Threats and violence, or the risk of being subjected to threats and violence in the workplace, is a major health and safety problems that exists in many professions. To be exposed to threats and violence can have serious consequences for the employee's health but also for the organization in question. The aims of this study was to investigate the prevalence of threats and violence at the Swedish Migration Board detention, and to examine whether there were health problems among employees who may be exposed to threats and violence. A web-based questionnaire was answered by 29 employees (response rate = 51 percent). The results showed that more employees had been exposed to verbal threats (66 percent), compared to physical violence (10 percent), and they showed a greater tendency to report physical violence, as opposed to verbal threats. The participants indicated that factors such as working alone, convey rejection decisions and escape attempts increased the risk of threats and violence. However, the majority of the participants reported that the preventive work, including, alarms, training and available equipment, worked fine. Further everyone reported that they received good support from colleagues when they were subjected to a threat or violent situation. Generally the employees reported that their health was good, which might partly be related to an experience that the preventions worked well at this workplace. Future research should investigate such a relationship.

Assessment of indoor air quality in an office building in South Africa

Introduction The problems associated with poor indoor air quality and the possible subsequent effects thereof on workers, as experienced by the Department of Health in the Regional office situated in the Karel Schoeman building (KSB) in Pretoria, were investigated and are discussed. The possible association between the physical indoor environmental conditions and the sick building syndrome (SBS) like symptoms as well as the seemingly low productivity and the above average rate of absenteeism are included in these discussions. To find a possible association between these poor physical environmental indoor conditions and the potential adverse affects on the health and motivation of workers, a cross sectional survey was conducted to assess the physical, biological and chemical quality of indoor air (Refer to as IAQ – Indoor Air Quality) as well as the chemical and biological quality of the drinking water. Objectives This research report documents a project to measure the environmental conditions which include CO(Carbon Monoxide), CO2(Carbon Dioxide), relative humidity( RH), temperature, air velocity, the microbial and chemical composition of the drinking water and culturable micro organisms in specific work environments within the KSB and to compare the outcome to standards for acceptable indoor working environments. The specific objectives of this study are:  To describe the environmental conditions CO, CO2, RH, temperature and air velocity exposures in specific work environments during the period 2003-2004;  To describe the microbial exposures to Legionellae (which is actually one of a group of similar diseases collectively known as Legionellosis.)  To compare measured environmental exposures to the Environmental Regulations for workplaces promulgated in terms of the Occupational Health and Safety Act 1993 (Act 85 of 1993) (OHSA), the National Institute for Occupational Safety and Health (NIOSH) standards and other national and internationally recognised standards and  To compare the microbial monitoring results including the evaluation results of the drinking water for culturable micro organisms and E.coli, to the NIOSH and other national and international recognized standards. Methods To control for confounding, samples of the drinking water as well as from a stagnant pool in the basement were taken and analysed for bacteriological and chemical compliance as well as identification of any microbes and chemicals that can contribute to SBS Environmental conditions which include CO, CO2, RH, temperature, air velocity, the microbial and chemical composition of the drinking water and culturable micro organisms including E.coli, in specific work environments were measured. Measurements were conducted in accordance with the procedures prescribed in the ASHRAE 41.6 of 1994, ACGIH (Handbook for industrial ventilation, EPA building Air quality guidelines, US Department of Labour –OHSA Technical manual),the respective manufacturers recommendations and the SABS Code 0400 of 1990 and sampling of the water in terms of the procedures prescribed by NIOH in the case of sampling for Legionellae and the Microbiological laboratory of the Rietvlei accredited water research laboratory with reference to sampling for identification of micro organisms including E.coli and the chemical composition. Appropriate recognized analytical methods were used to analyse and interpret the data generated from both the chemical and biological sampling results. Results Based on the outcome of the project results, it is clear that the air conditioning system does meet the mentioned minimum prescribed physical environmental national and international norms and standards for indoor air quality. The findings from the study indicate compliance of the physical environmental conditions to universally accepted standards and no association between those poor physical indoor environmental conditions and the sick building syndrome (SBS) like symptoms as well as the seemingly low productivity and the above average rate of absenteeism could therefore be made. The presence of coliform organisms above the norm in the air samples is however a concern. Discussion The OHSA requires employees to access the health and safety risks that hazards pose to their employees in their respective working environments and to take reasonably practicable steps towards eliminating or controlling these hazards. Like any other risks in a workplace environmental conditions need to be identified and controlled based on the principles of risk management. Conclusion The above high rate of absenteeism, as perceived by management, could not be associated with the environmental conditions covered in this project. It could however be ascribed, directly or indirectly to the following variables and assumptions which could be categorised under environmental conditions and the management thereof as well as the management of employees/workers located within the building. Environmental conditions such as substandard quality water supply, the management processes during the decommissioning of the old HVAC system and the commissioning of the replacement system, poor general housekeeping and unacceptable management principles of good quality clean working environments location of the building in the mid city centre of Pretoria and the possible contamination of indoor air by external sources bio quality could be variables contributing to the high rate of absenteeism. The seasonal occurrences of influenza and colds as well as home and external exposures could also possibly be considered as contributing factors in this regard. In addition poor general workers or people‟s management which could have resulted in low staff morale and personal motivation linked to poor supervision during a period of a very serious organizational restructuring which affected many workers could have also being considered as contributing factors to high absenteeism. Substandard ergonomics could have also played a role during this process.

Occupational allergy and asthma among table grape farm workers in the Hex-river Valley, Western Cape

International studies suggest excessive pesticide use and biological factors such as outdoor mites as important factors responsible for asthma symptoms. Studies in Korea suggest that spider mites may be responsible for allergic asthma symptoms among workers on fruit (citrus, apple, and . pear) furms. The fuming of wine and table grapes in South Africa involves about 3000 farms employing over 50,000 workers. Workers on table grape farms, in contrast to other fruit farms have not been previously investigated for occupational respiratory allergy to spider mites. Objectives • To determine the spectrum and prevalence of work-related allergy and asthma among table grape farm workers • To determine the environmental and host factors associated with work-related symptoms and allergic outcomes. Methods: A cross-sectional study was conducted on 207 workers employed on nine table grape farms in the Hex River valley of South Africa. A modified European Community Respiratory Health Survey questionnaire was used to interview workers. Skin prick tests used 8 commercial extracts of common airborne allergens (ALK) and potential occupational allergens, which included grape mould (Botrytis cinerea) and an in-house extract of spider mite, Tetranychus urticae. Specific IgE to Tetranychus urticae was determined using enzyme-linked immunosorbent assay (ELISA) and to house dust mite (Dermatophagoides Pteronyssinus) and storage mite (Lepidoglyphus Destructor) using Pharrnacia CAPRAST. Results: The mean age of the workers was 36 years with a standard deviation of 11 years.

Workplace and home exposure to respiratory sensitisers : examining the work to home pathway

Background: Contamination of the skin and clothing may lead to workers inadvertently bringing harmful materials home and exposing family members, so called para-occupational exposure. This study investigates whether workplace contamination with respiratory sensitisers such as laboratory animal allergens, flour, isocyanates and enzymes is transported from work to the home environment. Methods: 3 laboratory animal facilities, 92 bakeries, 47 car body workshops, and 2 hospitals in the Grampian region of Scotland were invited to take part in a series of linked studies to increase understanding of the ‘take-home' pathway. Control subjects were recruited from staff and students at the University of Aberdeen. Take-home exposure assessment was carried out using two techniques: surface wipe sampling and vacuum sampling in workplaces, cars and homes. Samples were also collected in the homes of control subjects. Samples from bakers were analysed for total protein, wheat flour antigen (WFA) and fungal alpha amylase (FAA) while samples from laboratory animal workers were analysed for mouse urinary protein (Mus m 1). Enzyme cleaning agents were analysed for subtilisin proteolytic activity. Similar methods using SWYPE™ aliphatic pads for isocyanate contamination assessment were conducted among car body repairers. The pads were scanned and images of SWYPE™ pads were used to estimate contamination against the quantitative assay MDHS 25/3. All analyses were done by the Health and Safety Laboratory (HSL) apart from the SWYPE™ RGB tests and gravimetric measurements. Results: A total of 13 laboratory animal workers in 3 animal facilities, 38 bakers in 5 bakeries, 13 car painters in 5 car body workshops and 20 control subjects participated in the study. Two hospitals were surveyed for enzyme exposures and 3 endoscope cleaning technicians were monitored. Evidence of take-home exposure was found for bakery workers, with potential contamination that could lead to home exposure in the car body repair and hospital workers. Higher levels of Mus m 1 contamination were detected on house door handles of non-exposed controls compared to the exposed laboratory workers (0.62 vs. 0.1 ng/wipe, p<0.001) probably due to exposure variability, might be because exposed laboratory workers being involved in a job that requires more hand washing than the general population, or suggesting widespread environmental contamination with this allergen, and these making it impossible to determine if work-home pathway exists for these workers. There was detectable WFA and FAA found on the hands, forehead, shoes, cars and homes of bakers. Compared to controls, bakers had higher median levels of WFA and FAA in house vacuum samples; the difference was statistically significant for WFA/total protein (516x10-6 vs. 164x10-6, p=0.031), FAA/total protein ratios (1.45x10-6 vs. 0.04x10-6, p<0.001) and FAA loading (1.2 pg/cm2 vs. 0.1 pg/cm2, p<0.001). Among car painters, SWYPE™ colorimetric colour changes score showed three positive SWYPE™ colour changes on skin, and three positive results on shoes of car body workshop workers. However quantitative colour analysis of the SWYPE™ pads proved ineffective for field measurements. Hand wipes of hospital workers during mid-shift and post-shift showed evidence of proteolytic activity, indicating possible spread of contamination from hands, unsatisfactory hygiene practices and the potential for take-home contamination of enzyme. Presence of contamination on footwear indicated that possible transfer of enzyme to other places including homes may occur. Conclusion: These data demonstrate the existence of pathways for take-home exposure of allergens among bakers via skin and clothing from workplaces to cars and workers' homes. The take-home pathway for laboratory animal allergens and isocyanates was not demonstrated and further investigation should be performed for enzyme cleaning agents used in healthcare settings by monitoring dermal take-home exposure with comparison to controls. Further work is needed to ascertain how widespread the take-home of respiratory sensitisers may be and the possible implications to the health of workers' families and the wider community. If parental occupation can lead to take-home exposure to respiratory asthmagens, and consequently to childhood asthma, then this represents a potentially modifiable risk factor for these cases of para-occupational asthma. There is a need for greater understanding of the take-home pathway of exposure to asthmagens and sensitisers and for a programme of education and control measures to limit the transfer of such material from the workplace to the home and wider community.
