Orientador: Wilza Carla Spiri / Resumo: RESUMO O objetivo desse estudo é desenvolver um ambiente de aprendizagem para discussão de temas com enfoque nos componentes que possibilitam o Gerenciamento e a Prática baseados em Evidência junto aos gerentes de Enfermagem de unidades hospitalares. O método é a pesquisaação, apoiada em encontros denominados journalclub, ou clube de leitura, meio empregado para a coleta de dados com a participação de enfermeiros em posição gerencial. Tem a finalidade de fomentar discussões e sensibilização para pesquisa científica em um processo educativo sobre gerenciamento baseado em evidências. A técnica de grupo focal foi utilizada para apreender, na perspectiva dos participantes, o significado do processo educativo e as possibilidades de transformação da prática gerencial. Os encontros foram gravados e os discursos transcritos e codificados com ajuda do NVivo9 software© QRS International. Os dados foram analisados considerando o diagrama proposto para a pesquisa-ação: Planejamento, Desenvolvimento, Descrição e Avaliação do Processo Educativo adotando-se a análise de conteúdo conforme referencial de Graneheim e Lundman. Os resultados revelaram os temas que interferem no Gerenciamento Baseado em Evidências. The method is an action research, through a strategy of meetings called Journal Club, a methodology used for data collection with a participation of nurses in managerial position, aiming to discussions and sensitization for scientific research in an educational process about evidence-based management. The focus group technique was used to understand from the perspective of the participants the meaning of the educational process and the possibilities of transformation of the managerial practice. The meetings were recorded and the speeches were transcribed and decoded with the help of NVivo 9 QRS International software. The data were analyzed considering the proposed diagram for action research: Planning, Development, Delimitation and Evaluation of the Educational Process, adopting a content analysis according to Graneheim and Lundman. The results revealed the issues that interfere with evidence-based management. Místo výzkumu a Evidence Based Practice v ošetřovatelství / The place of research and Evidence Based Practice in nursing.

Nursing is a permanently developing field of activity. Research plays an irreplaceable role in this field. We moreover more and more often see the term Evidence Based Practice nowadays. However a lot of general nurses do not know that this term also relates to the field of nursing, where it is called Evidence Based Nursing. Quantitative research was chosen for this thesis. The research was performed in South Bohemian and Central Bohemian regions among general nurses with bachelor degree. The aim of the questionnaire research was to confirm or refute the hypotheses set for this thesis. The first goal of the thesis was to find out whether general nurses with bachelor degree were willing to participate in research in nursing. The second goal was to find out whether general nurses with bachelor degree had come across the term Evidence Based Practice. The third goal was focused on finding whether general nurses with bachelor degree knew the names of databases that might be used in planning nursing care based on evidence. The fourth goal was to find out whether general nurses with bachelor degree were willing to participate on application of research results to nursing practice. The fifth goal was to find out whether general nurses with bachelor degree had met with application of research results to nursing practice. The goals of the thesis have been met. Five hypotheses were set for achievement of the above goals. H1: General nurses with bachelor degree are willing to participate in research in nursing. H2: General nurses with bachelor degree are able to define the term Evidence Based Practice. H3: General nurses with bachelor degree know the names of databases they may use in planning nursing care based on evidence. H4: General nurses with bachelor degree are willing to participate in introduction and application of research results to nursing practice. H5: General nurses with bachelor degree meet application of research results to nursing practice. The first, second and fourth hypotheses were confirmed, the third and fifth ones were not confirmed. Although the issues of Evidence Based Practice appear more and more often in literature, and we might expect nurses with tertiary education to apply the practice based on evidence in nursing, there are still general nurses who have lack of information in this sphere. Nevertheless there are a lot of general nurses with tertiary education who would like to participate in research and application of the research results to nursing practice. A proposal for a whole-life educational course focused on application of Evidence Based Practice to nursing was elaborated upon the research results. The results may also be used in hospital seminars and in education of nursing students.

Using Evidence Based Practice: The Relationship Between Work Environment, Nursing Leadership and Nurses at the Bedside

Indiana University-Purdue University Indianapolis (IUPUI) / Evidence based practice (EBP) is essential to the practice of nursing for purposes of promoting optimal patient outcomes. Research suggests that the implementation of EBP by staff nurses is problematic and influenced by beliefs, nursing leadership and the work environment. The purpose of this descriptive study was to examine variables that describe the relationship among beliefs about EBP, the work environment and nursing leadership on the EBP implementation activities of the staff nurse. The variables of interest were 1) individual staff nurse characteristics, 2) beliefs about EBP, 3) the EBP work environment and 4) nursing leadership for EBP. A descriptive, quantitative method was used. A sample of 422 Registered Nurses from two urban hospitals (one Magnet and one non-Magnet) completed an online 58 item survey that included questions related to individual belief’s about EBP, the EBP work environment and nursing leadership for EBP as well as EBP implementation activities. Education, tenure and Magnet status were not significantly related to EBP implementation activities in either the univariate or multivariate analysis. EBP beliefs had a significantly positive relationship with EBP implementation activities in both the univariate and multivariate analyses. Work environment and nursing leadership support for EBP had significant positive relationships with self-reported implementation activities in only the univariate analysis. The most surprising finding was that there were no differences between Magnet and non-Magnet work environments for EBP implementation scores, yet the Magnet hospitals reported higher means on the EBP Beliefs Scale than the non-Magnet hospital. The results of this have implications for identifying and testing strategies to influence EBP implementation activities through development of nursing leadership skills for EBP and creating a more EBP friendly work environment.
