Analise tardia do grau de paralisia facial em pacientes operados de Schwannoma vestibular / Long-term facial palsy evaluation folowing vestibular Schwannoma surgery

Sua reversibilidade é um questionamento persistente por parte do paciente e do neurocirurgião. Objetivos: Este estudo objetiva analisar o grau de paralisia facial em pacientes operados de SV e correlacionar o tamanho do tumor com a função facial na avaliação a longo prazo destes pacientes. Método: Estudo transversal com análise seriada de 20 pacientes com SV operados no HC/UNICAMP entre Janeiro de 1999 e Outubro de 2002, pela via retrosigmóide-transmeatal. A função do nervo facial foi avaliada através da Escala de House-Brackmann no pré-operatório, pós-operatório imediato e pós-operatório tardio (mínimo de 18 meses). Os tumores foram classificados como pequenos (_.O em), médio (2.1-4.0 em) ou grande (_4.0 em). O teste t de Student foi aplicado para análise estatística. Resultados: A média de idade dos pacientes do estudo foi de 51 anos (variação de 17 a 77 anos), sendo 75% do sexo feminino. A média do tamanho do tumor foi de 3.38 em. O maior tempo de avaliação a longo prazo foi de 5 anos e 10 meses e o menor tempo foi de 1 ano e 7 meses (média de 3 anos e 10 meses). No pós-operatório imediato, 65% dos pacientes apresentaram graus variados de paralisia facial, sendo que 53% destes obtiveram melhora de pelo menos um grau de House-Brackmann na avaliação tardia. Os pacientes com melhora insatisfatória na avaliação final já apresentavam algum grau desta paralisia no período pré-operatório. Houve diferença significativa no resultado da função facial no pós-operatório tardio quando o tamanho do tumor foi considerado (p<0.05). Conclusões: A cirurgia do SV tem como uma das morbidades a paralisia facial, que pode ser definitiva ou temporária. A maioria dos pacientes (65%) apresentou melhora desta disfunção em um tempo médio de 3 anos e 10 meses. A análise do grau de paralisia facial em pacientes operados de SV permitiu o acompanhamento da evolução a longo prazo destes pacientes e a identificação do tamanho do tumor como fator associado ao prognóstico desfavorável no pós-operatório tardio / Abstract: Introduction: The evaluation of facial palsy is an important issue after vestibular schwannoma (VS) surgery due to its physical and social morbidity. Its reversibility is a . persistent questioning on the part of the patient and the neurosurgeon. Objetives: This study aimed to evaluate facial palsy in patients undergoing VS resection and to correia te tumor size and facial function in a long-term follow-up. Method: Transversal study of 20 patients with VS operated in HCIUNICAMP between January 1999 and October 2002 by the retrosigmoid approach. Facial function was evaluated by House-Brackmann Scale before, immediate and 18 months or longer after surgery. Tumors were classified as small (::2.0 cm), medium (2.1-4.0 cm) or large (>4.0 cm). The Student t test was applied for statistic analysis. ResuIts: The mean age ofpatients was 51 years (range 17 to 77 years) and 75% of the cases were females. Mean tumor size was 3.3 8 cm. The longest time of postoperative evaluation was 3 years and 10 months and the shorter one was 1 Year and 7 months (mean time of3 years and 10 months). In the immediate postoperative evaluation, 65% ofpatients presented facial palsy of different grades. Improvement of facial nerve function (at least of one grade) occurred in 53% in the long-term follow-up. Patients with unsatisfactory improvement in the final evaluation had alreagy had some degree of this palsy preoperatively. There was a statistically significant difference in facial nerve outcome in the long-term follow-up when tumor size was considered (p<0,05). Conclusions: VS surgery has as morbidity the facial palsy that can be definitive or temporary. The majority of patients had improvement this disfunction in a mean time of3 years and 10 months after VS surgery (65%). A função do VII e VIII nervos cranianos foi monitorizada durante a cirurgia e a reconstrução da abertura do conduto auditivo interno (CAI) foi realizada com retalho vascularizado de duramáter, músculo e cola de fibrina. A exérese foi completa em 99% dos casos, com mortalidade de 1,6%. Houve preservação da função do nervo facial em 85% dos casos e da audição em 40% dos pacientes, com audição prévia e tumores menores de 1,5 cm. A incidência de fístula liquórica foi de 5,8% e meningite de 2,9%. Embolia gasosa foi registrada em 3% dos casos, não associada à mortalidade. O tratamento cirúrgico dos SV utilizando-se a posição de mastóide e o acesso retrossigmóide transmeatal tem várias vantagens, com baixa morbidade e mortalidade, permitindo elevadas taxas de remoção radical, com a cura do paciente. O estudo é comparado com outras séries cirúrgicas publicadas de grupos com grande experiência na cirurgia de SV, bem como em relação aos resultados de pacientes de outros grupos que foram submetidos a tratamento radioterápico. / Abstract: To evaluate the result of the surgical treatment of Vestibular Schwannoma (VS) operated in dorsal decubitus (mastoid position). Material and methods: 240 patients with VS underwent a retrosigmoid craniotomy for tumor resection in dorsal decubitus (mastoid position). The function of 7 and 8 cranial nerves was monitored during surgery and the opened internal auditory canal (ICA) was reconstructed using a vascularized dura flap, muscle and fibrin glue. Results: Complete tumor removal was achieved in 99% of the cases, with a mortality of 1.6%. The facial nerve function was preserved in 85% of cases and hearing in 40% of the patients (with preoperative hearing) with tumors of up 1, 5 cm in diameter. The incidence of cerebrospinal fluid leak was 5.8% and meningitis 2.9%. Venous air embolism was registered in 3% of cases; it was not associated to mortality. Nas cirurgias de ressecção de tumores do ângulo pontocerebelar, a preservação do nervo facial está entre os seus principais objetivos. Diversas técnicas neurofisiológicas vêm sendo empregadas com o objetivo de predizer déficit imediato do nervo facial no pós-operatório, como a eletromiografia contínua para registro de atividade neurotônica tipo A, a estimulação direta do nervo facial no sítio tumoral e a estimulação elétrica transcraniana com captação em músculos de face. Analisamos 23 pacientes de forma prospectiva, submetidos à cranectomia retrossigmoide para ressecção de tumores em fossa posterior no período de janeiro de 2008 a março de 2010. Os pacientes foram avaliados clinicamente de acordo com a gradação de House-Brackmann para a função do nervo facial. Foram observadas a ocorrência de atividade neurotônica do tipo A, também chamada de atividade irritativa, a queda da amplitude do potencial motor do nervo facial obtido com a estimulação elétrica transcraniana e o aumento na intensidade do estímulo elétrico da estimulação elétrica transcraniana para a obtenção desse potencial. Esses dados foram relacionados à condição clínica do nervo facial no pós-operatório imediato e na última data de acompanhamento. Observamos que a queda da amplitude do nervo facial acima de 60% do seu valor inicial basal foi a variável mais sensível (89%) e com maior valor preditivo positivo (92%) para a piora clínica do nervo facial. A ocorrência de atividade irritativa isolada foi a variável menos sensível (7%) e com valor preditivo positivo de apenas 25%. O aumento na intensidade da estimulação elétrica transcraniana não mostrou significância estatística (p = 0,287). Concluímos que das variáveis estudadas, a queda na amplitude do potencial evocado motor do 7º nervo craniano acima de 60% foi a melhor na predição de piora clínica imediata do nervo facial / The preservation of facial nerve function is one of the most important goals of the cerebellopontine angle tumor resection surgeries. Valuable neurophysiologic techniques have been used to predict the facial nerve outcome, such as continuous electromyography to identify neurotonic activity type A, direct facial nerve stimulation in the vicinity of the tumor and transcranial motor evoked potential recording in face muscles. We analyzed 23 patients undergoing retrossigmoidectomy approaches for posterior fossa tumor resection between January 2008 and March 2010. Their facial nerve function was evaluated using the House-Brackmann score. We correlated the occurrence of type A neurotonic activity, facial nerve motor evoked potential amplitude obtained with transcranial electrical stimulation and increase in the electric stimulus threshold to the immediate and last follow-up facial nerve outcome. In this series, the increase in electric stimulus threshold showed no statistical significance (p = 0,287). The drop of the facial nerve motor evoked potential amplitude equal or above 60% was the most sensitive (89%) and with the highest positive predictive value (92%) in identifying poor facial nerve outcome. The occurrence of neurotonic activity type A showed to be the least sensitive (7%) and with the poorest positive predictive value (25%) of them. We conclude that among the used parameters, the drop of the final 7º nerve motor evoked potential amplitude equal or above 60% of basal recording was the best one in predicting poor facial nerve outcome

O potencial evocado motor facial (PEMF) tem-se mostrado um excelente método de monitorização do nervo facial, gerando resultados bastante confiáveis e reprodutíveis no que tange à predição da função facial pós-operatória. O critério eletrofisiológico mais utilizado até então para tanto tem sido a relação de amplitude do PEMF final-valor de base. Os objetivos deste trabalho foram avaliar as alterações intraoperatórias da amplitude e da complexidade do PEMF, correlacioná-las com o prognóstico facial no pós-operatório imediato e tardio e verificar se amplitude e complexidade constituem variáveis independentes de predição funcional. Os registros dos potenciais intraoperatórios dos músculos orbiculares do olho e da boca de 35 pacientes portadores de schwannoma vestibular (SV) foram coletados e analisados retrospectivamente de acordo com tempos cirúrgicos preestabelecidos: inicial, abertura da dura-máter, dissecação do tumor (TuDis), ressecção do tumor (TuRes) e final. No pós-operatório imediato, a função facial apresentou uma significativa correlação negativa com as relações de amplitude do PEMF durante a TuDis, a TuRes e ao final do procedimento nos músculos orbiculares do olho (p =0,003, 0,055 e 0,028, respectivamente) e da boca (p=0,002, 0,104 e 0,014, respectivamente). No último seguimento, entretanto, a correlação foi significativa apenas para o músculo orbicular da boca, durante a TuDis (p=0,005) e ao final do procedimento (p=0,102). As variações da complexidade dos potenciais alcançaram resultados mais significativos tanto no pós-operatório imediato, quanto no tardio, de forma que houve uma correlação negativa no músculo orbicular do olho apenas nas medidas finais (imediato, p=0;023; seguimento, p=0,116) e no músculo orbicular da boca durante a TuDis, a TuRes e a medida final (imediato, p=0,071, 0,000 e 0,001, respectivamente; seguimento, p=0,015, 0,001 e 0,01, respectivamente). As alterações intraoperatórias das relações de amplitude e de complexidade dos PEMFs parecem representar variáveis independentes, podendo ser utilizadas na predição da função facial pós-operatória durante cirurgias de ressecção de SV. Baseados nos resultados deste trabalho, a monitorização evento-valor de base é bastante útil, justificando mudanças imediatas da estratégia cirúrgica, com o intuito de reduzir as chances de uma lesão definitiva do nervo facial. / Facial motor evoked potential (FMEP) amplitude ratio reduction at the end of the surgery has been identified as a good predictor for postoperative facial nerve outcome. Intraoperative orbicularis oculi and oris muscles FMEP data from 35 patients undergoing surgery for VS resection were collected, then analyzed by surgical stage: initial, dural opening, tumor dissection (TuDis), tumor resection (TuRes) and final.
