Até então a prática clínica das equipes limitava seu foco de atuação à demanda espontânea, sem uma postura pró-ativa, mantendo dispersas e desarticuladas, a prevenção e a cura. Tal contexto nos motivou a estudar um arranjo institucional capaz de ag1utinar saberes e práticas comuns da assistência individual e da saúde coletiva, visando garantir o princípio da integralidade. Assim, o trabalho é uma descrição da implantação de um Núcleo de Saúde Coletiva (NSC), com atribuição de agenciar as ações coletivas no nível local do sistema. O NSC tem composição multidisciplinar, contando com trabalhadores da própria Unidade e serve de referência técnica para atividades de prevenção, promoção à saúde e controle de agravos. Utilizamos a metodologia da pesquisa-ação e observação participante em uma Unidade Básica de Saúde (UBS) que atende a uma população de aproximadamente 35 mil habitantes, na região Sudoeste de Campinas. Evidenciamos a influência das ações coletivas qualificando a prática clínica. Contudo percebemos a necessidade de qualificação permanente dos trabalhadores, e referência mais direta de profissional sanitarista. A constituição e efetivação do NSC dá apoio às ações em saúde coletiva, quando articula o traba1ho entre as equipes de saúde da iàrm1i::t e contribui para a identificação das desigualdades e riscos no território. A necessidade de descentralização das ações coletivas, com retaguarda do nível regional e central, reafirmam a relevância de tal tema nos municípios e a importância de formulações que apontem para o aumento da eficácia no traba1ho em saúde / Abstract: The management of the hea1th in Campinas, since 2001, indicates the Health Family Program as an important strategy for the consolidation of the SUS, investing in a therapeutical care particularized to the individuals and their families. Until then the practica1 clinic of the teams did not foresee classification and intervention according to the different degrees of risk, limiting its performance focus to the spontaneous demand, without a pro-active position, keeping dispersed and disarticulated the prevention and the cure. Such context motivated them to study an institucional arrangement capable to agglutinate knowledge and common practices of the individual assistance and the collective hea1th, aiming at to guarantee the principIe of the completeness. Thus, the work intends to describe the implantation of a Collective Health Nucleous (CHN), with attribution of manage ring the collective actions in the local leveI of the system. The CHN has multidiscipline composition counting on workers of the own Unit and servers of reference technique for activities of prevention, hea1th promotion and hurt control We use the methodology ofthe research-action and participant comment in a Basic Unit of HeaIth (BUH), which takes care of 35.000 inbabitants nearly, in the Southwestem region of Campinas.
