Propriocepcijos lavinimo įtaka sportininkų pusiausvyrai ir funkciniams judesiams / Proprioception exercise impact for athletes balance and functional movement

Čiupkevičius T., Propriocepcijos lavinimo įtaka sportininkų pusiausvyrai ir funkciniams judesiams, magistro baigiamasis darbas – mokslinė vadovė dr. B. Zachovajevienė. Lietuvos sveikatos mokslų universitetas, Medicinos akademija, Slaugos fakultetas, Sporto Institutas – Kaunas, 2013. Darbo tikslas: Įvertinti propriocepcijos lavinimo įtaką sportininkų pusiausvyrai ir funkciniams judesiams. Tyrimo uždaviniai: 1. Įvertinti statinę ir dinaminę pusiausvyrą vyrų ir moterų grupėse prieš ir po propriocepcijos lavinimo programos taikymo. 2. Įvertinti funkcinius judesius vyrų ir moterų grupėse prieš ir po propriocepcijos lavinimo programos taikymo. 3. Palyginti vyrų ir moterų rezultatus prieš ir po propriocepcijos lavinimo programos taikymo. 4. Įvertinti sąsajas tarp statinės ir dinaminės pusiausvyros bei funkcinių judesių. Tiriamųjų kontingentas: Tiriamųjų kontingentą sudarė 24 sportininkai vyrai, kurių amžiaus vidurkis buvo 21,29±2,42 metai, ir 24 sportininkės moterys, kurių amžiaus vidurkis buvo 22,00±2,23 metai. Tiriamieji turėjo sportuoti ne trumpiau kaip 2 metus. Tyrime taikyti metodai: Statinei psusiausvyrai įvertinti: balansavimo (pusiausvyros) platforma SIGMA. Dinaminei pusiausvyrai įvertinti: modifikuotas žvaigždės nuokrypio testas. Funkciniai judesiai įvertinti 7 testai: gilus pritūpimas, žingsnis per kliūtį, įtūpstas vienoje linijoje, peties mobilumas, tiestos kojos pakėlimas, atsispaudimas ir rotacinis stabilumas. Tyrimo objektas: skirtingų kinezioteipo metodikų poveikis blauzdą tiesiančių raumenų jėgos momento variabilumui ir tikslaus judesio atkartojimui tiesiant blauzdą. Tikslas - išsiaiškinti kaip skirtingos kinezioteipavimo metodikos veikia kelio sąnario propriocepciją. Uždaviniai: 1. Ištirti ir palyginti skirtingų kinezioteipavimo metodikų poveikį blauzdą tiesiančių raumenų jėgos momento variabilumui, su ir be vaizdinės grįžtamosios informacijos, kai kelio sąnarys sulenktas 60º kampu. 2. Ištirti ir palyginti skirtingų kinezioteipavimo metodikų poveikį blauzdą tiesiančių raumenų jėgos momento variabilumui, su ir be vaizdinės grįžtamosios informacijos, kai kelio sąnarys sulenktas 90º kampu. 3. Ištirti ir palyginti skirtingų kinezioteipavimo metodikų poveikį tikslaus judesio atkartojimui, tiesiant blauzdą be vaizdinės grįžtamosios informacijos. Hipotezė. Žinant, kad raumeninės kinezioteipavimo metodikos (KT1) naudojimas labiau veikia raumeninius judesio valdymo aspektus, manome, kad judesio variabilumas sumažės ypač prie vidutinio raumens ilgio, o raištinė kinezioteipavimo metodika (KT2) veikianti sąnarinius judesio valdymo aspektus pagerins tikslaus judesio atkartojimą tiesiant blauzdą. Merenje parametara proprioceptivnog osećaja nakon dislokacije patele kod adolescentkinja / Measurement of parameters of proprioception after dislocation of patella in adolescent women

<p>I&scaron;ča&scaron;enje, luksacija ili dislokacija ča&scaron;ice (patele), predstavlja povredu kolena prilikom koje dolazi do pomeranja patele iz patelofemoralnog dela zgloba kolena. Pri ovoj povredi dolazi do o&scaron;tećenja medijalnih mekotkivnih struktura kolena koje su zadužene za pravilno pomeranje ča&scaron;ice unutar patelofemoralnog zgloba. Nakon prve povrede kod velikog borja pacijenata razvije se hronična nestabilnost patelofemoralnog zgloba sa posledičnim recidivantnim luksacijama patele. Mehanizam nastanka hronične nestabilnosti nije u potpunosti razja&scaron;njen i smatra se multifaktorijalnim, a za jedan od faktora smatra se naru&scaron;avanje neurolo&scaron;ke regulacije pokreta kolena. Propriocepcija je složen neurolo&scaron;ki mehanizam koji nam omogućava mentalno mapiranje delova tela, sprečavajući prekomerne i nepravilne pokrete koji mogu dovesti do povređivanja. Za njegovu procenu u zglobu kolena najče&scaron;će se procenjuje osećaj pozicije zgloba (JPS &ndash; eng. &bdquo;Joint Position Sense&ldquo;) kao jedne od komponenti propriocepcije. Nerazja&scaron;njeno je koji je najbolji način za njegovu procenu u pogledu izbora različitih uglova fleksije kolena i pozicija ispitanika, ali i kakav je uticaj povreda struktura kolena na njega. U istraživanju su učestvovale isključivo osobe ženskog pola (15-17 godina), svrstane u tri kategorije od po 30 ispitanika. Prvu, kontrolnu grupu, činile su zdrave ne sportistkinje; drugu, grupu sportistkinja, činile su ispitanice koje su se takmičarski bavile ko&scaron;arkom; treću, grupu pacijentkinja, činile su osobe sa luksacijom ča&scaron;ice kolena. Za merenje JPS kori&scaron;ten je VICON optički sistem (&copy;Vicon Motion Systems Ltd. UK registered no. 1801446) za analizu pokreta. Za svaku grupu meren je JPS metodom pasivnog pozicioniranja-aktivne reprodukcije sa kolenom u različitim nivoima fleksije (30&deg;, 45&deg; i 60&deg;), u uspravnom, ležećem i sedećem položaju, nakon čega su rezultati bili poređeni između grupa. Na&scaron;i rezultati su pokazali da odabir pozicije ispitanika i ugla merenja ne utiče na JPS u zdravim populacijama, kontrolnoj grupi i grupi sportiskinja, niti postoji statistički značajna razlika odgovarajućih pozicija i uglova između ovih grupa. Za grupu pacijentkinja dobili smo da postoji deficit propriocepcije, i da je sedeći stav najosetljivijim za procenu deficita osećaja pozicija zgloba kolena. U odnosu na odgovarajuće uglove kontrolne grupe i grupe sportistkinja, kod pacijentkinja postoji statistički značajno veća gre&scaron;ka traženih uglova u sedećem stavu, zbog čega je ovaj stav karakterističan za ovu grupu. Takođe smo uočili da je najveća razlika pri uglu od 45&deg;, čineći ga kritičnim uglom za grupu pacijentkinja. Ono &scaron;to je ključni zaključak je da nakon povrede kolena po tipu luksacije ča&scaron;ice postoji neurolo&scaron;ki deficit kontrole pokreta kolena, koji potencijalno može dovesti do daljeg povređivanja. Obzirom da je JPS metod kumulativne procene proprioceptivnog sistema, dalja istraživanja bi trebala da budu usmerena na određivanje mesta o&scaron;tećenja istog, čime bi se postiglo bolje razumevanje Propriocepcija zgloba kolena posle kidanja prednjeg ukrštenog ligamenta kod profesionalnih sportista / Knee joint proprioception after anterior cruciate ligament tear in professional athletes

<p>Ova studija je bila prospektivnog karaktera. Uz dopu&scaron;tenje etičkog komiteta Kliničkog centra Vojvodine istraživanje je sprovedeno na Klinici za ortopedsku hirurgiju i traumatologiju i obuhvatilo je 60 pacijenata mu&scaron;kog pola, koji su metodom slučajnog izbora na randomizirani način uključeni u ispitivanje, a koji se aktivno i profesionalno bave fudbalom, ko&scaron;arkom ili odbojkom,&nbsp; primljenih na Kliniku za ortopedsku hirurgiju radi artroskopske rekonstrukcije pokidanog prednjeg ukr&scaron;tenog ligamenta. U ispitivanje su uključeni samo oni pacijenti koji su dali potpisani informisani pristanak da učestvuju u ispitivanju, koji su zadovoljii sve kriterijume za uključivanje i koji nisu imali niti jedan kriterijum za isključivanje iz studije. Kriterijumi za uključivanje u studiju&nbsp; podrazumevali su sledeće: da je pacijent&nbsp; primljen na Kliniku za ortopedsku hirurgiju i traumatologiju Kliničkog centra Vojvodine u Novom Sadu radi operativnog lečenja prekida prednjeg ukr&scaron;tenog ligamenta kolena, da je potpisao informisani pristanak za uključivanje, da je starosne dobi od 18 do 45 godina, da se aktivno i profesionalno bave fizičkom aktivno&scaron;ću regulisanom pravilima (fudbal, odbojka, ko&scaron;arka). Kriterijumi za isključivanje pacijenata iz istraživanja bili su sledeći: prisustvo udružene povrede i spolja&scaron;njeg pobočnog ligamenta koja zahteva operativno lečenje, pojava težih op&scaron;te &ndash; hirur&scaron;kih komplikacija, želja pacijenta da bude isključen iz daljeg ispitivanja, bez obaveze da tu svoju odluku obrazloži. U prvoj fazi konstruisan je aparat, digitalni goniometar, uz pomoć kojeg je urađen eksperimentalni deo ovog ispitivanja i napravljena je baza podataka sa poljima za upis deskriptivnih i antropometrijskih parametara. Potom je uz pomoć aparata svim ispitanicima testirana sposobnost propriocepcije (JPS). Testiranje&nbsp; je obavljeno preoperativno na povređenom i nepovređenom ekstremitetu, u dva maha: odmah po zadavanju ciljnog ugla od 35&deg; i nakon 5 minuta. Druga faza je obavljena minimum 6 meseci nakon operativnog lečenja, hirur&scaron;kom, artroskopskom rekonstrukcijom pokidanog prednjeg ukr&scaron;tenog ligamenta kost-tetiva-kost tehnikom (bone-tendo-bone, BTB). Klinički pregled&nbsp; svakog pacijenta je podrazumevao proveru rezultata Lachman testa (pozitivan/negativan), Lysholm i IKDC bodovne skale za koleno i to preoperativno a potom i tokom postoperativnog kontrolnog pregleda. Po uzoru na mnoge sajtove renomiranih ortopedskih organizacija (http://www.orthopaedicscore.com/), naparavljen je on-line upitnik na Google drive-u gde su u elektronskom obliku prikupljani podaci za skale koje smo koristili. Dobijeni rezultati su za svaku skalu ponaosob potom prebacivani u Excel i dalje&nbsp; obrađivani adekvatnim statističkim alatkama u adekvatnom programu. U rezultatima je uočeno da dobijena razlika u preciznosti pozicioniranja potkolenice sa o&scaron;tećenim ligamentarnim aparatom kolena u odnosu na nepovređenu nogu pre hirur&scaron;ke rekonstrukcije prednjeg ukr&scaron;tenog ligamenta ne pokazuje statistički značajnu razliku. Međutim, postojala je statistički značajna razlika u brzini kojom se postiže zadati ugao, tj. povređena noga imala je veći intenzitet ugaonog uspona &scaron;to ukazuje na kvalitativne razlike u samom obrascu pokreta. Takođe, na ovom uzorku dobijeno je da postoji statistički značajna razlika u preciznosti pozicioniranja potkolenice sa o&scaron;tećenim ligamentarnim aparatom kolena u odnosu na nepovređenu nogu nakon hirur&scaron;ke rekonstrukcije prednjeg ukr&scaron;tenog ligamenta tj. pacijenti su nakon rekonstrukcije statistički značajno &raquo;prebacivali&laquo; zadatu vrednost od 35&deg;. Test pozicioniranja ekstremiteta za ovo ispitivanje konstruisanim aparatom se pokazao kao senzitivna i specifična dijagnostička procedura gubitka sposobnosti propriocepcije usled kidanja prednjeg ukr&scaron;tenog ligamenta.</p> / <p>In this prospective study examined group consisted&nbsp; of&nbsp; 60&nbsp; male&nbsp; patients&nbsp; with&nbsp; an anterior&nbsp; cruciate&nbsp; ligament&nbsp; tear&nbsp; and&nbsp; all participants&nbsp;&nbsp; were&nbsp;&nbsp; at&nbsp;&nbsp; the&nbsp;&nbsp; time&nbsp;&nbsp; before injury&nbsp;&nbsp; actively&nbsp;&nbsp; playing&nbsp;&nbsp; afootball, basketball&nbsp; or&nbsp; volleyball.&nbsp; The&nbsp; study&nbsp; was conducted&nbsp; at&nbsp; the&nbsp; Clinic&nbsp; for&nbsp; Orthopaedic Surgery&nbsp;&nbsp;&nbsp; and&nbsp;&nbsp;&nbsp; Traumatology&nbsp;&nbsp;&nbsp; Clinical Center&nbsp; of&nbsp; Vojvodina&nbsp; in&nbsp; Novi&nbsp; Sad&nbsp; where participants&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; were&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; admitted&nbsp;&nbsp; for arthroscopic ligament reconstruction. All participants&nbsp; were&nbsp; informed&nbsp; of&nbsp; the&nbsp; study by&nbsp;&nbsp; their&nbsp;&nbsp; clinicians&nbsp;&nbsp; and&nbsp;&nbsp; gave&nbsp;&nbsp; written consent. Next, by using the apparatus,&nbsp; all&nbsp; subjects&nbsp; were&nbsp; tested&nbsp; for proprioception&nbsp; ability&nbsp; (JPS).&nbsp; The&nbsp; tests were&nbsp; performed&nbsp; preoperatively&nbsp; on&nbsp; the injured&nbsp; and&nbsp; the&nbsp; uninjured&nbsp; limb&nbsp; in&nbsp; two instances:&nbsp; straight&nbsp; after&nbsp; determining&nbsp; the target&nbsp;&nbsp; angle&nbsp; of&nbsp;&nbsp; 35&deg;&nbsp; and&nbsp;&nbsp; 5&nbsp;&nbsp; minutes afterwards.&nbsp;&nbsp;&nbsp; The&nbsp;&nbsp;&nbsp; second&nbsp;&nbsp;&nbsp; stage&nbsp;&nbsp;&nbsp; was performed&nbsp; postoperatively&nbsp; in&nbsp; the&nbsp; same maner,&nbsp;&nbsp; following&nbsp;&nbsp; a&nbsp;&nbsp; minimal&nbsp;&nbsp; 6-month period&nbsp;&nbsp;&nbsp; after&nbsp;&nbsp;&nbsp; a&nbsp;&nbsp;&nbsp; surgical&nbsp;&nbsp;&nbsp; arthroscopic reconstruction&nbsp;&nbsp;&nbsp; of&nbsp;&nbsp;&nbsp; the&nbsp;&nbsp;&nbsp; torn&nbsp;&nbsp;&nbsp; anterior cruciate&nbsp; ligament&nbsp; by&nbsp; bone-tendon-bone (BTB) technique.
