Kvalita života pacienta po systémové trombolýze / Patient Quality of Live after Systematic Thrombolysis

Diploma thesis "Quality of life of patients after systemic thrombolysis" is dealing with the impact of systemic thrombolysis to the quality of life of patients affected by the ischemic stroke (IS) and treated with intravenous administration of thrombolytics. The goal is to objectively assess quality of life resulting from health improvement after systemic thrombolysis. The work consists of theoretical and practical part. The theoretical part describes ischemic stroke, its etiology, symptoms and treatment options. The second chapter is devoted to the practical part of the term "quality of life", methods that are used to measure quality of life. This chapter also deals with the consequences of IS for patient's life. In the practical part we focused on monitoring the physical and mental health in patients with IS, who were treated with intravenous systemic thrombolysis, and in patients who have not undergone this treatment. Each study group of patients had 18 patients. The results of both groups we evaluated and compared. In the survey we used a retrospective analysis of medical data and a survey using the questionnaire SF-36 on the health-related quality of life. SUMMARY The master’s degree study , called “The assessment of patient flow and posture towards the quality of services in a civic health center” has an objective to analyze the alterations of patient flow in the health center from 1999 to 2003 as well to rate their posture towards the quality of services they get. The study consists of three parts. The theoretical guesswork of patient flow and posture towards the quality of services in the civic health center is analyzed in the first part of the study. The enhanced information statement analysis of Šeškinė civic health center during the period between 1999 and 2003, including the computation of dynamical parameters and designating the trends by the specialties, was made in the second part of this study. The empirical results of the trial are presented in the third part. It comes to a conclusion that the unified quality of service rating system, which lets timely identify the problems of the service and effectively eliminate the demerit, is missing in the health care institutions; the national health care institutions do not orient towards the client demands, the sociological researches, about the satisfaction of patients with the level of services offered, aren’t performed. Kvalita života osob s revmatickými chorobami / Quality of life in rheumatoid arthritis patients

The diploma thesis deals with the quality of life with rheumatic diseases. The thesis focuses on patients suffering from rheumatoid arthritis (RA) in particular, since the rheumatic diseases are the most common chronic inflammatory rheumatoid disease spread worldwide. The aim of the diploma thesis is to explore the physical quality of life with RA of selected patients aged from 19 to 70 years. The theoretical part offers a literature review on the stated topic with focus on the quality of life with RA. The second part offers the results of a quantitative research which was conducted using a questionnaire in a rheumatologist´s office in Saint Anna Faculty Hospital in Brno and in the Hodonin Spa. The HAQ-DI index; GARS questionnaire; a visual-analogical scale rating the intensity of pain and DAS28 score assessing the activity of hospitals were used to achieve the aim. Two research hypotheses were proposed in order to conduct the research: 1. The patients who have been suffering from RA for up to five years will achieve significantly more positive score; 2. The differences between genders in the physical quality of life will be statistically more important. The hypotheses were statistically verified using the IBM SPSSS Statistics 24 software. The results of the thesis suggest, that the quality of life with RA is influenced by pain, which is more intense for women. We also claim that the patients suffering from RA might be less self-reliant, therefore, most of them are dependent on help of others or they need to use various devices. Furthermore, the results show that gender provides statistically crucial differences in the physical quality of life. The outcomes found by this thesis can be used for patients suffering from RA; to raise awareness of the disease and its impact on life.

Patient satisfaction with the services provided is extensive and actual topic. The goal of this thesis is to find out patient satisfaction with the services of general practitioners for adults in České Budějovice, Trhové Sviny a Borovany. The patient satisfaction is evaluated primarily in connection with the satisfaction of their needs during illness. The thesis is divided into theoretical and practical parts. The theoretical part consists of seven chapters. The first chapter is an introduction to the medical practice and deals with the concept of quality of health care. The sixth chapter examines satisfaction and its impact on the assessment of the quality of services by patients. The last, seventh chapter focous on the relationship of health workers and patients. The practical part deals with the research, which was conducted using quantitative research strategies, methods and questioning survey techniques. Questionnaire that we created for assessing patient satisfaction includes fourteen questions. This part sets out the objective, hypotheses, methodology, results of research, discussion and conclusion. This thesis has two hypotheses. H1: Doctors who have office hours in the afternoon are better at assessment than those who have office hours only in the morning. H2: Patients from smaller towns will be satisfied with the services of general practitioners more than patients from large cities. The research sample consisted of 453 respondents. The data were processed using Microsoft Office Excel 2010 and then were processed into graphs or tables. Hypotheses were tested using the homogeneity test, the Pivot Table. The results of this thesis can serve as an extension of the information for general practitioners for adults on the current topic.

Structural and Functional Changes in the Central Nervous System Following Cancer Therapy

Chemotherapy Induced Peripheral Neuropathy (CIPN) is known to impact negatively on patients' quality of life. It has been reported that these patients tend to have sensitivity thresholds to stimuli, such as pain and temperature, that are different from those of normal subjects. The effect of chemotherapeutic agents on the central nervous system (CNS) has been observed; however, most of the mechanisms involved are not exactly understood. A quantitative investigation into the temperature sensitivity changes in the spinal cords and brains of chemotherapy patients would provide important information in understanding the side effects of this treatment modality. In the first part of the project, the temperature perceptional changes in terms of brain activation patterns of the chemotherapy patients with CIPN are studied using brain function MRI. In the second part of the project, the structural changes of the brain and spinal cord of chemotherapy patients with CIPN are studied using diffusion tensor imaging (DTI). High b-value (b = 1500 s/mm2) and low b-value (b=650 s/mm2) settings will be use during the spinal cord DTI scans. Due to the sample size limitation, no comparison between healthy volunteers and CIPN patients can be done based on the existing temperature fMRI data. However, the developed temperature fMRI protocol shows good reliability in detecting temperature response. Based on the spinal cord DTI result using b = 1500 s/mm2, decrease in FA value has been observed. The corresponding FA values of CIPN patient and healthy volunteers are 0.28±0.10 and 0.41±0.02 , respectively. (t-test = 2.63 >2.447, p=0.05 level of significant) However, no significant difference is observed in other diffusion parameters. This results also suggests that application of high b-value setting is more suitable as it is better at detecting diffusion at microstructure. / Thesis / Doctor of Philosophy (PhD)

The impact of the rights and obligations of nurses on patient care in a critical setting in Gauteng Province

This study explored and described the impact of the rights and obligations of nurses on the delivery of quality patient care in a clinical setting. Quantitative research approach was utilised. Data was collected using a self-administered questionnaire. The research results revealed that respondents who participated in this study were aware of their rights and obligations, the effects and impact of factors influencing such rights and obligations on patient care. It was established that “patient abandonment” observed when nurses embark on a strike as their constitutionally enshrined right is not due to a lack of insight about their rights and obligations, but on how such rights and obligations are implemented. The researcher recommends that further research be undertaken to explore the causes of nurses embarking on strike actions despite their high level of knowledge concerning the impact of such actions on patient care in a clinical setting / Health Studies / M.A. (Health Studies)
