INEFTI foram consideradas importantes por ambos os grupos, destacando-se, entre elas, as relacionadas à Segurança e à Informação. Os familiares do hospital privado apresentaram maior grau de satisfação do que os do hospital público (p=0,002), as necessidades pouco atendidas estavam relacionadas a Conforto e Suporte. A RLM revelou que, na instituição pública, a necessidade que mais contribuiu para o escore de importância foi "saber porque determinados tratamentos foram realizados com o paciente" (b=0,578) e, na particular, "saber quais as chances de melhora do paciente" (b=0,479); o escore de satisfação, na UTI particular, foi mais influenciado pelo item "ter perguntas respondidas com franqueza" (b=0,404) e, na unidade pública, pela necessidade "ser informado a respeito de tudo que se relacione à evolução do paciente" (b=0,396). As diferenças observadas entre os familiares deste estudo, sugerem que o atendimento de suas necessidades requer intervenções direcionadas à especificidade de cada tipo de instituição. / It’s a transversal, observational comparative study, which proposed to analyze comparatively the family members’ needs of adult patients admitted in ICUs at a public and a private hospital, seeking to evidence possible differences between these two types of facilities. Data collection was made from November 2004 to February 2005. A total of 91 family members were interviewed, in which 47 were from a public facility and 44 from a private one. Family members’ needs were measured by the importance and satisfaction degree through the Inventory of Family Stressors and Needs in Intensive Care (IFSNIC) [ the Brazilian adaptation of the Critical Care Family Needs Inventory (CCFNI)]. Chi-square, Mann Whitney and t-Student and Multiple Linear Regression were used for statistic analysis. About 89% of 43 needs from IFSNIC were considered important for the two groups, especially those ones related to Safety and Information. The family members from the private hospital presented higher satisfaction score than the ones from the public hospital (p=0.002), the needs of lower satisfaction were related to Comfort and Support. Regression analysis revealed that in the public facility the need which contributed the most to Importance score was “knowing why specific procedures were made with the patient" (b=0.578) and, at the private one, "knowing the patient’s cure chance" (b=0.479); satisfaction score at the private ICU was mostly influenced by the item “having questions answered with sincerity" (b=0.404) and, at the private one, by the need of “being informed of everything that is related to the patient’s evolution" (b=0.396). The difference observed among family members in this study suggests that their need fulfillment requires interventions guided to the specificity of each type of facility.
