Das multiple Myelom, eine maligne Plasmazell-Dyskrasie, ist bis heute unheilbar. Eine Verbesserung des medianen Überlebens von weniger als 2 auf über 3 Jahre wurde erstmals 1996 durch eine prospektiv randomisierte Studie von Attal et al. gezeigt. Die retrospektive Analyse zweier zeitlich definierter Kohorten, Kohorte 1 (1990-1996) und Kohorte 2 (1997-2002), zur Überprüfung des Therapieerfolges am Universitätsklinikum Würzburg zeigt unter Berücksichtigung aller Patienten keinen Überlebensvorteil für Patienten der 2.Kohorte. Signifikant profitiert haben allerdings Patienten bis max. 65 Jahre der Kohorte 2, die durch die HD-Therapie ein 5-JÜL von 50% (Kohorte 2) vs.32% (Kohorte 1) erreichten, wohingegen sich für ältere Patienten keine signifikant messbaren Überlebensvorteile ergaben. / The median overall survival of patients with multiple myeloma did not exceed more than 2 years for a long time. 1996 Attal et al. published a randomized prospective trial that showed a median survival of more than 3 years in patients who went through a high-dosed chemotherapy and autologous stem cell transplantation. This retrospective analysis in two cohorts compares the overall survival rates of newly diagnosed myeloma patients between 1990-1996 (cohort 1) and 1997-2002 (cohort 2). A significant benefit in 5 year survival rates could be shown for patients younger or max. 65 years(50% C.2 vs. 32% C.1). The overall survival of patients older than 65 years, who did not receive a high-dose therapy, did not improve. The high-dose therapy with autologous stem cell retransfusion seems to be the most important factor to improve the overall survival of patients.

Das multiple Myelom ist eine Erkrankung der Plasmazellen mit einer unkontrollierten Produktion an Immunglobulinen und macht etwa 10% aller hämatologischen Erkrankungen aus. Bisher stellt die Stammzelltransplantation für die jüngeren Patienten und die Chemotherapie für die älteren Erkrankten die Standardtherapie für diese unheilbare Krankheit dar. Fast immer kommt es allerdings zu einem Rezidiv. Für eine solche Situation gibt es bisher kein festes Behandlungsschema. In den letzten Jahren ist es jedoch gelungen, neue Medikamente zu entwickeln, die sowohl effektiver als auch nebenwirkungsärmer sind. Zu diesen zählen unter anderem die sogenannten IMiDs (immun modulatory drugs), denen auch Lenalidomid (Revlimid®) angehört. In dieser klinischen, retrospektiven Arbeit werden die Wirkung und die nicht- hämatotoxischen Nebenwirkungen von Lenalidomid und Dexamethason in Kombination mit verschiedenen Zytostatika bei intensiv vortherapierten, refraktären oder rezidivierten Myelom- Patienten und einem an einer primär systemischen Amyloidose Erkrankten analysiert. Da diese Untersuchung in einer sehr frühen Entwicklungsphase des Immunmodulators Revlimid® stattfand, war die Substanz noch nicht auf dem deutschen Markt zugelassen. Deswegen wurde die Studie mit insgesamt 21 Teilnehmern im Rahmen eines sogenannten „named patient programme“ in Zusammenarbeit mit der Europäische Arzneimittelagentur (EMEA) noch vor der Zulassung der Substanz durchgeführt. Insgesamt wurde eine Ansprechrate von 42% und ein durchschnittliches EFS- Intervall von 8,3 Monaten erreicht. Weiterhin stellte die Analyse, wie schon viele Studien davor, fest, dass Lenalidomid von diesem Patientenkollektiv alles in allem gut toleriert wurde. Mit einer ausreichenden Supportivtherapie gelang es in den meisten Fällen die aufgetretenen Nebenwirkungen schnell unter Kontrolle zu bringen. Dieser Erfolg war auch in Spezialfällen, wie zum Beispiel bei Patienten mit vorbestehender Niereninsuffizienz oder einer zusätzlichen Faktor V- Leiden- Mutation, zu beobachten. Außerdem schien der Zusatz von weiteren Chemotherapeutika keine Potenzierung der Nebenwirkungen hervorzurufen. Im Gegenteil, die Grunderkrankung konnte damit noch effektiver zurückgedrängt werden. Der Patient mit der primär systemischen Amyloidose, der zuvor keine Vorbehandlungen erhalten hatte, vertrug die Behandlung trotz der ausgeprägten gastroenterologischen und kardialen Mitbeteiligung ein Jahr lang sehr gut. Ansonsten ist dieses neuere IMiD auch für ältere Patienten geeignet. Diese wiesen trotz ihrer multiplen Komorbiditäten eine gute Verträglichkeit und Wirkung auf. Auch eine Vortherapie mit Thalidomid und der häufig damit assoziierten PNP, stellt keine Kontraindikation für die Behandlung dar. Im Unterschied zur Zulassungsstudie wurde in dieser Untersuchung ein reales Patientenkolletiv untersucht. Das bedeutet, dass die Probanden beispielsweise bereits mehr als drei Vortherapien hatten und zum Teil deutlich älter als 65 Jahre waren. Trotz dieser erschwerten Bedingungen konnten abermals objektive Remissionen erreicht werden. Die zusätzliche Erfassung des sogenannten „QoL (Quality of Life)- Indexes“ wäre wünschenswert gewesen. Dadurch wäre zu evaluieren gewesen, ob neben einer objektiven Reduktion der Myelom- Parameter auch eine Symptomverbesserung eingetreten ist. / Multiple myeloma is a clonal plasma cell malignancy with an uncontrolled production of immunoglobulins that accounts for slightly more than 10% of all hematologic diseases. Currently, the standard treatment for this incurable However, in most cases the relaps occur and no guidelines exist for treatment of relapses. But during the last few years, new drugs for the treatment of myeloma have become available which are more effective and have fewer side effects compared to older drug treatments. One of these new substrates is the immune modulatory drug (IMiD) lenalidomide (revlimid®). This clinical, retrospective study analyses the efficacy and non- hematologic adverse events of lenalidomide and dexamethasone in combination with several cytostatic drugs at intensively pretreated, refractory or relapsed myeloma patients and a patient with primary systemic amyloidosis. Due to the fact that this analysis as conducted at an early stage of the development of the IMiD revlimid®, the drug was not approved in germany. Therefore, the study with a total of 21 patients was based on a so called “named patient programme” and conducted in cooperation with the European Medicines Agency (EMA). Overall, a response rate of 42% and an average event-free survival of 8,3 months was achieved. Tolerability of the drug was good, which is in line with numerous previous publications. Eventual adverse events could be controlled by suffient supportive therapy. This success was also seen in special cases like patients with a preexisting renal insuffiency or an additional factor V Leiden mutation. Moreover, adding another cytostatic drug did not obviously increase in side effects, but helped control the disease more effectively. The patient with the primary systemic amyloidosis, who was not pretreated, tolerated the treatment very well for one year despite of the gastrointestinal and cardiac involvement. Apart from that this new IMiD is also a treatment option for older patients. They showed good tolerability and effects despite of the underlying multimorbidity. Also the pretreatment with thalidomide and its often associated polyneuropathy was no contraindication for the therapy with lenalidomide. In contrast to the pivotal study, this analysis investigated a real patient group, e.g. including patients with more than three pretreatments or being older than 65 years of age. Nevertheless, also in this study group a good objective remission was achieved. An additional evaluation of the “quality of life”- index would have been desirable, helping to conclude if an objective remission of the paraprotein level coincides with a considerable improvement.

Evaluation von BAFF-Rezeptor und B-cell maturation Antigen als Targets in der Immuntherapie des Multiplen Myeloms / Evaluation of BAFF-Receptor and B cell maturation antigen as targets for Multiple Myeloma immunotherapy

Bei den Rezeptoren BAFF-R und BCMA handelt es sich um Mitglieder der TNF-Familie, die mit ihrem Liganden BAFF eine wichtige Rolle in der Homöostase und der Entwicklung der B-Zelllinie spielen. Mehrere Autoren zeigten bereits einen Zusammenhang dieses Ligand-/Rezeptorsystems mit der Proliferationskapazität und dem Überleben hämatopoetischer maligner Zellen auf. Die Expression dieser Rezeptoren als auch die Antikörperbildung gegen BCMA bei Patienten mit Multiplen Myelom (MM), die eine Lymphozytenspende erhalten hatten, führte zu der Annahme, dass BAFF-R und BCMA wichtige Zielantigene für die Entwicklung eines monoklonalen Antikörpers als Immuntherapeutikum des Multiplen Myeloms darstellen könnten. Um Ihre Eignung näher zu evaluieren, haben wir eine breit gefächerte Analyse der Expression von BAFF-R und BCMA auf hämatopoetischen und primären MM-Zellen und auf verschiedenen humanen Geweben vorgenommen. Wir konnten zeigen, dass BAFF-R auf B-und T-Lymphozyten und lymphoiden Dendritischen Zellen als auch in sehr unterschiedlicher Stärke auf primären MM-Zellen exprimiert wird. BCMA, dessen Expression bisher nur auf (malignen) Plasmazellen bekannt war, konnte nur auf primären MM-Zellen nachgewiesen werden. Nachdem BAFF-R durch die Entdeckung seiner Expression auf Hirngewebe als mögliches target für weitere Untersuchungen wegfiel, fokussierten wir uns auf das weitere Expressionsprofil von BCMA. Durch RT-PCR und immunhistochemische Untersuchungen konnte gezeigt werden, dass auch BCMA nicht selektiv von Plasmazellen oder ihrem malignen Pendant, sondern auch signifikant auf relevanten humanen Geweben wie Niere und Leber exprimiert wird. Eine targeted therapy mit diesen beiden Rezeptoren als Zielantigene ist somit weitestgehend ausgeschlossen, da eine Therapie mit einem monoklonalen Antikörper keine Selektivität für MM-Zellen besäße und somit die Gefahr einer Schädigung lebenswichtiger Organe nach sich ziehen könnte. / BAFF-R and BCMA are two of the three known binding receptors for BAFF, a TNF family member playing a critical role in the maintenance and survival of B cells and their malignant counterpart. Both receptors have been proposed as immunotherapeutic targets in B cell malignancies as antibody responses to both antigens have been detected in patients after allogeneic stem cell transplantation for B-cell malignancies. Therefore, we have evaluated the suitability of both antigens for immunotherapy by analyzing extensively both primary haematological and non-haematological tissues. Consistent with previous reports, B cells as well as plasma cells express BAFF-R. In addition, we also found expression of BAFF-R on dendritic cells and activated T-cells. In contrast, analysis of BCMA surface expression on the same panel of cells demonstrated highly restricted expression only to plasma cells. However, further investigation revealed expression of BCMA on both mRNA and protein level in different human tissues such as liver, kidney, lung and stomach. Collectively, our findings suggest that both antigens represent no safe targets for MM immunotherapy.

Identifizierung des Mutationsspektrums und Charakterisierung relevanter Mutationen im Multiplen Myelom / Identification of Mutation Spectrum and Characterization of relevant Mutations in Multiple Myeloma

Das Multiple Myelom (MM) ist eine maligne B-Zell-Erkrankung, welche von einer großen Heterogenität auf der biologischen und klinischen Ebene sowie in der Therapieantwort geprägt ist. Durch die biologische Interpretation von whole exome sequencing (WES)-Daten der Tumor- und Normalproben von fünf MM-Patienten und sechs MM-Zelllinien (ZL) sowie dem Einbezug von publizierten next generation sequencing (NGS)-Daten von 38 MM-Patienten konnten in dieser Dissertation sowohl somatische tumorrelevante Mutationen identifiziert als auch ein MM-spezifisches Signaltransduktionsnetzwerk definiert werden. Interessanterweise wurde in fast 100 % der MM-Patienten mindestens eine Mutation und in ~50 % der MM-Patienten sogar mehr als eine Mutation innerhalb dieses Netzwerkes beobachtet, was auf eine inter- und intra-individuelle Signalweg-Redundanz hinweist, die für die individuelle Therapieentscheidung möglicherweise von Bedeutung sein könnte. Außerdem konnte bestätigt werden, dass identische, positionsspezifische und genspezifische Mutationen im MM selten wiederholt auftreten. Als häufig mutierte Gene im MM konnten KRAS, NRAS, LRP1B, FAM46C, WHSC1, ALOX12B, DIS3 und PKHD1 identifiziert werden. Interessanterweise wurde die DIS3-Mutation in der MM-ZL OPM2 gemeinsam mit einer copy neutral loss of heterozygosity (CNLOH) im DIS3-Lokus detektiert, und in der MM-ZL AMO1 wurde eine noch nicht näher charakterisierte KRAS-Mutation in Exon 4 in Verbindung mit einem copy number (CN)-Zugewinn und einer erhöhten KRAS-Genexpression gefunden. DIS3 ist ein enzymatisch aktiver Teil des humanen RNA-Exosom-Komplexes und KRAS ein zentrales Protein im RTK-Signalweg, wodurch genetische Aberrationen in diesen Genen möglicherweise in der Entstehung oder Progression des MMs eine zentrale Rolle spielen. Daher wurde die gesamte coding sequence (CDS) der Gene DIS3 und KRAS an Tumorproben eines einheitlich behandelten Patientensets der DSMM-XI-Studie mit einem Amplikon-Tiefen-Sequenzierungsansatz untersucht. Das Patientenset bestand aus 81 MM-Patienten mit verfügbaren zytogenetischen und klinischen Daten. Dies ergab Aufschluss über die Verteilung der Mutationen innerhalb der Gene und dem Vorkommen der Mutationen in Haupt- und Nebenklonen des Tumors. Des Weiteren wurde die Assoziation der Mutationen mit weiteren klassischen zytogenetischen Alterationen (z.B. Deletion von Chr 13q14, t(4;14)-Translokation) untersucht und der Einfluss der Mutationen in Haupt- und Nebenklonen auf den klinischen Verlauf und die Therapieantwort bestimmt. Besonders hervorzuheben war dabei die Entdeckung von sieben neuen Mutationen sowie drei zuvor unbeschriebenen hot spot-Mutationen an den Aminosäure (AS)-Positionen p.D488, p.E665 und p.R780 in DIS3. Es wurde des Weiteren die Assoziation von DIS3-Mutationen mit einer Chr 13q14-Deletion und mit IGH-Translokationen bestätigt. Interessanterweise wurde ein niedrigeres medianes overall survival (OS) für MM-Patienten mit einer DIS3-Mutation sowie auch eine schlechtere Therapieantwort für MM-Patienten mit einer DIS3-Mutation im Nebenklon im Vergleich zum Hauptklon beobachtet. In KRAS konnten die bereits publizierten Mutationen bestätigt und keine Auswirkungen der KRAS-Mutationen in Haupt- oder Nebenklon auf den klinischen Verlauf oder die Therapieantwort erkannt werden. Erste siRNA vermittelte knockdown-Experimente von KRAS und Überexpressionsexperimente von KRAS-Wildtyp (WT) und der KRAS-Mutationen p.G12A, p.A146T und p.A146V mittels lentiviraler Transfektion zeigten eine Abhängigkeit der Phosphorylierung von MEK1/2 und ERK1/2 von dem KRAS-Mutationsstatus. Zusammenfassend liefert die vorliegende Dissertation einen detaillierten Einblick in die molekularen Strukturen des MMs, vor allem im Hinblick auf die Rolle von DIS3 und KRAS bei der Tumorentwicklung und dem klinischen Verlauf. / Multiple Myeloma (MM) is a malignant B-cell neoplasm that is characterized by a great heterogeneity on the biological and clinical level as well as by a heterogeneous response to therapeutic approaches. Biological interpretation of whole exome sequencing (WES) data of tumor and normal samples of five MM patients and six MM cell lines (CL), as well as the inclusion of published next generation sequencing (NGS) data of 38 MM patients, identified somatic tumor relevant mutations as well as a signal transduction network that was commonly affected in MM. Interestingly, almost 100 % of the MM patients harbored one mutation and ~50 % of the MM patients harbored more than one mutation in different genes of this defined network, which predicted an inter- and intra-individual pathway redundancy that might be of particular importance for individual therapeutic approaches. Furthermore, it was confirmed that the recurrent occurrence of point-specific mutations and even gene specific mutations are rare events in MM. KRAS, NRAS, LRP1B, FAM46C, WHSC1, ALOX12B, DIS3 and PKHD1 were among the most recurrently mutated genes in MM. Of note, one of the DIS3 mutations was accompanied by a copy neutral loss of heterozygosity (CNLOH) in the CL OPM2 and a so far undefined exon 4 mutation in KRAS was associated with an increased copy number (CN) and gene expression level of KRAS in the CL AMO1. DIS3 is one of the active parts of the human RNA exosome complex and KRAS is a central protein in the RTK pathway leading to the hypothesis that one or more genetic abberations within these genes may play an important role in the development and progression of MM. To further investigate this hypothesis the whole coding sequence (CDS) of DIS3 and KRAS of tumor samples of a uniquely treated patient set of the DSMM XI was sequenced using an amplicon deep sequencing approach. The study included 81 MM patients for whom cytogenetic and clinical data were available. This approach revealed information about the mutational landscape within DIS3 and KRAS and the occurrence of mutations in major and minor clones. In addition, we were able to investigate the association of the DIS3 and KRAS mutations with additional cytogenetic alterations (such as deletion of chr 13q14, translocation t(4;14)) and we studied the impact of mutations in major and minor clones on the clinical outcome and response to therapy. In particular, we discovered seven unknown mutations and three previously undescribed hot spot mutations at amino acid positions p.D488, p.E665 and p.R780 in DIS3. An association of DIS3 mutations with deletion of chr 13q14 and IGH-translocations, that was described previously, was confirmed. Interestingly, a trend towards a lower median overall survival of MM patients with a DIS3 mutation was observed. Patients with a DIS3 mutation in the minor clone also showed a worse response to therapy as compared to patients with a mutation in the major clone. Published mutations in KRAS were confirmed. Moreover, we revealed no impact of these mutations (in major or minor clones) on the clinical outcome or response to therapy. First siRNA mediated knockdown experiments on KRAS and lentivirus mediated overexpression of KRAS WT and mutated KRAS (p.G12A, p.A146T and p.A146V) showed that the phosphorylation status of MEK1/2 and ERK1/2 is dependent on the mutation status of KRAS. In summary, this present doctoral thesis allowed more detailed insights into the molecular structure of MM, specifically with regard to the role of DIS3 and KRAS in tumor development and outcome.

Die Alloimmuntherapie beim rezidivierten Multiplen Myelom: Ergebnisse einer retrospektiven Analyse / Allogeneic stem cell transplantation in relapsed multiple myeloma: results of a retrospective analysis

Die Behandlung des Multiplen Myeloms stellt eine Herausforderung sowohl für den Arzt als auch für den Patienten dar. Neue Therapeutika wie IMiDs und Antikörper ermöglichen Therapieerfolge, die bis dato nicht möglich schienen. Allerdings sind diese Erfolge regelhaft von kurzer Dauer, was den Wunsch nach einer potentiell kurativen Option noch verständlicher macht. Zudem verbleibt die Frage, wer tatsächlich von einer Alloimmuntherapie profitiert. In der vorliegenden Arbeit wurden im Rezidiv allogen transplantierte Myelom-Patienten als besondere Hochrisiko-Gruppe bezüglich der Therapieergebnisse der Alloimmuntherapie untersucht. Die retrospektive Analyse zeigte, dass nicht jeder der analysierten Patienten von der Therapie profitierte und die Ergebnisse insgesamt eher moderat waren. Innerhalb der Patientenkohorte konnten unabhängige Risikofaktoren identifiziert werden, die das Therapieergebnis nach alloSZT beeinflussten. Von der Alloimmuntherapie profitierten demnach Patienten mit folgenden Risikofaktoren nicht: Patienten mit extramedullärem Myelom, in schlechtem Allgemeinzustand, mit Hochrisiko-Zytogenetik, mit vielen Vortherapien und schlechter Remission vor alloSZT. Zusammengefasst könnte die Identifizierung von Risikofaktoren helfen, eine optimale Patientenselektion zu betreiben, um schlussendlich nur den Patienten eine Alloimmuntherapie anzubieten, die schließlich auch davon profitieren. Im Kontext des Wissenszuwachses beim Multiplen Myelom erscheint es außer Frage, dass eine genetisch derart heterogene Erkrankung optimalerweise mit einem immuntherapeutischen Konzept vorsorgt wird. Bei optimaler Patientenauswahl kann sicherlich eine Subgruppe einen erheblichen therapeutischen Benefit erreichen, was sogar der Kuration entsprechen könnte. / The treatment of multiple myeloma presents a challenge for physicians as well as for patients. New therapeutics such as IMiDs and antibodies enable therapy successes that seemed to be impossible to date. However, these successes are of short duration, which makes the desire for a potentially curative option even more understandable. In addition, the question remains who actually is affected by an alloimmune therapy. In the present study, allogeneic transplanted myeloma patients as a particular high-risk group were investigated in terms of therapeutic results of the alloimmune therapy. The retrospective analysis showed that not all of the patients analyzed were affected by the therapy and the overall results were rather moderate. Within the patient cohort, independent risk factors could be identified, which influenced the therapy result after alloSCT. Alloimmune therapy has benefited patients with the following risk factors: patients with extramedullary myeloma, in poor general condition, with high-risk cytogenetics, with many previous therapies and poor remission before alloSZT. In summary, the identification of risk factors could help to achieve an optimized patient selection in order to only offer those patients alloimmune therapy that could eventually benefit from it. In the context of the growth of knowledge in multiple myeloma, there is no doubt that a genetically such heterogeneous disease, should ideally be treated with a immunotherapeutic concept. With optimal patient selection a subgroup can certainly achieve a considerable therapeutic benefit, which could even correspond to curation.

Diese Dissertation analysiert BMP2 und BMP2-Derivate als neue therapeutische Strategien für die Behandlung des Multiplen Myeloms (MM). Das MM ist eine maligne neoplastische Erkrankung des Knochenmarks mit Plasmazellvermehrung und erhöhten Leveln an Aktivin A im Blutserum, wobei eines der Hauptsymptome das Auftreten von schmerzvollen Osteolysen ist. In den letzten Jahren rückte Aktivin-A als interessantes Target zur Behandlung des Multiplen Myeloms in den Vordergrund. Die Reduzierung der Aktivin-A Level durch decoy-Rezeptoren führte zu einer signifikanten Verbesserung der Osteolysen und einem reduzierten Proliferationsverhalten der neoplastischen B-Zellen, sowohl im Tierexperiment als auch in Studien der klinischen Phase II. Die Aktivin-A-Antagonisierung ist somit ein neuer und vielversprechender Ansatz in der Therapie des Multiplen Myeloms. Das Bone Morphogenetic Protein 2 ist aufgrund seiner molekularen und biologischen Eigenschaften ein interessantes Target für die Therapie des Multiplen Myeloms. Es ist auf molekularer Ebene ein Aktivin-A-Antagonist, besitzt aber auch osteoinduktives Potential und apoptotische bzw. anti-proliferative Eigenschaften auf neoplastische B-Zellen. Da die in der Literatur bereits beschriebenen, durch Mitglieder der TGF-β-Familie induzierten Apoptosemechanismen, noch nicht genauer untersucht waren, wurde in dieser Arbeit die BMP2-induzierte Apoptose in 10 unterschiedlichen humanen MM-Zellen analysiert. Erstens konnte dabei nachgewiesen werden, dass 7 von 10 Zelllinien nicht BMP2-responsiv waren. Eine genauere Untersuchung ergab, dass neben der Expression spezifischer BMP-Rezeptoren auch die Expression von inhibitorischen Smad-Proteinen über die BMP2-Responsivität entscheidet. Zweitens zeigte die genauere Analyse der Apoptosemechanismen, dass entgegen der in der Literatur publizierten Ergebnisse, BMP2 keine apoptotische Wirkung auf die von uns untersuchten Zelllinien hat. Mehrere verschieden durchgeführte Experimente, u.a. die Verwendung von spezifischen Inhibitoren des programmierten Zelltodes, unterstützen dieses Ergebnis und klassifizieren BMP2 als einen rein anti-proliferativen Faktor. Der letzte Teil der Arbeit befasst sich mit der Analyse von potentiellen Aktivin-A-Antagonisten in Form verschiedener BMP2- und GDF5-Derivate und inwiefern sie sich zum Einsatz in der Therapie des Multiplen Myeloms eignen. Die unterschiedlichen Eigenschaften der einzelnen Mutanten wurden in verschiedenen Zellsystemen getestet. So konnte aufgezeigt werden, dass neben einer erhöhten biologischen Aktivität in Form eines gesteigerten osteoinduktiven und anti-proliferativen Potentials auf neoplastische B-Zellen (Superagonisten), sich die verschiedenen Derivate als Super-Antagonisten zu Aktivin A eignen und damit unterschiedlichen Ansprüchen der adjuvanten Therapie im Multiplen Myelom gerecht werden. / This dissertation analyses BMP2 and BMP2 derivatives as new therapeutic agents in multiple myeloma (MM). MM is a malignant neoplastic disease of bone marrow with plasmatic cell proliferation and increased Activin-A-level in blood serum. A particular symptom of this disease is the occurrence of painful osteolysis. In the last few years Activin A has become an important target for the treatment of multiple myeloma. Reducing Activin A levels by decoy receptors led to a significant improvement in osteolysis and reduced proliferation behavior of neoplastic B cells, both in animal experiments and clinical phase II trials. Activin A antagonization is thus a new and promising approach in the treatment of multiple myeloma. Bone Morphogenetic Protein 2 is a promising prospect for the treatment of multiple myeloma due to its molecular and biological properties. It is an Activin-A-antagonist at the molecular level, but also has osteoinductive potential and apoptotic or anti-proliferative properties on neoplastic B cells as already described in literature. Since the apoptotic mechanisms, which members of the TGF-β family induced in MM-cells, have not yet been investigated in detail, the BMP2-induced apoptosis was analyzed in 10 different human MM cells. Firstly it was shown that 7 out of 10 cell lines were not responsive to BMP2. A more detailed analysis revealed that, besides the expression of specific BMP receptors, the expression of inhibitory Smad proteins determines BMP responsiveness. Secondly the more precise analysis of the apoptotic mechanisms revealed that, contrary to the results published in the literature, BMP2 has no apoptotic effect on the cell lines we have examined. Several different experiments, e.g. the use of specific inhibitors of programmed cell death, support this result and classify BMP2 as a purely anti-proliferative factor. The last part of this research deals with the analysis of potential Activin-A-antagonists in form of different BMP2 and GDF5 derivatives and how they are suitable for use in the therapy of multiple myeloma. The different properties of the individual mutants were tested in diverse cell systems. The results demonstrate that in addition to increased biological activity in form of increased osteoinductive and anti-proliferative potential on neoplastic B cells (superagonist), the various derivatives are suitable as super-antagonist to Activin A and cope with different requirements of adjuvant therapy in context of multiple myeloma.
