Molecular Brain Adaptations to Ethanol: Role of Glycogen Synthase Kinase-3 Beta in the Transition to Excessive Consumption

Alcoholism is a complex neuropsychiatric disease that is characterized by compulsive alcohol use and intensifying cravings and withdrawals, often culminating in physiologic dependency. Fundamental alterations in brain chemistry underlie the transition from initial ethanol exposure to repetitive excessive use. Key mediators of this adaptation include changes in gene expression and signal transduction. Here we investigated gene expression pathways in prefrontal cortex and nucleus accumbens following acute or chronic ethanol treatment, to identify genes with potentially conserved involvement in the long-term response of the corticolimbic system to repeated ethanol exposure. We investigated Gsk3b, which encodes glycogen synthase kinase 3-beta, as a highly ethanol responsive gene associated with risk for long-term maladaptive responses to ethanol. On the level of the protein, we found that GSK3B and to a lesser extent the GSK3A isoform showed robust increases in inhibitory phosphorylation following acute ethanol. This inhibition may underlie aspects of the behavioral response to acute ethanol, as pre-treatment with a GSK3B inhibitor (tideglusib) augmented ethanol’s locomotor effects. Following long term ethanol exposure, we re-tested GSK3B phosphorylation and found that its ethanol response is blunted, consistent with molecular tolerance as a corollary to increased consumption. As the prefrontal cortex (PFC) plays a vital role in the reward pathway via its glutamatergic projections to the nucleus accumbens, we investigated the role of the Gsk3b gene specifically in PFC and in glutamatergic neurons. Overexpression of Gsk3b in the PFC robustly increased ethanol consumption, while deletion in Camk2a-positive neurons significantly attenuated ethanol consumption. Pharmacologic antagonism of GSK3B also decreased drinking in a model of binge-like consumption. Collectively this data implicates GSK3B as a mediator of excessive ethanol intake via its kinase activity, wherein inhibition of the kinase via phosphorylation exerts a protective effect in the context of acute ethanol, but desensitizes with repeated exposure.

An Investigation of the Influence of Cooperating Teachers on the Educational Goal Ranking Behavior of Student Teachers

The purpose of the study was to identify the effects of the influence of cooperating teachers on how student teachers prioritize particular goals of education. This was accomplished by administering a predetermined list of eighteen educational goals to a group of student teachers prior and subsequent to their quarter-long student teaching experience, and to their respective cooperating teachers during their student teaching quarter. The list enabled the teacher groups to rank the goals in order of priority. In this way the cooperating teachers' goal rankings were compared to both the student teachers' pre and post student teaching goal rankings. Twenty-three student teachers and their respective cooperating teachers in secondary and special education served as subjects. The Phi Delta Kappa Goal Setting Instrument was used as the goals list in the study. The questions explored were: 1) are there differences between the relative importance as signed to selected educational goals by student teachers prior to the student teaching experience and the relative importance assigned to the same goals by the cooperating teachers; 2) are there differences in the relative importance assigned to selected educational goals by student teachers before their student teaching experience as compared to their assigned rankings after their student teaching experience; and 3) is there a relationship between any changes in the relative importance assigned by the student teachers prior and subsequent to the student teaching experience and the relative importance assigned by the cooperating teachers. To test the hypotheses under investigation, eighteen one-way analyses of variance with repeated measures were computed. Significant F ratios were found for two of the eighteen goals; the remaining F ratios were not statistically significant. The results suggest some tentative support for student teachers' goal prioritizations of two goals changing after the student teaching experience. Some tentative support was also suggested on these two goals for the student teachers' goal prioritizations changing after the student teaching experience to become more similar to the cooperating teacher' s goal prioritizations. However, the lack of significant change in sixteen of the eighteen goals more strongly suggested that the influence of a) the experience of the student teaching activity and b) the cooperating teachers' own goals prioritization biases upon the student teachers did not markedly affect student teacher goal prioritization behavior. The possibilities that the teacher groups had initial general agreement on goal priorities, that the goals may represent stable educational values, and that instrumentation concerns may have affected the results were then discussed.

Relationship of Cognitive Reserve and Decline in Alzheimer's Disease: A Population Study

According to the theory of cognitive reserve, cognitively enriching aspects of life experience (e.g., education, occupation, and leisure activity) foster the development of more efficient neural networks and cognitive strategies, enabling individuals to cope more effectively with the pathology of dementia. Using extant data from a population-based study, we examined: (1) the effect of reserve accrued through middle life on course of neuropsychological decline; and (2) the role of ongoing engagement in mentally stimulating leisure activities in rate of general cognitive and functional deterioration. In linear mixed models, level of occupational attainment did not affect rate of cognitive or functional decline, although women were found to undergo more rapid deterioration in cognitive ability. Occupational skill area was associated with trajectory of decline in several neuropsychological domains. Specifically, vocations emphasizing practical, hands-on skills were associated with slower deterioration in auditory-verbal and visual memory, as well as visuospatial and constructional abilities. Teaching and helping professions, in contrast, were associated with more rapid decline in memory and executive functioning. Increased engagement in cognitive leisure activities through late life was associated with slower deterioration in general cognitive ability in mild dementia, but its effects were no longer evident in more severe AD. An understanding of how rate of decline intersects with patients' past histories and efforts to maintain and enhance cognitive capacity will enable clinicians to target areas for cognitive training and rehabilitative therapy.

Prediction of Academic Success in the College of Forestry

The College of Forest, Range and Wildlife Management (hereafter referred to as the College of Forestry) at Utah State University has been concerned for some time with the problem of selecting from the applicants to their school the ones whom they think will be able to complete the requirements for graduation. The large number of freshman students placed on probation at the end of the fall quarter and of succeeding quarters each year fives some idea of the magnitude of the problem. It is not unusual for 40 or 50 percent of the entering freshmen to be in this category. The faculty of the College of Forestry is interested in the students, not only as potential professional foresters, but also as individuals. They recognize the fact that failure is not only emotionally upsetting but also expensive, especially for students who have come, as many of them have, from out of state. The loss of time and money on the part of the students, as well as the effort expended by the professors, is a waste of human resources. Because of their desire to give each student as much opportunity as possible to succeed, the College of Forestry has devised a very elaborate probation program. If the student fails to maintain a 2.0 (C average) for two consecutive quarters, he is asked to sign a statement indicating that if he is unable to get satisfactory grades the next quarter he will withdraw from school and not register again for two quarters. This provides the student with a "cooling off period'' during which he can re-evaluate his goals in terms of his past performance in school.

The Effects of Restraint on Hallucinatory Behavior Under Conditions of Perceptual Deprivation

The purpose of this study was to determine if physical restraint is a major factor in the elicitation of the hallucination phenomena associated with perceptual (or sensory) deprivation studies. Experimental subjects were exposed to three one-hour sessions of perceptual deprivation one week apart, with physical restraint being used during the second session. A group of control subjects was used to determine t he effects of three unrestrained sessions of perceptual deprivation. No significant differences were found between sessions for the experimental group in terms of number of reports or the cumulative duration of the reports. There was also no difference found between the two groups for any session. The data and experiences of the individual subjects are discussed at length with particular attention to the effects of the restraint procedure on indications of stress or anxiety levels (expressed in terms of time estimation and subjective reports). Recommendations for the use of more objective measures of anxiety such as biofeedback and electroencephalographic equipment are made as well as better defining procedures for the measurement of the hallucination phenomena. It is further suggested that the group design is not suited to the study of hallucinations due to the great degree of variability. A new procedure for the unobtrusive measure of the duration of the hallucinations is used successfully in this study and may prove to be a useful tool for future studies in this area.

Changing Delay Discounting: Identification and Evaluation of Ecologically Valid Methods for Reducing Impulsive Choice

Impulsivity takes many forms, one of which is termed impulsive choice. Impulsive choice entails preference for an outcome due to its immediacy relative to more optimal outcomes that take longer to come to fruition. For example, one may wish to have another serving of a decadent dessert after dinner—but doing so may undermine a longer-term goal of improved health and nutrition. If having the extra serving becomes a habit, the consequences of that choice compound and may lead to, for example, obesity. A high degree of impulsive choice such as this is indeed related to issues such as obesity, drug addictions (e.g., alcohol, opiates), and more; it may also cause these conditions. Because impulsive choice may lead to the development of poor health conditions, being able to reduce impulsive choice may reduce the occurrence of these conditions and/or help treat them. To date, a variety of studies have been conducted to examine ways to reduce impulsive choice, but it was unclear what methods may be most useful for clinical use in humans. Thus, the first portion of the enclosed research was a literature review in which successful methods for reducing impulsive choice were identified. A particular intervention called Episodic Future Thinking (EFT), which entails vivid imagination of one’s future, was one of the most promising found. However, it was unclear if its positive effects on impulsive choice were due to EFT itself or a placebo-like effect, which can arise from being able to guess the purpose of the intervention. The remaining portions of this dissertation focused on determining whether people are able to identify the purpose of EFT, and subsequently, if this awareness accounts for the positive effects of EFT on impulsive choice. Across three experiments, we demonstrated that naïve individuals are able to figure out the purpose of EFT (Experiments 1a and 1b), but that being aware of its purpose is unrelated to its positive effects (Experiment 3). These findings give hope that this intervention could be clinically useful, but it did appear that its benefits did not generalize well to novel settings (Experiment 2). Overall, the results of the research showed that EFT produces genuine changes in impulsive choice, but that further research will need to be conducted to understand why it works, and ultimately, how its generalizability can be increased.

Li Fem Anpil: The Lived Experience of Haitian Immigrant Women with Postpartum Depression

Purpose: The purpose of this interpretive phenomenological study is to explore the lived experience of Haitian immigrant women living in Massachusetts with PPD. Specific Aims: Aim 1: To explore the lived experience of PPD among Haitian immigrant women. Aim 2: To explore how the experience of being Haitian influences Haitian immigrant women in their response to PPD. Framework: Leininger's Theory of Cultural Care (1988) guided the phenomenological approach and data collection. The Transcultural Care Decision & Action model contains three predictive modes for guiding nursing care judgments, decisions, or actions to provide care. Design: Interpretive phenomenology guided this qualitative study. Individual face-to-face interviews were conducted. The data from each interview were transcribed into a written document and analyzed using the Crist and Tanner five-step process. Results: This study yielded two themes; each theme has three dimensions. The first theme is “Feeling Disconnected” with three dimensions: (a) lack of support; (b) partner conflict; and, (c) nostalgia of Haiti. The second theme is “Feeling Reconnected” with three dimensions: (a) realization of needed help; (b) spirituality; and, (c) resilience. Conclusion: This study provides insight into the lived experience of Haitian women with PPD. Awareness of Haitian women’s actual experiences with PPD will help health care providers to identify and provide culturally appropriate care to this population.

CROSS-VALIDATION OF THE VALIDITY-10 SUBSCALE OF THE NEUROBEHAVIORAL SYMPTOM INVENTORY

The present study is a cross-validation of the Validity-10 embedded symptom validity indicator from the Neurobehavioral Symptom Inventory (NSI) for the detection of questionable response validity during evaluation for mild traumatic brain injury (TBI). The sample and data derived from a three-site Veterans Affairs (VA) parent study to validate the TBI Clinical Reminder, a routine set of questions asked of all recently returned veterans at VA facilities to screen for history of TBI. In the parent study, veterans recently returned from Iraq and Afghanistan underwent an evaluation for TBI with a physician and completed an assessment battery including neuropsychological tests of cognitive performance and indicators of symptom and performance validity, psychiatric assessment measures, a structured interview for post-traumatic stress disorder (PTSD), and various behavioral health questionnaires. The present study estimated the test operating characteristics of Validity-10, using NSI results gathered during the physician evaluation to compute Validity-10 scores, and using results on several other measures of symptom and performance validity from the assessment battery as criteria for questionable response validity. Only individuals who had positive screen results for TBI on the TBI Clinical Reminder prior to full evaluation were included in the present sample. Sensitivity of Validity-10 to questionable validity was moderately high (.60 - .70) to excellent (.90 - 1.00) at high levels of specificity (> .80). Effects of different base rates of and different criteria for questionable validity on the utility of Validity-10 were explored as well. Chi-square analyses to determine the effect of PTSD symptoms on the utility of Validity-10 demonstrated overall classification accuracy in general, and false positive rate in particular, were relatively poorer when used with individuals who reported significant PTSD symptoms. Overall, these findings support the use of Validity-10 (at cut score Validity-10 ≥ 19) to identify those veterans being evaluation for mild TBI in the VA system who should be referred for comprehensive secondary evaluation by a clinical neuropsychologist using multiple forms of symptom and performance validity testing. Further studies of the effects of PTSD symptoms on the accuracy of Validity-10 for this purpose are recommended.

Developmental Trajectories of Alcohol Use and Alcohol Use Disorder

Alcohol use (AU) and alcohol use disorder (AUD) are leading causes of morbidity, premature death, and economic burden. They are also associated with high levels of disability and many other negative outcomes. Twin and family studies have consistently shown that AU and AUD are complex traits influenced by both genetic and environmental factors. Although much has been learned about the genetic and environmental etiology of AU and AUD, significant gaps remain. These include the need for a more comprehensive understanding of the roles of risk and protective factors, and the nature of developmental trajectories underpinning the progression from AU to AUD. The aims of this dissertation are: (1) to examine the roles of resilience and personality disorders in the etiology of AU and AUD; (2) to investigate the nature of longitudinal changes in genetic and environmental risk factors responsible for individual differences in AU; and (3) to determine the moderating roles of key environmental risk factors on the impact of aggregate molecular, or polygenic, risk for AU during adolescence. Using both biometrical behavioral genetic and molecular genetic methodologies, five key findings were observed: (1) Resilience is strongly associated with a reduction in risk for AUD, and this relationship appears to be the result of overlapping genetic and shared environmental influences; (2) Borderline and antisocial personality disorders are the strongest and most stable personality pathology predictors of the phenotypic and genotypic liability to AU and AUD across time; (3) Genetic influences on the development of AUD from early adulthood to mid-adulthood are dynamic, whereby two sets of genetic risk factors contribute to AUD risk; (4) The specific genetic influences on AU follow an unfolding pattern of growth over time, whereas unique environmental risk factors are consistent with an accumulation of environmental impacts and risks across time; and (5) High peer group deviance and low parental monitoring are associated with increased AU, while early parental monitoring moderates the polygenic risk for AU at age 20. The implications of these results with regard to prevention and intervention efforts are discussed.
