Estudo comparativo dos efeitos clínicos e anestésicos da bupivacaína, levobupivacaína ou ropivacaína por via intratecal em ovelhas

A bupivacaína teve seu uso limitado após relato de acidentes em humanos, assim, a ropivacaína e a levobupivacaína surgiram como fármacos alternativos. O objetivo deste estudo foi avaliar e comparar os efeitos clínicos e anestésicos da bupivacaína, levobupivacaína ou ropivacaína por via intratecal em ovelhas. Seis ovelhas foram distribuídas em três grupos de igual número (n=6), os quais receberam, por via intratecal, 0,5 mg/kg de bupivacaína (grupo B), levobupivacaína (grupo L) ou ropivacaína (grupo R). As variáveis sistêmicas avaliadas foram frequências cardíaca (FC) e respiratória (f), temperatura retal (TR), pressões arteriais sistólica (PAS), média (PAM) e diastólica (PAD), pH, PaO2, PaCO2 e HCO3-. Estes parâmetros foram mensurados antes da aplicação intratecal (T0) e após, a cada 10 minutos, durante 120 minutos (T10 a T120). As variáveis anestésicas avaliadas foram os períodos de bloqueios sensitivo (BS) e motor (BM), extensão do bloqueio sensitivo (EBS) e períodos de regressão do bloqueio sensitivo (RBS) e de recuperação (PR), a cada 10 minutos, até a recuperação total dos movimentos dos membros pélvicos. Os dados, quando paramétricos, foram submetidos a ANOVA RM ou ANOVA e post hoc Tukey, ou teste t. Quando não paramétricos, utilizou-se análise de Friedmann ou Kruskal Wallis com post hoc Dunnet, ou teste de Mann-Whitney. O nível de significância utilizado foi 5%. A FC diminuiu nos grupos B e L, de T80 a T120 e em T110 e T120, respectivamente. A f reduziu no grupo L entre T30 e T60 e de T80 a T100, e no grupo R, em T50, T60 e T80. A TR aumentou no grupo B, a partir de T80. Quanto às variáveis anestésicas, o grupo B apresentou BS mais prolongado que BM. Nestas condições, a adminstração intratecal destes anestésicos locais não causa alterações sistêmicas deletérias, sendo que a bupivacaína proporcionou os maiores períodos de BM e BS. _______________________________________________________________________________________ ABSTRACT / Intrathecal anesthesia is a technique with lower cost, higher safety and easier execution when compared with general anesthesia, in sheep. Bupivacaine had its use limited after some reports of accidents in human beings; thus, ropivacaine and levobupivacaine can be used as alternative drugs. The aim of this study was to evaluate and compare the clinical and anesthetic effects of intrathecally administered bupivacaine, levobupivacaine or ropivacaine, in sheep. Six ewes were allocated in three groups (n=6) that received 0.5 mg/kg of bupivacaine (B group), ropivacaine (R group) or levobupivacaine (L group). The systemic variables evaluated were heart (HR) and respiratory (f) rates, rectal temperature (RT), systolic (SAP), mean (MAP), and diastolic (DAP) blood pressures, pH, PaO2, PaCO2, and HCO3-. These parameters were measured before intrathecal injection (T0) and then, every 10 minutes up to 120 minutes (T10 to T120). The anesthetic variables evaluated were the periods of sensory (SB) and motor (MB) blockades, sensory blockade extension (SBE), period of sensory blockade regression (SBR) and period of recovery (PR), every 10 minutes, until the full restoration of the mobility of pelvic limbs. The data were submitted to ANOVA RM or ANOVA with post-hoc Tukey, t test, Friedmann or Kruskal-Wallis with post-hoc Dunnet, or Mann-Whitney test, as appropriated. Data were considered significant when P<0.05. The HR decreased in B and L groups, from T80 to T120 and from T110 to T120, respectively. The f decreased between T30 and T60 and from T80 to T100 in L group, and in T50, T60, and T80 in R group. The RT increased in B group from T80 to T120. The SB was longer then MB in B group. In these conditions, the intrathecal application of these local anesthetics does not cause systemic deleterious changes in sheep. Furthermore, bupivacaine provided the longest MB and SB.
